Transthoracic approach to an intramedullary vascular malformation of the thoracic spinal cord.

A transthoracic approach with resection of a vertebral body was used to obtain access to an intramedullary vascular malformation of the thoracic spinal cord. The malformation consisted of three discrete intramedullary tufts of vessels fed by an abnormal branch of the anterior spinal artery arising from the T-9 intercostal artery. Although the abnormal branch of the anterior spinal artery did not appear to supply normal cord, the patient developed transient weakness postoperatively after occlusion of this artery. Follow-up angiography showed that one part of the malformation was still present and was fed by the T-y intercostal artery.

[General practice of nutrition diagnosis].

Important condition for a parenteral, enteral or parenteral-enteral nutrition, especially in patients before extended operations of during conservative therapy, is an exact nutritional assessment before beginning of this therapy. By means of biochemical examinations (visceral proteins), anthropometric measurements (body mass, triceps skinfold, arm muscle circumference, creatinine index) and immunological methods (total lymphocyte count in the blood, skin testing with test antigens) it is possible to classify nutritional conditions in severity and kind of malnutrition. By evaluating the digestive and resorptive capacity it is possible to determine the way of application.

[Kinetics of intestinal absorption in cases of dumping syndrome following partial gastrectomy and pyloric stenosis (author's transl)].

By means of analyses of intestinal absorption using the modified D-Xylose test the absorption kinetics of nutrients in cases of dumping syndrome after partial gastrectomy and in cases of pyloric stenosis could be traced. There was no decrease of absorption in the small gut. Changes of the intestinal absorption in cases of gastric dumping were caused by rapid gastric emptying. The delayed gastric emptying in cases of pyloric stenosis is followed by a decrease of Xylose resorption due to the reduction of resorption. The D-Xylose-test allows disorders of gastric emptying to be differentiated from those of enteral resorption.

Interactions of magnesium and verapamil on tone and contractility of vascular smooth muscle.

Previous studies on isolated blood vessels indicate that (1) withdrawal of magnesium ([Mg2+]0) induces calcium-dependent contractile responses, and (2) Mg2+]0 and verapamil (VE) inhibit calcium influx across the cell membrane. The present study, using isolated rat aortic strips and portal veins, was designed to assess the interactions of [Mg2+]0 and VE on increases in active tension and contractility induced by withdrawal of [Mg2+]0.[Mg2+]0 was found to: (1) enhance VE-induced inhibition of portal vein amplitude, and (2) antagonize VE-induced enhanced frequency of spontaneous phasic contractions in portal veins. Both [Mg2+]0 and VE could prevent and reverse the increases in active tension developed in aortic smooth muscle upon removal of [Mg2+]0.[Mg2+]0 markedly potentiated the inhibitory effect of verapamil on calcium-induced contractions of K+-depolarized aorta but not in K+-depolarized portal vein. This synergistic effect in aortal could be due to the influence of Mg and VE at the membrane and/or the influence of Mg at intracellular sites resulting in decreased levels of functional (activator) cytoplasmic Ca2+. On the other hand, the lack of synergism in portal vein could indicate that both of these agents might be inactivating the same membrane channels involved in calcium influx.

Analysis of T cell function in autoimmune murine strains. Defects in production and responsiveness to interleukin 2.

In the studies reported here, we have analyzed the production and consumption of T cell growth factor, more recently termed interleukin 2 (IL-2), as well as some cell-mediated immune functions, in murine strains [MRL, BXSB, NZB, and (NZB x NZWF1] manifesting systemic lupus erythematosus (SLE)-like syndromes. Young (4-6 wk) or old (4-8 mo) autoimmune or normal mice were studied and compared with regard to the following T cell functions in vitro after stimulation with concanavalin A (Con A): (a) mitogenic response; (b) IL-2 levels in culture supernates; and (c) the ability to respond to and adsorb IL-2. In addition, proliferative activity in the allogeneic mixed leukocyte culture and frequency of alloreactive cytotoxic T lymphocyte precursors (CTLp) were analyzed in some of these strains. Reduced Con A-induced mitogenic responses and IL-2 production appeared at 3-6 wk of age in the early, severe SLE developing strains MRL-Mp-lpr/lpr (MRL/l) and male BXSB and progressed thereafter. Similar defects appeared at a later stage in MRL/Mp-+/+ and (NZB x NZW)F1 hybrid mice, which develop late disease. Detailed analysis of cells from the enlarged lymph nodes and spleens of older MRL/l mice demonstrated that such cells: (a) responded poorly to Con A or allogeneic stimulator cells, even in the presence of exogenous IL-2; (b) did not suppress IL-2 production by normal spleen cells; (c) were relatively incapable of adsorbing or inactivating IL-2; and (d) had a markedly reduced anti-H-2b CTLp frequency in the mesenteric lymph nodes but a normal one in spleen. These results indicate that the proliferating Thy-1.2+, Lyt-1+ T cells in MRL/l mice are defective in their responses to mitogenic stimuli, in IL-2 production, and in expression of acceptor sites for IL-2. The relevance of these defects to the MRL/l disease as well as to the role of IL-2 in autoimmunity in general remains to be determined.

