[Psychodrama versus patient club activity (leisure group): effects of a 25 hours group psychotherapy with psychiatric patients (author's transl)].

UNLABELLED: The effects of psychodramatherapy versus patient club activity group is compared in a controlled study with psychiatric patients in a period of two month (8 times a three hour session per week). SAMPLE: 22 patients (11 patients psychodramatherapy (PD); 11 patients patient club activity (PCA)). Diagnoses: neuroses, subacute psychoses. Patients were matched according to age, sex and syndrome (resp. diagnoses). Applied tests: semistandardized interview with formulation of personal therapeutic goals, personality tests (Giessen, MMPI), mood scales (EWL, Bf-S v. Zerssen, 100 mml line), psychopathology (AMP 3/interview), social adjustment (SAF). Testing periods: before beginning with PD and PCA, attendant to therapy, after therapy, follow up (three month after end oftherapy). RESULTS: it was advantageous to have both psychotic and neurotic patients in the same therapy group. In the course of psychodramatherapy and patient club activity there was an improvement with a regressive trend to baseline after end of therapy. PD in comparison to PCA appeared to be more effective in the improvement of some psychiatric relevant personality dimensions, in the improvement of psychopathology, in reaching the personal therapeutic goals and in a positive opinion of therapy. A psychodramatherapy lasting two month seems revealed to be more attractive and to have more therapeutic efficacy than a patient club activity group. Because of the instability of some effects a more prolonged therapy period is suggested.

[Isosorbide-dinitrate in myocardial perfusion (author's transl)].

UNLABELLED: The effect of isosorbide dinitrate (ISDN) (10 mg sublingually) on myocardial perfusion during ischemic conditions was analyzed in 14 patients with angiographically severe coronary artery disease and typical angina pectoris, using the 201-Thallium-myocardial scintigraphy. All patients underwent two identical scintigrams with the same work load during bicycle ergometry; a control scintigram leading to angina and ST-depressions of greater than 0.1 mV was followed 4--6 weeks later by a scintigram after ISDN; all drugs -- except sublingual nitroglycerin -- were withheld for an entire week. -- RESULTS: 25 of 39 new or enlarged, exercise-induced defects (64%) were normalyzed after ISDN; 14 new or enlarged defects remained unchanged (p less than 0.0005). In the 11 patients, in whom ISDN led to complete abolishment of angina, 23 of 30 new defects (76%) were normalized against none in the 3 patients with persistent angina and ST-depressions. Quantitation of perfusion was attempted by calculating the average impulse rate (counts/min/matrix point): in those patients reacting favorably to ISDN average impulse rate decreased significantly especially in the normal area (p less than 0.001), resulting in a more homogeneous distribution of Thallium activity, indicating a marked reduction in local oxygen consumption.

[Pharmacokinetics and plasma albumen binding of iotroxic acid (Biliscopin), iodoxic acid (Endomirabil) and ioglycamine (Biligram) (author's transl)].

Pharmacokinetic data after injection of the new cholegraphic contrast medium iotroxic acid (Biliscopin) in man are reported and compared with the results of injections of ioglycamate and iodoxic acid. Iotroxic acid is less completely bound to plasma proteins than ioglycamate, but significancy more so than iodoxamate. Plasma protein binding depends on contrast concentration in the plasma, as does excretion in the urine. Biliary transport rate and maximal iodine concentration in the gall bladder are higher after injections of iotroxinate and iodoxamate than after ioglycamate.

[Effect of sublingual isosorbide dinitrate on hemodynamics in coronary patients with and without congestive heart failure (author's transl)].

In 17 patients with coronary heart disease, hemodynamic measurements were performed before and after sublingual application of 10 mg isosorbide dinitrate (ISDN). 10 patients showed signs of heart failure with pulmonary congestion and a left ventricular filling pressure above 15 mmHg, resting hemodynamics were normal in 7 patients. Eight of the patients with left ventricular failure had sustained acute myocardial infarctions the size of which was assessed by serial determinations of serum creatine phosphokinase. Application of ISDN resulted in a significant decrease of systemic and pulmonary artery pressures and pulmonary capillary wedge and right atrial pressures of patients both with and without left ventricular failure. Cardiac index and stroke index as well as systemic and pulmonary resistances did not change significantly. ISDN did not affect left ventricular stroke work in patients with elevated filling pressures; however, a decrease of normal filling pressures was associated with a decrease of stroke work. Thus, in coronary patients with chronic congestive heart failure, sublingual application of nitrates results in a beneficial hemodynamic unloading. However, if the acute infarct size is taken into account, it can be demonstrated that hemodynamic improvement after ISDN--judged by the relation stroke work/filling pressure--becomes less pronounced with increasing infarct size.

[Treatment of arthritic psoriasis with Demoplas Clinical and scintigraphic study in 10 patients (author's transl)].

Clinical, X-ray, and scintigraphic (6mCi 99mtechnetium pertechnetate, 4mCi 99mtechnetium pyrophosphate) tests for joint involvement were conducted on 11 inpatients with psoriasis (4 female, 7 male), 6 of them cases of what is conventionally termed psoriatic arthropathy. These tests were performed both before and after a 12-day period of therapy with daily 4 x 1 coated tablets of Demoplas containing 150 mg phenylbutazone Scintigraphic tests showed that pathological radionuclide concentrations were present in at least one joint or group of joints in 10 out of 11 patients. Following therapy the previously pathological scintigraphic findings had stabilized completely in 4 patients and partially in 3 patients, but had not stabilized in 3 patients. The results of therapy with Demoplas as demonstrated by joint scintigraphy were thus good to satisfactory in 7 patients and unsatisfactory in 3 patients- possibly owing to the short duration of therapy. The improvement in scintigraphic results testifies to the therapeutic action of phenylbutazone on arthritic psoriasis. This can also be regarded as additional proof that psoriatic joint involvement is of an inflammatory and proliferative nature.

[Hemodynamic and electrocardiographic prolonged nitrate effect during frequency load in coronary disease].

Hemodynamic and electrocardiographic analysis during rapid right atrial stimulation was performed before and one, two, and four hours after oral application of longacting nitroglycerin (5 mg) and isosorbide dinitrate (20 mg) in 11 and 9 patients, respectively with coronary heart disease. Atrial stimulation without nitrate induced significant ischemic ST segment depression. Cardiac output showed a small decrease and the mean arterial, pulmonary artery, and pulmonary wedge pressure increased. Isosorbide dinitrate reduced the ischemic reaction by 40% from the first to the fourth hour after application. Cardiac output, stroke volume, aterial, pulmonary artery, and pulmonary wedge pressure also decreased continuously. Nitroglycerin caused a similar reduction of ischemic ST segment depression for two hours. Systolic, diastolic, and mean arterial pressure decreased significantly. Cardiac output, stroke volume, and pulmonary artery pressure remained unchanged. It was concluded that the applied dose of isosorbide dinitrate showed a more extensive longacting effect.
