[Case of postoperative subdural tension pneumocephalus].

In this paper, a case with subdural tension pneumocephalus secondary to bifrontal craniotomy and VP shunt for ruptured Acom aneurysm is reported. In this patient, the mechanisms for entry of air into the subdural space and producing mass effect (Tension pneumocephalus) seem to be one way valve mechanism and negative pressure due to excess of CSF drainage owing to shunting system. Only 29 reported cases of tension pneumocephalus following surgery were found in an extensive review of the literature. We discussed about the clinical symptoms and signs, therapy, and especially about the mechanisms leading to this condition, and prevention for it.

[Unilateral visual field defect due to optic nerve compression by non-sclerotic internal carotid and ophthalmic arteries. A case report].

A case of unilateral visual field defect due to optic nerve compression by non-sclerotic internal carotid and ophthalmic arteries was reported. A 14-year-old boy was admitted to our department because of constricted visual field of the right eye. The right ophthalmic artery originated from more distal portion of the internal carotid artery than usual on angiogram. CT, PEG and other roentgenologic examinations revealed no abnormalities. But the possibility of para-sellar tumor still remained, so the right optic nerve was explored by a right frontotemporal approach. The optic nerve was found to be compressed not by any sort of mass lesion but by the non-sclerotic internal carotid and ophthalmic arteries against the rim of the optic canal. Then a microsurgical unroofing of the optic canal was performed to relieve the nerve from the compression. The decompressed upper surface of the nerve was found to be discolored yellow-white. Unfortunately, no improvement in the visual field was obtained after operation. Visual field defect due to vascular compression of the optic nerve has been discussed in only several reports. There have been only 8 cases of surgically treated unilateral visual field defect due to vascular compression. Most of them were the results of compression by the sclerotic or fusiformly enlarged internal carotid arteries and were found in the patients of middle age. [Parathyroid carcinoma--a case report and a review of the Japanese literature].

The patient, a 42-year-old female, presented with bone pain and a cervical tumor. The tumor was operated at another hospital under a diagnosis of thyroid adenoma and the bone pain subsequently disappeared. Three years later, a cervical tumor was palpated just under the operative scar; the patient experienced nausea, vomiting. The tumor and the left thyroid lobe were removed and modified radical neck dissection was performed. The histological diagnosis of the resected specimen was parathyroid carcinoma.

[Functional saving in the treatment of cervical cancer].

The radicality of Okabayashi's radical hysterectomy for treatment of cervical cancer has been well-known. However, its radicality also brought up the disturbance of fertility, endocrine and sexual function as well as the dysfunction of micturition and defecation. As early cervical cancer cases have increased yearly, and social rehabilitation became more necessary nowadays, functional saving is important and must be considered as the responsibility of the oncologists. This report denotes the preservation of ovarian function, sexual rehabilitation as well as the fertility in the treatment of cervical cancer.

A new feature of angiotensin-converting enzyme in the brain: hydrolysis of substance P.

Highly purified rat brain angiotensin-converting enzyme hydrolyzes substance P which contains a C-terminal amino acid with an amidated carboxyl group. The hydrolysis of substance P verified by amino-group fluorometry and by high-performance liquid chromatography is inhibited by captopril, but not by phosphoramidon. The presence of sodium chloride is essential for the hydrolysis. The analyses of cleavage products indicate that the enzyme hydrolyzes substance P between Phe7-Phe8 and Phe8-Gly9 by an endopeptidase action, followed by successive release of dipeptides by a dipeptidyl carboxypeptidase action.

Experimental pancreas imaging study with 13N-glutamate using positron computer tomography.