Presence of the Philadelphia chromosome (Ph1) in pokeweed mitogen stimulated lymphocytes during chronic phase of chronic myelocytic leukemia (CML).

There has been a lack of direct cytogenetic studies on lymphocytes in the chronic phase of chronic myelocytic leukemia (CML). Peripheral blood mononuclear cells from seven patients in the chronic phase and on in the blastic phase of Ph1-positive CML were cultured in the presence of phytohemagglutinin (PHA) and pokeweed mitogen (PWM). Preparations adequate for analysis were obtained from eight PHA and five PWM stimulated cultures. Cytogenetic studies on the PWM stimulated cultured cells revealed, in one patient, the presence of the Philadelphia chromosome (Ph1) in 23/25 metaphases. The PHA stimulated lymphocytes from the same patient were all Ph1 negative. Lymphocytes from PHA and PWM stimulated cultures from the other six patients in the chronic phase and the one patient in blastic crisis were all Ph1 negative. The one positive result suggests that some B lymphocytes contain the Ph1 and that there is a common hematopoietic stem cell for B lymphocytes as well as the myeloid, erythroid, and megakaryocytic series.

[Postoperative small intestinal resorption after retropubic extravesical bladder neck adenomectomy (Millin)].

For the recommencement of the physiological nutrition after surgical and urological interventions the normalisation of the postoperatively temporarily disturbed gastrointestinal functions is a decisive prerequisite. With the help of the modified D-xylose-test the postoperative resorption of the small intestine was objectified after retropubic extravesical adenectomy of the vesical neck. While at the day of the operation still a clearly retarded and restricted enteral resorption could be proved, already at the first day after operation a vast normalisation of the resorption of the small intestine developed. At the fourth and eighth day after operation sufficient capacities of resorption were at the patients' disposal, so that they again could be unrestrictedly nourished parenterally. The postoperative functional examination for the assimilation of food is suited for the elaboration of scientifically founded directives for the nutrition of freshly operated patients.

[Hepatic coma - experimental study with swine (part 1)].

An acute coma hepaticum was produced in 8 pigs by means of one intraperitoneal dose of thioacetamide. Distinct changes in the behaviour of the animals, changes of the biochemical parameters inclusively the enzyme pattern with a typical lesion pattern of the liver parenchyma and a displacement of the plasma amino acids were shown. All pathological changes in comparison with the normal animal characterize the liver insufficiency after 24 hours as a comatose state. This acute toxic model serves as the base for investigations within the physiology of nutrition about parenteral feeding of L-amino acid mixtures in coma hepaticum. The nearly exclusive hepatotropic lesion degree of thioacetamide is suitable as an experimental model for this purpose.

[Asynchronic changes of absorbing and renal excretion function in aging (author's transl)].

Founded upon experimental and theoretical investigations the age-dependence of the absorbing system and renal excretion function in human being have been analysed. We used a modified D-xylose-test with oral and parenteral xylose-application and mathematical calculation on the base of results of blood-level analysis. Starting from a compartment model the velocity of D-xylose absorption and excretion could be by means of this functional test examined. The velocity of intestinal absorption and renal excretion decrease with age asynchronous.

[Comparative studies of enteral resorption kinetics of D-xylose in humans and experimental pigs].

