Volvulus of the transverse colon in a four-year-old boy.

A 4-year-old boy with a volvulus of the transverse colon is presented. The mildness of the obstruction made direct visualization of the twisted loop with barium enema possible. At laparotomy a congenital band between the transverse colon and the small intestine was found to have been the predisposing factor of the volvulus.

Peliosis hepatis. Report of two autopsy cases with a review of literature.

Two cases of peliosis hepatis, treated with a large dose of predonine for aplastic anemia and multiple myeloma, respectively, were reported. Case 1 showed no abnormal liver function, but, in Case 2 hepatomegaly, retention of ICG, and elevation of LDH level despite the normal serum transaminase were noted. In both cases, peliotic lesions were incidentally observed at autopsy. In Case 1, the lesion was localized in the right lobe only, but, in Case 2 the lesion was present throughout the entire lobes of the liver involving also a part of the spleen and bone marrow of the sternum. Innumerable blood-filled cavities were surrounded by incomplete reticulin or injured hepatocytes. In smaller lesions observed in Case 1, focal necrosis, hemorrhage, inflammation, and thrombi were prominent features. On the other hand, in extensive lesions, as in Case 2, a marked liver cell dissociation with sequent irregular reticulin arrangement was present. Though the etiology and peliosis hepatis is still uncertain in these cases, it was assumed that a large dose of predonine might have some correlation to the occurrence.

Alterations of polypeptide composition of mature granulocytes obtained from patients with chronic myelogenous leukemia.

In order to compare the polypeptide composition of the CML mature granulocytes with that of the normal whole cell lysates, mature granulocytes from four healthy volunteers and six CML patients were analyzed by two-dimensional gel electrophoresis. In the normal subjects, 60 major spots were commonly and reproducibly identified. In the CML, there were constant alterations in some of these major spots. Four spots were totally absent in all the CML samples, and another four spots were absent in five of the six samples. In addition, one spot was larger in CML than in normal cells, and another spot, which was only faintly visible or not detectable in the normal samples, was massively present in all the CML samples. Our data suggest specific changes in the polypeptide compositions of CML granulocytes. This method could be clinically applied for the analysis of granulocytic disorders.

Factor(s) responsible for the increase in alkaline phosphatase activity of postmitotic granulocytes from normal individuals and patients with chronic myeloid leukemia.

Using postmitotic granulocytes (PMGs) with low neutrophil alkaline phosphatase activity (NAP activity), factor(s) having the capacity to increase their NAP activity were examined in vitro. A high activity of the factor was demonstrated in the cystic fluid of a human squamous cell carcinoma, which is known to produce a large amount of granulocyte-macrophage colony-stimulating factor (GM-CSF). The NAP-stimulating factor increased NAP values both in PMGs from normal bone marrow and PMGs from patients with chronic myeloid leukemia (CML), and NAP values in cells treated with the factor approached or rose above those of normal peripheral granulocytes after 48 hr of culture. The effect of the factor was specific in that the factor caused stimulation only in granulocytic series. These findings may indicate that increases in NAP activity reflect maturation or granulocytes and that low NAP activity of neutrophils derived from patients with CML is due to the immaturity of these cells. The relationship between the factor responsible for the increase in NAP activity and GM-CSF is also discussed.

Colony formation by T-lymphocytes infiltrating human tumours.

Cell suspensions from 10 breast cancers and 4 melanomas were cultured in soft Agar with phytohaemagglutinin-stimulated lymphocyte-conditioned medium (PHA-LCM). Between 3 and 10 days, some of the plated cells formed colonies of greater than 20 cells indentifiable as T-lymphocytes by morphology, cytochemical staining and capacity to form rosettes with sheep erythrocytes. The frequency of colony-forming T-lymphocytes was 0-32 per 2 X 10(5) viable cells plated, and correlated with the degree of lymphocytic infiltration within the tumour. This cloning procedure appeared to select for a subpopulation of T-cells which is well represented within primary tumours. It should prove useful for investigating lymphocyte tumour relationships in vivo.

Umbilical core-out operation for a completely patent vitelline duct.

An umbilical core-out operation for a completely patent vitelline duct in a premature baby with successful preoperative manual reduction of the prolapsed ileal loops is described. In certain cases with this rare anomaly this procedure can be applied. The procedure is easy, timesaving, bloodless and necessitates no laparotomy incision.

An ultrastructural study of the primary follicle in the lymph node.

The present authors have studied the cellular composition of the primary follicle (PF) in the germ-free rat lymph nodes and the development of PF in the human lymph node anlages by using the light- and the electron microscopy. According to the ultrastructural study, the cellular composition of PF in the germ-free rat lymph nodes and classified into five cell types, 1) lymphocytic cells (small and medium-sized lymphocytes), 2) large lymphocytes (= large germinal center cells), 3) reticulum cells with desmosomes (RED), 4) desmodendric cells (DDC), and 5) histiocytes. The interstitial cells with desmosome are classified into DDC and RED based on the relationship to collagenous fibers and the morphological differences. There are amorphous electron-dense intercellular materials among the cytoplasmic processes of DDC, which are distinguished from histiocytes based on the morphological differences. PF emerges as a spherical aggregation of young DDC straight underneath the marginal sinus in the outer cortex of the human embryonal lymph node anlages on the 16th fetal week.

