Immunochemical characterization of a band 3-like anion exchanger in collecting duct of human kidney.

Poly- and monoclonal antibodies have been prepared against the cytoplasmic domain (43 kDa) and the 17-, 20-, and 35-kDa fragments of the membrane-spanning domain of the human erythrocyte anion exchanger, band 3. The antibodies were used to localize and further characterize analogues of band 3 in the human kidney. We report here that the basolateral membrane of intercalated cells of the connecting tubules and collecting ducts contains an analogue of band 3 that appears to be highly homologous to the erythrocyte anion exchanger. This band 3-like protein is probably important for reabsorption of bicarbonate in the collecting duct system and thus for acidification of the forming urine. The band 3-like protein of the intercalated cells contain immunoreactive sites of both the cytoplasmic domain and the three major fragments of the membrane-spanning domain of erythrocyte band 3. Although no immunological differences were detected between the membrane-spanning domains of band 3 in erythrocytes and intercalated cells, there are at least three sites along the cytoplasmic domain of kidney band 3 that differ from erythrocyte band 3 in either amino acid composition or posttranslational modifications. The main kidney analogue of band 3 that contains epitopes of the cytoplasmic domain as well as the 17- and 35-kDa membrane-spanning domain of erythroid band 3 is a polypeptide with an apparent molecular mass of 100-110 kDa. Further immunoreactive polypeptides at approximately 180, approximately 140, approximately 38, approximately 25-30 kDa that were detected at lower stringency and higher sensitivity of the immunoblotting procedure may be members of a multigene family that encodes a series of related proteins.

Prognoses about vocational achievement behavior of first admitted psychiatric patients under special consideration of the diagnoses of depressive illnesses.

In this study prognoses about the posthospital vocational achievement behavior of 55 first admitted depressive patients were made at the time of discharge. With a structured and standardized questionnaire the patients themselves, the treating physicians, and the nurses were interviewed. As a first step of analysis the prognoses of the three groups were compared with each other and tested in respect of differences. This comparison was made with other psychiatric diagnoses too, whereby endogenic depressive patients and neurotic depressive patients were discriminated. In the second step of analysis the prognoses of the three 'rater groups' were compared with the real vocational reintegration of 1 year after discharge. The result was that the prognoses of the treating physicians and the nurses were more compatible with reality than those of the patients themselves.

Factors involved in the increase of K+ efflux of erythrocytes in low chloride media.

The rate constant of the Rb+(K+) efflux in isotonic NaCl solutions containing sucrose is increased when the NaCl concentration is reduced. When NaCl is replaced by Na-gluconate no change of the rate constant can be observed. The ionic strength of the solution is discussed as one factor influencing the Rb+(K+) efflux. The addition of furosemide (0.5 mM) inhibits the rate constant of the Rb+(K+) efflux in solutions of different ionic strength by about 50%. The increase of the rate constant in solutions of low ionic strength containing sucrose is interpreted as a stimulation of the chloride-dependent Na,K cotransport and the K,Cl cotransport.

Studies on the embryotoxic risk of exposure to caffeine and ethanol during the preimplantation period in the mouse.

Pregnant mice were exposed before implantation to caffeine and ethanol to determine the dose-response relation for embryolethality during the preimplantation period. For risk estimation the embryotoxicity was evaluated at term and also 24 h after implantation. For ethanol no embryotoxic risk could be detected. Caffeine unexpectedly exhibited a high risk for embryoethality when compared to the maternal LD50. However, when taking into account realistic exposure levels an embryotoxic risk in early pregnancy can be excluded in humans for both caffeine and ethanol.

Leukemia of early infancy. Early B-cell lineage associated with t(4:11).

A case of infantile acute leukemia associated with translocation t(4:11)(q21:q23) is reported. This leukemia has a very poor prognosis, and this patient survived for only 9 months. The blast cell morphology was L1/L2 according to the FAB classification and showed a lymphoid appearance on transmission electron microscopy. The histochemical stains showed a pattern of periodic acid-Schiff positivity and variable alpha-naphtyl acetate staining. The cells were TdT-positive and surface-marker phenotyping was positive for Ia-like and B4 antigens but negative for CALLA, T-cell markers, myelocyte and monocyte markers. The leukemic cells represent a frozen state of a very early precursor, corresponding to the earliest recognizable stage of the B-cell lineage. This observation may contribute to the controversion regarding the cell origin of this unique leukemia associated with t(4:11), lymphatic versus null cell, early myeloid, or mixed, and points to the possibility of a very early B-cell lineage leukemia.

Successful non-neurotoxic therapy (without radiation) of a multifocal primary brain lymphoma with a methotrexate, vincristine, and BCNU protocol (DEMOB).

A 3.5-year-old boy with a multifocal primary lymphoma of the brain was treated successfully without neurotoxicity with a treatment regimen that did not include radiation. The protocol of Dexacort (dexamethasone), methotrexate, Oncovin (vincristine), and BCNU (carmustine) (DEMOB), which was developed with the use of MTX pharmacokinetic studies, was given over 7.5 months, and resulted in tumor disappearance on computerized tomography scans and marked improvement in clinical status. The patient remains in good health 3 years after diagnosis (March 1985).

