[Serum thyrotropin binding inhibitory immunoglobulin (TBII) activity before and after subtotal thyroidectomy in Grave's disease].

Serum level of TBII activity was measured in 81 patients with Graves' disease by the Smith's radioreceptor assay method and the activity more than +15% was designated as "positive". All the patients had been under medical treatment with antithyroid drugs and 71 of them underwent subtotal thyroidectomy. The negative TBII activity was seen in 4 (22%) of 18 patients in whom the duration of antithyroid drug treatment was less than 2 years and in 6 (60%) of 10 patients whose medical therapy was for more than 2 years. TBII activity was not correlated with serum MCHA-, TGHA-titers and circulating lymphocyte counts, but correlated well with the degree of lymphoid cell infiltration in the resected thyroid tissue. All the 7 patients was negative had never relapsed, but 2 of them remained at the euthyroid state 6 to 24 months after operation, 3 hypothyroid and 2 at a borderline between the normal and hypothyroid. The postoperative states of the 13 patients who showed positive TBII activity before operation were euthyroid in 7, hypothyroid in 2, borderline hypothyroid in 4 and relapse in one. A gradual decline of TBII activity following surgery was generally associated with postoperative euthyroidism. It is concluded that determination of TBII activity is of help to estimate the thyroid functional state in Graves' disease.

[Nuclear magnetic resonance investigations of DMBA-induced rat breast cancers--the relationship between tumor doubling times and water proton spin-lattice relaxation times].

Our studies of human neoplastic tissues had revealed that the T1 values of many malignant tissues had a tendency to be longer than those of normal ones. But the T1 values did not always reflect the grade of malignancy on account of the variety of their histologies. Therefore, by using DMBA-induced rat breast cancer, we investigated the relation between the degree of malignancy and the T1 values, and the following results were obtained. 1) The growth rate of the cancers had a correlation with their T1 values, and 2) the water content of the cancers correlated with their T1 values, but 3) there was no correlation between their growth rate and their water content.

[Studies on penetration of antibiotics to gallbladder tissue and bile, its surgical significance. Mainly with cefmenoxime].

Basic and clinical studies in 37 patients with biliary tract disease on comparison between cefmenoxime (CMX) and cefotiam (CTM) were studied and the following results were obtained. In vitro antibacterial activities of CMX and CTM against 25 strains (15 organisms) isolated from bile of patients with biliary tract disease were stronger than that of cefazolin (CEZ). In cholecystectomized patients, CMX (2 g) or CTM (2 g) was injected intravenously, followed by determination of concentration in bile and gallbladder tissue about 2 hours after administration. In CMX administration, the mean concentration in gallbladder bile was 812.1 micrograms/ml, and the mean concentration in duct bile was 1,050.6 micrograms/ml, and the mean concentration in gallbladder tissue was 100.7 micrograms/g. In CTM administration, the mean values were, 1,092.5 micrograms/ml, 1,287.8 micrograms/ml, 28.5 micrograms/g, respectively. The concentration of CMX and CTM were almost similarly. The bile concentration of CMX (i.v.) was compared with CTM (i.v.) by cross-over method in cases of T-tube drainage. The peak bile concentrations of CMX and CTM were as high as 172.4 micrograms/ml and 182.2 micrograms/ml, respectively, 1 approximately 2 hours after 2 g intravenous administrations. Furthermore, the concentration of them were highly gained, 16.1 micrograms/ml of CMX and 33.8 micrograms/ml of CTM, even at 5 approximately 6 hours. In choledochostomized patients, CMX (4 g/day) was injected intravenously, followed by determination of concentration in intraperitoneal exudate. The mean concentration of CMX was 15.3 micrograms/ml on the first day after the operation, and 6.0 micrograms/ml even on the third day after the operation. Those results suggest that the high antibacterial activity of CMX against organism in bile and, the high penetration of CMX to bile, gallbladder tissue and intraperitoneal exudate will promise its important role in treatment of biliary tract infections.

[Acute myeloid leukemia with intracranial tumor formation--case report].

A 44-year-old house-wife was admitted to the hematological clinic of our hospital on December 21, 1981 for the treatment of acute myeloid leukemia. Hematological examination showed severe anemia, leukocytosis and thrombocytopenia. She was soon started on DCMP therapy (Daunomycin, Cytosine arabinoside, 6-MP and prednisolone). After several repeats of remission and aggravation, she suffered from vertigo, nausea and vomiting in early August of 1982. The CT scan and angiography showed a mass lesion in the middle of the posterior cranial fossa. On August 26, surgical extirpation of the tumor locating in the vermis of the cerebellum was performed successfully. On gross examination the tumor revealed smooth yellowish-red surface, elastic in consistency and measured 40 X 40 X 30 mm in size. Histologically it comprised the leukemic cells. The pathology of the central nervous system due to leukemic cells was discussed in relation to the pertinent literature, including the CT findings and the indication of the surgical treatment.

[Neurofibromatosis associated with intra- and extracranial aneurysms and extracranial vertebral arteriovenous fistula].

