[A case of rectal cancer with malacoplakia].

A 59-year-old man was hospitalized for rectal bleeding. Barium enema and proctoscopy revealed an elevated tumor, completely stenosing the rectum. The microscopic features of the specimens obtained by proctoscopic biopsy of the tumor comprised aggregations of histiocytes with intracytoplasmic inclusions (von Hansemann cells and Michaelis-Gutmann bodies). Rectal malacoplakia was diagnosed. In the course of laparotomy, the tumor was found to be strongly infiltrated to the pelvis. Thus, radical surgery was not performed, and an artificial anus was made. Excisional biopsy from the tumor tissue revealed the features of well-differentiated adenocarcinoma and malacoplakia. Forty-three cases of malacoplakia of the gastrointestinal tract from a review of the literature including ours were collected and discussed.

[A case of pulmonary metastasis from renal cell carcinoma in which complete remission was achieved by interferon therapy (r-IFN-alpha A, Ro 22-8181)].

A case of pulmonary metastasis from renal cell carcinoma in which complete remission was achieved using interferon therapy is herein reported. A 54-year-old male underwent left nephrectomy following diagnosis of left renal cancer. Radiotherapy and hormonal therapy were performed postoperatively. Solitary pulmonary metastasis was found in the right lobe 4 months after surgery. The interferon (recombinant human leukocyte A interferon, rIFN-alpha A, Ro 22-8181) was initially administered i.m. at a dose of 3 X 10(6) units per injection every day. Marked response was observed one month after starting the interferon therapy (total: 183 X 10(6) U) and complete disappearance of the tumor was obtained 2 months after commencement of therapy (total: 345 X 10(6) U). The interferon was later injected at a dose of 6 X 10(6) U., 3 times a week because of general fatigue. The duration of complete response has lasted 8 months.

Induction of alkaline phosphatase in neutrophilic granulocytes, a marker of cell maturity, from bone marrow of normal individuals by retinoic acid.

We examined whether chemical agents reported to induce differentiation of leukemic cells also have differentiating effects on normal human granulocytes using alkaline phosphatase activity as a marker. Among 11 compounds examined, only vitamin A analogues were shown to induce this activity in granulocytes from bone marrow of normal individuals. Retinoic acid was the most potent inducer of the activity followed by retinal, whereas retinol and retinol acetate did not induce any activity. The effect on the alkaline phosphatase activity by retinoic acid and retinal was considered to reflect their effect on normal granulocytic differentiation and maturation.

Characterization of neutrophil alkaline phosphatase-inducing factor (NAP-IF).

A factor, termed neutrophil alkaline phosphatase-inducing factor (NAP-IF), that has the capacity to increase the NAP activity of granulocytes was characterized by using two samples: cystic fluid (CF) and conditioned medium of a tumor cell line (T3M5). The molecular weight of NAP-IF was shown to be between 13,000 and 45,000, and its isoelectric point was between 5.5 and 6.2. It was sensitive to heat and proteolytic enzymes, but was resistant to DNase and RNase, suggesting that NAP-IF is an acidic protein or glycoprotein. These characteristics of NAP-IF seem to be similar to those of granulocyte-macrophage colony-stimulating factor (GM-CSF) that is also present in the CF. NAP-IF rich fractions obtained by isoelectric focusing from CF were also found to be rich in a subclass of GM-CSF: granulocyte-CSF (G-CSF). Furthermore, a high correlation was noted between the activities of G-CSF and NAP-IF (gamma = 0.798, P less than 0.005). These results suggest that the two activities, i.e., G-CSF and NAP-IF, may be attributable to an identical macromolecule.

Electron microscopic study on Langerhans cells and related cells in lymph nodes of DNCB-sensitive mice.