Predictors for the social adjustment of first admitted psychiatric patients.

In a prospectively designed follow-up study of 258 first admitted psychiatric patients, 1 year after discharge 224 patients and 175 significant others were asked about the social adjustment of these patients and some predictors for this aspect of outcome could be identified. The sample consisted of five different diagnostic groups: organically caused psychiatric diseases, schizophrenic psychoses, affective psychoses, neurotic or personality disorders and alcohol or drug dependency. The study shows that statements about the social adjustment of psychiatric patients largely depend on the diagnostic group, both with respect to degree of adjustment and the predictors. Schizophrenic patients were found to be less well socially adjusted than the other patients, with the exception of the alcohol- and drug-addicted patients. For schizophrenic patients, post-hospital social adjustment was primarily determined by indicators of mental illness, such as psychopathological symptoms and length of hospitalization. The social adjustment of addicted patients was primary influenced by vocational variables. For the patients with organic psychiatric disorders, affective psychoses or neurotic/personality disorders, prediction by pre-hospital or hospital variables did not prove to be very useful.

Periodontal and caries experience in children with insulin-dependent diabetes mellitus.

This study compared the periodontal and caries experience of young patients with insulin-dependent diabetes mellitus (IDDM) with a nondiabetic population of the same age. The plaque scores of children with IDDM were statistically higher. The caries experience of a child with closely monitored IDDM and a family history of diabetes was significantly lower than that of a child with IDDM and no such family history, even though the gingival and plaque indexes of both children were the same.

[Occupational reintegration of psychiatric patients hospitalized for the first time in the year following their discharge--results of a prospective longitudinal study].

A short-term occupational prognosis had been given for first-admission psychiatric patients, occasioned by information from the field that, in case of mentally ill persons, measures for their vocational rehabilitation frequently either are started too late, or are inappropriate in scope, because associated impairments are insufficiently dealt with, underrated, or even overlooked at all in the early stages of the illness. Following Wing's concept of disability, an attempt was moreover made to identify barriers for occupational resettlement. The findings provided the basis for considering the question of scope and type of within-clinic or external measures to be initiated, or stepped up, to facilitate these clients' access to gainful employment and, hence, able to positively influence the future course of their condition.

[Occupational handicaps as risk factors for the readmission of psychiatric patients hospitalized for the first time--results of a prospective longitudinal study].

In dealing with the issue of readmission of psychiatric patients into in-patient care, the focus had in the past primarily been on clinical variables such as diagnosis, or severity of the clinical picture. The present study, however, had centered on occupational variables, such as vocational training, occupational status, periods of gainful employment, and unemployment. This complex was studied in a sample of psychiatric patients, with analyses directed at: percentage of the initial sample readmitted after a catamnestic period of 1 and 2 1/2 yrs. resp., diagnostic groups with increased readmission incidence, and occupational variables associated with the need for readmission. Summing up, it may be stated that occupational handicaps arose in particular in the diagnostic groups of endogenous psychoses, whereas in other groups (such as neuroses, alcoholism) the issue of "social handicaps", in particular, had been in the foreground.

Studies on the embryotoxic risk of drug treatment during the preimplantation period in the mouse.

For risk evaluation of exposure to drugs in early pregnancy the dose-response relationship for embryotoxicity was determined in mice during the preimplantation period using cytotoxic drugs (cyclophosphamide, mitomycin, vinblastine) and therapeutic drugs which are embryotoxic in laboratory animals or in humans during organogenesis (diazepam, doxycycline, phenobarbital, rifamycin, tolbutamide). No malformations but only signs of retardation and a dose related increase in the resorption rate could be detected after treatment with some of the drugs at term even at dose levels close to the maternal LD50 (MLD50). A comparison of the embryolethal dose during the preimplantation period (ELD50) with the MLD50 revealed no risk for the therapeutic drugs, a slight risk for mytomycin and vinblastine and an unusually high risk for cyclophosphamide. The ELD50 for all drugs in this study was higher than the embryotoxic doses during organogenesis which is routinely determined for all drugs. It is concluded that for therapeutic drugs additional testing for embryotoxicity during early pregnancy is not required.

Metastatic neuroblastoma and testicular involvement.

Neuroblastoma metastatic to the testes is thought to be an extremely rare event and is therefore not routinely considered in the evaluation of neuroblastoma patients. A review of metastatic sites in a series of neuroblastoma patients, however, revealed 11 cases with testicular involvement. Analysis of these cases suggests that the phenomenon may not be uncommon in a subset of neuroblastoma patients, namely, male patients with infra-diaphragmatic primaries and advanced-stage disease. Because the testes constitute a sanctuary site for tumor cells, occult testicular involvement may play a role in the relapse of patients judged to be in complete remission. Accordingly, it is proposed that in the above group of patients special diagnostic attention should be directed at the scrotal contents and, if testicular involvement is detected, orchiectomy or local irradiation should be considered.