Neurofibromatosis associated with vascular disorders is generally known as a vascular neurofibromatosis. In the neurosurgical aspect, it has been pointed out that "moyamoya disease" -like stenosis and/or occlusion of the internal carotid artery are seen in some of the patients with neurofibromatosis. This 58-year-old female with multiple café-au-lait spots and neurofibromas all over the body was admitted to our hospital with complaints of left orbital tumor and tinnitus. She had been suffering from the left intraorbital neurofibroma since ten years of age. Radiological examinations revealed left intraorbital neurofibroma extending into the intracranial cavity, multiple aneurysms and arteriovenous fistula. The aneurysms were located at the left vertebral-posterior interior cerebellar artery junction and at the extracranial internal carotid artery. The arterio-venous fistula was demonstrated at the left extracranial vertebral artery. Pathological changes of the blood vessels in the vascular neurofibromatosis are essentially obstructive lesions due to intimal thickening. Only few cases of neurofibromatosis associated with aneurysms and arteriovenous fistula have been reported, and eleven cases of them could be reviewed in the literature. Six are with intracranial aneurysms, two with extracranial aneurysms and three with extracranial vertebral arteriovenous fistula,. It is interesting that in all cases the extracranial vertebral arteriovenous fistula are located on the left side. This is corresponding to the fact that the bone defect around the asterion is predominantly seen on the left side in cases of neurofibromatosis.

[The activities of phospholipase A, PI-specific phospholipase C, lipase, lysophospholipase and acylCoA: lysophospholipid acyltransferase in ischemic brain microsomal fraction].

It has been well recognized that acyl groups of phospholipids play an important role for structure and function of biomembrane. The turnover of these acyl groups in normal brain biomembrane is also well known. Some types of enzymic system related to this turnover has been investigated. Phospholipase A, PI-specific phospholipase C, lipase, lysophospholipase and acylCoA: lysophospholipid acyltransferase belong to these enzymic systems. In this report, the sequential changes of phospholipase A, PI-specific phospholipase C, lipase, lysophospholipase and acylCoA: lysophospholipid acyltransferase activities in ischemic rat brain were examined. The purpose of this study was to examine the enzymic changes of deacylation-reacylation cycle of biomembrane phospholipid in ischemic brain. Ischemic brain were produced by decapitation and activities of 5 enzymes were assayed in microsomal fraction. The activities of phospholipase A, PI-specific phospholipase C, lipase showed high value during early stage of ischemia for 15 or 30 min and then decreased gradually. Lysophospholipase activity was not changed for 120 min. On the other hand, acylCoA: lysophospholipid acyltransferase activity showed gradual decrease from the beginning of ischemia. There are some reports that in early ischemic stage, the concent of free fatty acids increase, while that of phospholipid decrease. The present results may suggest that the changes of free fatty acid and phospholipid in ischemic brain are related to these enzymic system.

[A clinical study on the penetration of aspoxicillin into tissue and wound exudates in breast cancer].

Penetration of aspoxicillin (ASPC), a new semisynthetic penicillin of broad spectrum, into normal and cancer tissue and wound exudate were examined in breast cancer. ASPC was administered by single injection of 1 or 2 g. The concentration of ASPC in the resected skin, normal mammary gland and cancer were similar to their serum levels, when ASPC was administered before radical mastectomy. ASPC concentration in the wound exudate which was collected from the inserted drains into the resected area reached the maximum at 3 approximately 4 hours after the injection at the levels of 19.4 micrograms/ml (1 g) and 35.1 micrograms/ml (2 g). The level decreased gradually by maintained still high at 3.7 micrograms/ml (1 g) and 11.4 micrograms/ml (2 g) even 8 hours after the injection. From these results it may be expected that ASPC is a useful penicillin in the breast surgery.

[Heterotopic gastric mucosa in the wall of the gallbladder: a case report and review of the literature].

A case of heterotopic gastric mucosa (pyloric gland) in the wall of the gallbladder was reported. A 58 year-old woman was admitted with a history of sudden attack of hypochondralgia radiating to the back, but she had no fever and jaundice. Ultrasonography demonstrated high echo at the neck and body of the gallbladder. Cholecystectomy was performed with a diagnosis of the gallbladder tumor containing gall stone. The surgical specimen revealed a 15 X 14 X 8mm subserosal tumor in the body and chronic cholecystitis, but showed no metaplastic differentiation in the mucosa. The cause was considered to be congenital. Heterotopic tissue in the wall of the gallbladder was very rare. In the literature, there were 29 cases of gastric mucosa and 27 cases of pancreatic tissue. The mean age of incidence were 31.9 and 42.0 year-old, respectively. There is no sex difference. The lesion was located in the cystic duct and the neck of the gallbladder in half the patients. Cholelithiasis and/or cholecystitis was complicated in many cases, and it might be associated with heterotopic tissue whereas the etiology is unknown.

[Reticuloendothelial function as a prognostic factor in breast cancer].

Reticuloendothelial function was investigated in breast cancer by the 131I-microaggregated albumin method using the parameters of phagocytic function (K value) and metabolic function (M value). Reticuloendothelial function of the patients with breast cancer was decreased compared with that of the controls, and it was related to the peripheral lymphocyte count. This finding indicates that reticuloendothelial function is one of the prognostic factors in breast cancer.