Accumulation of 13N-L-glutamate in the pancreas was experimentally evaluated by tissue distribution studies and by positron computer tomography imaging studies. The pancreas and salivary gland of Donryu rats exhibited a high rate of 13N-glutamate uptake. The percent administered dose/g of pancreas was 3.8 at 20 min, 4.4 at 30 min after injection, and the pancreas to liver ratio was 1.9 and 2.5, respectively. The same results were obtained with Golden hamsters. The role of the feeding protocols was studied to optimize the technique. The fasting group showed a significantly higher uptake in the pancreas than in the free feeding group. The uptake in the fasting followed by a high protein meal group was higher than fasting only group, but the difference was not significant. Positron emission tomography of a miniature-pig with 13N-glutamate showed a clear image of the pancreas with the highest activity. It was concluded that 13N-glutamate was quite useful in positron imaging of the pancreas.

Measurement of the area of the anterior horn of the right lateral ventricle for the diagnosis of brain atrophy by CT. Correlation with several ventricular indices.

The area of the anterior horn of the right lateral ventricle and five ventricular indices: (1) maximum width of the anterior horns, (2) minimum width of the anterior horns, (3) sum of these maximum and minimum widths, (4) ventricular index, and (5) cella media index were measured from the CT scans of 198 cases considered to be "normal". The area of the right anterior horn and indices 1-4 were measured on the slice which most clearly showed Monro's foramina and index 5 was measured on the slice 2 cm above. The size of the ventricular system increased steadily with age. The sum of the maximum and minimum widths of the anterior horns was most highly correlated with the area of the right anterior horn. Using this ventricular index, we graded brain "atrophy" in each age group.

Studies on 18F-labeled pyrimidines. Tumor uptakes of 18F-5-fluorouracil, 18F-5-fluorouridine, and 18F-5-fluorodeoxyuridine in animals.

Three 18F-labeled pyrimidines, 18F-5-fluorouridine (18F-5-FUR), 18F-5-fluorouracil (18F-5-FU), and 18F-5-fluorodeoxyuridine (18F-5-FdUR), were examined regarding tissue distribution and tumor uptake in ascitic hepatoma AH109A-bearing rats. The differential absorption ratios of tumors of 18F-5-FUR, 18F-5-FU, and 18F-5-FdUR were 0.75 +/- 0.21, 0.92 +/- 0.15, and 0.96 +/- 0.24 at 30 min, and 0.37 +/- 0.09, 0.64 +/- 0.34, and 0.60 +/- 0.17 at 120 min, respectively. The tumor-to-organ ratios obtained with three radiopharmaceuticals, especially with blood, heart, lung, muscle, and brain were high and these ratios increased with time. The tumor-to-organ ratios obtained with 18F-5-FdUR were always 1.3-4 times higher than 18F-5-FU and 18F-5-FUR. We concluded that 18F-5-FdUR was a suitable radiopharmaceutical for tumor imaging. Positron emission tomography of a rabbit tumor located on the chest with 18F-5-FdUR clearly showed the tumor within 1 h.

Denaturant-gradient chromatography for the study of protein denaturation: principle and procedure.

The particle volume of a protein which represents the expansion of the molecular chain is a useful probe to measure its conformation change in solution. To study the details of the conformation transition, we will report a novel device to trace the particle-volume change against increasing denaturant concentration. According to the denaturant gradient in the eluant of high-performance gel-permeation chromatography, information concerning the elution volume of proteins in the course of denaturation can be successively accessed. When a protein is injected into a denaturant solution, its effective molecular volume increases by the unfolding of a native compact structure, and the elution peak goes forward to separate from that of the native protein. Typical elution patterns were computer-simulated in an extended algorithm of the plate theory. The kinetic feature of denaturation and the effect of the denaturant gradient are discussed. The denaturation schemes of lysozyme, cytochrome c, and phosphoglycerate kinase were studied.

Difference between cholic acid and chenodeoxycholic acid in dependence upon cholesterol of hepatic and plasmatic sources as the precursor in rats.