Colonies formed in agar from human breast cancer and their identification as T-lymphocytes.

Single cell suspensions prepared from human breast cancer specimens by collagenase digestion were cultured in soft agar with phytohemagglutinin-stimulated human lymphocyte-conditioned medium (PHA-LCM). In 6 of 10 different tumors, PHA-LCM-dependent clonal growth was develop. After 12-14 days of incubation, two morphologic types of colony containing 20-500 cells were recognized. Both were composed of lymphocytes of T-cell nature, as judged by cell morphology in smears, cytochemical properties, capacity to form rosettes with sheep erythrocytes, and electron microscopic appearances. Contamination of the tumor cell suspensions by blood could be excluded as a source of the colony-forming lymphocytes, and the incidence of colony-forming cells correlated well with the degree of lymphocyte infiltration of the tumors. Some of the colonies in agar were expanded further in liquid culture in the continuous presence of PHA-LCM. These clones were apparently high in proliferating capacity as compared with the proliferating activity of peripheral T-cell clones obtained from normal blood. These clones were considered to be highly activated T-lymphocytes and to be stimulated to grow in vitro by the T-cell growth factor contained in PHA-LCM. The direct cloning and expansion of such activated T-lymphocytes infiltrating the tumors will be useful for studies on the functional characteristics of these cells.

[Transfer of cefotaxime to the pelvic organs (author's transl)].

The new antibiotic cefotaxime (HR 756, CTX) has been proved to be clinically effective against infections observed in the field of obstetrics and gynecology. The present study was intended to investigate the transfer of CTX to the internal genital organs and the dead pelvic space. The results were obtained as follows: 1. The concentrations of CTX transferred to the uteri and its appendages after CTX 1 g intravenous injection were sufficiently effective against the major pathogens (Gram-negative and anaerobic bacteria) demonstrated in the field of obstetrics and gynecology. 2. The concentrations of CTX transferred to the dead space of the pelvis were effective against almost all of the Gram-negative bacteria for 6 to 12 hours after CTX 1 g or 2 g intravenous injection. CTX was thus proved to be very effective for the prevention and treatment of infections of the dead pelvic space.

Colony growth in agar by human melanoma cells.

Colony-forming capacity of human melanoma cells was investigated in five established cell lines and isolated cells from seven metastatic or recurrent tumors. A single-cell suspension was added to 0.3% agar medium, and colonies were observed over 7 to 21 days. With the five cell lines, colony formation occurred in all cases and was usually accelerated by 2-mercaptoethanol, but there was wide variation in cloning efficiency between the cell lines (5 to 80%), in the appearance of the colonies, and in their recloning capacity. In those cell lines with low cloning efficiency, some colonies grew attached to the culture dishes, and these had a high recloning capacity. In the melanotic cell lines, colonies originated from slightly or nonpigmented cells which became more pigmented during incubation. Stimulation of colony growth by 2-mercaptoethanol led to reduced pigmentation of colonies, whereas theophylline caused increased melanin production and decreased colony size. With freshly obtained tumor specimens, both the frequency of colonies (0.01 to 0.26%) and the growth rate were much reduced compared to the cell lines. Both could be increased by addition of 2-mercaptoethanol to the medium or by separation of the tumor cell suspension on a density gradient before culture.

Renal cell carcinoma having heterogeneous histological appearance and homogeneous enzymatic property.

A case of renal cell carcinoma (RCC) presenting various histologic growth patterns, including papillary, tubular and solid arrangements, and an area resembling sarcoma are described. In order to identify RCC of high versatility of renal carcinosarcoma, electrophoretic studies of tissue alkaline phosphatase (Al-P) and gamma-glutamyl transpeptidase (gamma-GTP), as well as histochemical studies of these enzymes, were conducted. The Al-P was proved to be bond type alone and the gamma-GTP corresponded with the novel isozyme associated with RCC tissue. Moreover, these two enzymes were found to be distributed in every part of different histologic appearance. It was concluded from these results that the histologic pleomorphism of the case herein reported was due to morphologic transformation of the neoplastic cells sharing essentially identical enzymatic nature.

Detection and assessment of human tumours producing granulocyte-macrophage colony-stimulating factor (GM-CSF) by heterotransplantation into nude mice.

Production of granulocyte-macrophage colony-stimulating factor(s) (GM-CSF) by human tumours was investigated using heterotransplantation of a number of different tumours in nude mice. An increase in granulocyte numbers (> 20,000/mm3) in the peripheral blood of nude mice accompanied the growth of 9 of the 25 transplanted tumours. GM-CSF activity tested against normal human marrow cells was relatively high in 6 of these 9 tumours. Moreover there was either weak activity or none at all in 14 of the 16 tumours that failed to cause a definite granulocytosis. The correlation between granulocytosis and GM-CSF activity was 0.36, which was statistically significant (P < 0.01). These findings indicate that the transplantation of human tumours into nude mice can provide a useful tool for detection and characterization of granulopoietic factors derived from the tumours.