The enteric transport kinetics of peroral applied D-xylose was comparatively analysed with help of a modified D-xylose absorption test in man and in experimental pig. The animal experimental investigations were carried out in 10 female hybrid pigs at the age of 3 to 8 months. The absorption quickness and the dimension of the effective small intestine absorption in dependence on the time does not show any statistically significant differences between both biological systems. On the other hand the experimental animals show a statistically secured higher elimination quickness of D-xylose in contrast to the comparison group of healthy adults.

[Enteral resorption in the immediate postoperative phase following selective proximal vagotomy].

The postoperative absorption of the small intestine after selective proximal vagotomy was found out by means of the modified D-xylose absorption test in 23 patients on operating day, and on 1st, 5th, 8th, 9th and, 10th postoperative day. The constant of the adsorption speed was reduced on the operating day and decreased further to the 4th postoperative day. Afterwards the preoperative value was obtained approximately. The temporal state of the relative maximum of the curve of serum concentration (the moment tm) occurred in the same time interval. The tm-value obtained the preoperative value approximately. The dimensions of the absorption in the small intestine was reduced by 36% on the operating day, by 20,5% on the 1st postoperative day, and by 10% on the 4th postoperative day on the arithmetical average. The influence of the gastric emptying after SPV and of gastro-intestinal hormones on the process of absorption was discussed.

Lactoferrin: its role as a Ga-67-binding protein in polymorphonuclear leukocytes.

Gallium-67 bound to lactoferrin--an iron-binding protein found in high concentration in polymorphonuclear leukocytes--has been isolated from PMNs that have previously been incubated with Ga-67 citrate. Although the cell-labeling efficiency was highly variable (0.026-10%), much of the activity that did bind to the PNMs (74.8 +/- 10%) was recovered in the supernatant after sonication and centrifugation. About half (approximately 47%) of the PMN-bound activity was retained after dialysis and was presumably bound to macromolecules in the supernatant. When this retained activity was placed on a column containing immobilized antilactoferrin antibody, almost three quarters of the activity was bound to the column. This bound activity was (36 +/- 17%) of the total activity absorbed by the PMN. The addition to the antilactoferrin column of a known antigen-antibody-dissociating agent caused the disolution of the complex. No significant activity was bound to a control column. The findings indicate that lactoferrin is a major Ga-67-binding protein present in PMNs and suggest that it may play a major role in Ga-67 localization in an abscess. These results support the contention that molecules binding ferric iron have an important effect on Ga-67 distribution in vivo.

[Parenteral nutrition - present status and modern trends].

In the present paper practical references for the use of aimed balanced nutrition, particularly of parenteral nutrition, in surgery are given. The complete and total parenteral nutrition is an intervening therapeutic form for the patient. Before the beginning of the nutrition a possibly comprehensive diagnosis of the condition of the nutrition with the help of the state of nutrition is necessary, the volume of which will be depend on the existing diagnostic possibilities of an institution. Moreover, the success and the effectivity of the parenteral nutrition depend on the optimum composition of the nutrient and on a good nutritional technique as well as on an optimum control of the course of nutrition. Taking into consideration the high use for the improvement of the results of treatment a considerable expenditure must be put up with. This means a very strong indication.

Taurine transport by rat hepatocytes in primary culture.

Hepatic taurine concentration is 30--100 times that of plasma, suggesting an efficient taurine uptake mechanism in the hepatocyte. The characteristics of hepatic taurine transport were studied in primary cultures of adult rat hepatocytes. Taurine uptake was concentrative and linear for over 4 h. At taurine concentrations 2.5--100 microM, uptake was saturable with constants Km = 44 microM, V = 0.28 nmol/mg protein per min and EA = 13.2 kcal/mol. Uptake was inhibited 41% by incubation under N2 and was competitively inhibited by beta-alanine (Ki = 94 microM) and hypotaurine (Ki = 14 microM). Uptake was linearly dependent upon Na+ concentration from 0 to 140 mM. A second nonsaturable uptake process was identifiable only at taurine concentrations greater than 1 mM. This process was presumed to represent passive diffusion. At taurine concentrations existing in plasma, taurine enters the hepatocyte primarily by a single, Na+-dependent, carrier-mediated, oxygen-requiring transport process.