Some difference in functional pool of cholesterol acting as the precursor of bile acids is pointed out between cholic acid and chenodeoxycholic acid. In order to elucidate this problem further, some experiments were performed with rats equilibrated with [7(n)-3H, 4-(14)C] cholesterol by subcutaneous implantation. The bile duct was cannulated in one series of experiments and ligated in another. After the operation 14C-specific radioactivity of serum cholesterol fell, but reached practically a new equilibrium within three days. 14C-Specific radioactivity of serum cholesterol as well as of biliary bile acids in bile-fistula rats and urinary bile acids in bile duct-ligated rats was determined during a three days-period in the new equilibrated state. The results were as follows: (1) 14C-Specific radioactivity of cholic acid and chenodeoxycholic acid in bile was lower than that of serum cholesterol, and 14C-specific radioactivity of cholic acid was clearly lower than that of chenodeoxycholic acid. (2) 14C-Specific radioactivity of cholic acid and beta-muricholic acid in urine was lower than that of serum cholesterol, and 14C-specific radioactivity of cholic acid was lower than that of beta-muricholic acid. (3) Biliary as well as urinary beta-muricholic acid lost tritium label at 7-position entirely during the course of formation from [7(n)-3H, 4-(14)C]cholesterol.

Antigenic structure of adenylate kinase from porcine skeletal muscle--II. Immunochemical cross-reactivity of fragments from adenylate kinase.

Specific antibody to a fragment [CBb(2-56)] of porcine muscle adenylate kinase was purified from goat antiserum against adenylate kinase on an immunoadsorbent column. This anti-CBb antibody cross-reacted in solid phase radioimmunoassay with two other CNBr-fragments of adenylate kinase, CBfN(81-125) and CBfC(126-194). This cross-reactivity explained their high inhibition activities in quantitative precipitin reaction between adenylate kinase and goat antiserum shown in the previous work. Cysteinyl residues of the enzyme (positions 25 and 187) were S-cyanylated with 2-nitro-5-thiocyanobenzoic acid and the enzyme was cleaved at these residues. Fragment 1-24 thus obtained was purified. The fragment 1-24, composing the N-terminal half of CBb(2-56), accounted for full activity of CBb to anti-CBb in radioimmunoassay. Hence antigenic region(s) of CBb(2-56) exist in its N-terminal half, 2-24, and this determinant(s) may be closely related to the cross-reactivity among CB-fragments. CBfN also bound to the antibody fraction which had not been adsorbed to CBb-Sepharose (non-anti-CBb). CBfN carried additional antigenic regions. In conclusion, we propose that the antigenic reactive region(s) of adenylate kinase responsible for the cross-reactivity of the CB-fragments are as follows: -Glu-Glu-Lys-Leu-Lys-Lys- (2-7), -Glu-Glu-Phe-Lys-Arg-Lys- (103-108), -Glu-Glu-Thr-Ile-Lys-Lys- (143-148).

Theoretical and experimental studies on zone-interference chromatography as a new method for determining macromolecular kinetic constants.

Zone-interference chromatography is a new method for studying macromolecular interactions (S. Endo and A. Wada, Anal. Biochem. 124 (1982) 372). This method is a new style of affinity chromatography which requires no preparation of affinity-column materials but utilizes the velocity difference in a column between interacting molecular species. Using the stochastic theory on the behavior of solute molecules, both the association and the dissociation rate constants can be analytically obtained from the degree of deformation of elution patterns, i.e., the change of the first and second moments. In order to verify the present theory, computer simulation of elution profiles by the extended plate theory and a binding experiment between glutamate dehydrogenase and ADP have been carried out.

Postural effects on behavioral states of newborn infants--a sleep polygraphic study.

Physiological and behavioral correlates of neonates in the supine and prone positions were examined. Polygrams were recorded in 10 newborn infants in the prone and supine positions. Newborn infants slept more in the prone position than in the supine, and quiet sleep was significantly more in the prone position. Gross movement, jerky movement and twitch movement were less in the prone than the supine position. There was no difference in localized movement or tremor-like movement in the two positions. Respiration was more regular in the prone than the supine position. Sleep apnea (greater than or equal to 6 seconds) was less in the prone position. The pulse rate during quiet sleep was higher in the prone position.
