The maturation process of leukemic myelomonocytoid cells derived from peripheral blood of two typical cases of chronic myelomonocytic leukemia was investigated in vitro. In both cases, liquid culture of the leukemic cells in the presence or absence of 12-o-tetradecanoyl phorbol-13-acetate yielded either monocyte-macrophages or mature granulocytes, respectively, when examined by May-Giemsa staining. However, using cytochemical criteria, both of these mature cell fractions were found to be heterogenous and still contained substantial numbers of cells positive for both nonspecific and chloroacetate esterases. Phagocytic activity was markedly defective in the monocyte-macrophages recovered from culture as well as in freshly-isolated peripheral monocytes in both patients. In addition, active oxygen production from isolated monocytes stimulated by zymosan was also decreased in one of the cases. The results indicate that at least in some cases of chronic myelomonocytic leukemia, leukemic cells have the potential to differentiate in vitro to monocyte-macrophages which, however, show only limited development of functions, as well as freshly-isolated monocytes from peripheral blood.
A case of bilateral testicular germ cell tumor of different cell types was found to have a mature teratoma in the metastatic region. Histology of the right testis revealed seminoma and embryonal carcinoma while the left testis revealed seminoma and mature teratoma. A mature teratoma, containing cartilage components, was also found in the right para-aortic lymph node. The findings of the case are described and the literature on cases with maturation in the metastatic region are reviewed.

A case of bilateral testicular germ cell tumors of different cell types and maturation in the metastatic region is described. A 42-year-old man with bilateral testicular swelling visited our clinic. Bilateral high orchiectomy was performed. Subsequent histological examination revealed seminoma and embryonal carcinoma in the right testis and seminoma and mature teratoma in the left. Then bilateral retroperitoneal lymphadenectomy was performed and 7 metastatic regions were found in the para-aortic lymph nodes. One of these revealed histopathologically a mature teratoma containing cartilage constituents. After the operation, multiple drug treatment with Cis-DDP, vinblastine and pepleomycin was started. The patient has been in complete remission for about 1.5 year. In addition, we reviewed the literature about bilateral testicular germ cell tumors and maturation in the metastatic region.

In a typical case of chronic myelomonocytic leukemia (CMML), a chromosome abnormality, 46,XY,20q-, was observed in all the dividing cells including up to 16-ploid cells in the bone marrow and the blood. As the mitotic figures could be easily seen not only in myelomonocytoid cells but also in erythroblasts in the bone marrow smear, it was concluded that all the cell lineages except lymphocytes had the abnormality. The present case will support the view that the leukemic process in CMML affects a multipotent stem cell rather than a granulocyte-monocyte committed stem cell.
A newborn having a covered anus and two fistulas, one ano-urethral and one penile urethro-cutaneous, is presented. In spite of the resemblance of this ano-urethral fistula to an "H-type" ano-urethral fistula, which is thought to result from the failure of the division of the cloaca in the embryological period, three abnormalities seen in this case were thought to be the result of a serial event which occurred in the more distal part, the anal and the genital folds, during the embryological period.
A case of verrucous squamous cell carcinoma of the oesophagus treated with bleomycin is reported. Bleomycin brought prompt reduction of the tumour and relief of symptoms. There have been no previous reports on experience with anti-cancer agents for verrucous carcinoma of the oesophagus. Administration of bleomycin might be an effective treatment for verrucous carcinoma of the oesophagus.

Meth-A fibrosarcoma (10(5) cells) was implanted into the thigh of BALB/c mice. After a predetermined interval, the mice were injected i.v. with FC-43 (artificial blood) with or without oxygen, treated by irradiation or a combination of injection and irradiation and the effect of these treatments was studied. The mear tumor weight of animals treated by irradiation plus oxygen was 6.57 g and 3.18 g in experiments 1 and 2, respectively. Treatment with irradiation plus FC-43 with oxygen resulted in a significant decrease of the mean tumor weight, 3.17 g and 1.59 g in experiments 1 and 2, respectively (p less than 0.05, p less than 0.02). FC-43 with oxygen may have a synergistic effect for radiation therapy. When the preirradiated tumors were small (4.8 mm2), this effect was not obvious.
Two human malignant tumors, which we previously reported to produce colony-stimulating factors (CSFs), were found to be accompanied by remarkable hypercalcemia. A patient with a CSF-producing lower jaw cancer (squamous cell carcinoma) developed a marked granulocytosis (150,000/microliters) and hypercalcemia (more than 215 mg/dl). The tumor was successfully transplanted into nude mice, which developed marked granulocytosis (300,000/microliters) and hypercalcemia (20 mg/dl). White blood cell and serum calcium concentrations of these mice decreased promptly to normal levels when the tumor was excised. Treatment with prednisolone (1.5 mg/kg) or indomethacin (5 mg/kg) had no effect on the serum calcium level of these mice. Parathyroid hormone or prostaglandin E was not increased in the serum of the mice or in the tumor tissue. However, the mice bearing the tumor excreted extremely large amounts of calcium in their urine, and their bony tissues contained less calcium and phosphorus than controls. Moreover, histology of bony tissues of these nude mice clearly demonstrated the decrease in trabecular tissues and cortical thickness as well as remarkable activation of osteoclasts. Another patient with a CSF-producing bronchogenic squamous cell carcinoma showed mild granulocytosis and hypercalcemia. The biopsied tumor tissue was transplanted into nude mice, which developed marked granulocytosis (300,000/microliters) and also severe hypercalcemia (18 mg/dl). These results suggest the presence of a new syndrome of granulocytosis and hypercalcemia associated with CSF-producing tumors. The causal mechanism of the hypercalcemia was shown to be some humoral factor which activates osteoclasts other than parathyroid hormone. Neither prostaglandins nor osteoclast-activating factor seemed to be the cause of the hypercalcemia.
Activities of T cell growth factor (TCGF) of conditioned media from PHA stimulated cultures of peripheral mononuclear cells (PHA-MNC-CM) were compared among patients with cancer, patients with non-malignant disease and normal persons. In nine out of 10 cases of cancer with metastasis (including extensive infiltration to surrounding tissues), the activities were much lower than normal persons, whereas in four out of five cases of cancer without metastasis, the activities were almost the same as those of normal persons. The low activities of PHA-MNC-CM from patients with metastatic cancer were not due to the older age of patients, nor did it seem to be attributed to the malnutritional state of patients. When we examined the activity using non-adherent MNC instead of the whole MNC, the activities in patients with metastatic cancer rose up to almost the normal levels. In normal persons the decline of the activities after 3 days of culture were not observed after the removal of adherent cells. Our data indicate that the adherent cells have a suppressive effect on TCGF production in both normal persons and cancer patients, but that the effect may be exaggerated in patients with metastatic cancer.