To understand contact hypersensitivity, it is important to know the kinetics of Langerhans cells (LC) and related cells in the lymph node (LN), as well as in the skin. For this purpose, we tried experimentally to induce increased numbers of LCs, Birbeck granule-like structure (BgS)-containing cells, and interdigitating reticulum cells (IDC) in DNCB-sensitive mice and studied them by means of electron microscopy with the following results: (1) cytologically, LC, BgS-containing cells and IDC were closely related; (2) BgS seemed to arise from rough-surfaced endoplasmic reticulum (r-ER), and BgS-containing cells were midway in nature between LC and IDC from the morphological view point. From these findings, it appears that IDC, BgS-containing cells, and LCs were simultaneously involved in the contact hypersensitivity reactions of LNs.

Increased Langerhans cells and related cells in mesenteric lymph nodes of DNCB-sensitive mice.

We used electron microscopy to study mesenteric lymph nodes (LNs) of 2,4-dinitro-1-chlorobenzene(DNCB)-sensitive mice. It was found that Langerhans cells (LCs) and cells containing Birbeck-granule-like structures (BgSs) increased in number only after challenge with DNCB. This increase reached a peak 4 h after challenge. LCs and BgS cells were only found in the paracortical area of LNs. There were no appreciable differences in the morphology of LCs, BgS cells, and interdigitating reticulum cells (IDCs), except for the presence or absence of Bgs or BgSs. No LCs, BgS cells, or IDCs were found in the marginal sinus of mesenteric LNs or in the surroundings of afferent blood vessels. These findings were in complete agreement with those obtained in our previous study of cutaneous LNs. From our findings and the results of previously published studies, we propose the following conclusions: first, LCs and BgS cells observed in mesenteric LNs are the product of the differentiation and maturation of precursor cells pre-existing in the paracortical area of LNs caused by DNCB stimulation. Second, the activated T lymphocyte to DNCB pre-exists in the paracortical area; thus, the DNCB used for challenge stimulates this T lymphocyte to release lymphokine, which then causes some precursor cells to differentiation into LCs and/or BgS cells. Finally, morphological findings suggest that precursor cells of LCs and BgS cells are probably IDCs.

Suppression of mixed lymphocyte reaction by cells of human first trimester pregnancy endometrium.

In order to identify an immunological role for decidual tissue in pregnancy we have prepared single cell suspensions from the tissues of normal pregnant women and examined the effects of these cells on one-way mixed-lymphocyte reactions (MLR). The separated cells were heterogeneous, containing classical decidual cells, glandular epithelial cells, granular endometrial cells, macrophages and small lymphoid cells. [3H]Thymidine incorporation at day 6 of the MLR was suppressed by addition of the cells at the initiation of the cultures and the degree of suppression was inversely correlated to the gestational age of the decidual tissue, apparently through inhibition of the antigen recognition phase of the MLR. These findings support the view that the cells of the human first trimester pregnancy endometrium may play an important role in protecting the feto-placental unit from rejection, at least in the early phase of pregnancy.

Alteration of polypeptide synthesis and composition during differentiation of human leukemia cell line HL60.

Using two-dimensional gel electrophoresis, we have analyzed the polypeptide alteration of the human myeloid leukemia cell line HL60 during differentiation induced by dimethylsulfoxide (DMSO) and 12-0-tetradecanoylphorbol-13-acetate (TPA). Three polypeptides increased in synthesis after DMSO treatment, while 5 polypeptides increased after TPA treatment. Most other polypeptides were synthesized at a reduced rate though their net amounts remained unchanged, indicating that myeloid differentiation was associated not only with an increased synthesis of a few specific polypeptides but also with overall depression of most other polypeptides. One of the early differentiation marker proteins (mol. wt. approx. 20,000, pI approx. 6.0) of granulopoietic cells was increased specifically after DMSO treatment, while one of the monocyte specific marker proteins (mol. wt approx. 46,000, pI approx. 4.8) was detected after TPA treatment. But, the pattern of HL60 cells did not necessarily become similar to that of normal mature granulocytes after DMSO treatment, indicating that HL60 cell differentiation might be partial or incomplete.
