Histopathology of early effects of oral cadmium in the rat kidney.

Adult male Wistar rats were given 50 ppm Cd in drinking water over a period of 1-24 weeks. The rats were killed and the cadmium concentration of whole blood, blood plasma red cells, liver and kidneys estimated. The plasma metallothionein concentration was measured by radioimmunoassay. Kidney samples were taken for light, transmission and scanning electron microscopic examination. The accumulation of cadmium in the tissues was shown by a linear increase with time, after exposure for 12 weeks. Plasma Cd concentrations showed a clear increase after 3 weeks and preliminary investigation suggests that most is present as Cd-thionein. Early pathological changes in the rat kidney were seen around the 4-6 week period which coincided with the distinct rise in plasma Cd. At 12 weeks, signs of tubular necrosis, interstitial fibrosis and glomerular epithelial cell hypertrophy were present in small areas of the cortex. By 24 weeks, the renal cortex showed clear evidence of tubulo-interstitial nephritis at a Cd concentration of 60 micrograms Cd/g wet weight.

Netilmicin sulfate prophylaxis in the surgical treatment of renal stones.

The role of short course (three doses) and single dose peroperative antibiotic (netilmicin sulfate) prophylaxis were studied in 42 patients undergoing removal of renal calculi. In an untreated group, the urine sample taken preoperatively was infected in 37.5 per cent, but the removed stone demonstrated significant bacterial growth in 67 per cent, with a wound complication rate of 25 per cent. Use of antibiotic prophylaxis reduced the wound complication rate to 5 per cent. Satisfactory serum levels of the antibiotic were achieved during the operative period, with an acceptable rate of clearance of netilmicin sulfate from the blood stream.

Psychiatric referral after head trauma.

This preliminary study of 22 patients referred for psychiatric consultation after HT requires further investigation to better define the trends observed regarding diagnosis, injury site, and medication choice. The absence of a control population of HT patients not referred for psychiatric evaluation limits our ability to make definitive conclusions. Further work is also planned to systematically explore the injury site-medication relationship.

Experimental penetrating head injury: some aspects of light microscopical and ultrastructural abnormalities.

A high velocity model of penetrating head injury has been developed in the rhesus monkey and a lower velocity model in the baboon. It is apparent that pathological changes are widespread and develop early although the pathogenesis of the diffuse vascular changes is unknown. The present study involved the sampling of grey and white matter from 20 monkeys with high velocity injury, and 10 baboons with low velocity injury together with similar material from a number of normal control animals. 30 minutes after a high velocity injury swelling of perivascular astrocytes was present, sometimes associated with an increase in extracellular fluid. Animals with lower velocity injuries survived for some hours. Astrocytic swelling and perivascular oedema associated with cellular necrosis was frequently found in this group. The pathogenesis of these lesions is discussed.

Renal cadmium content in the West of Scotland.

The trace element content of kidneys has been documented in very few international centres. Significant differences can be demonstrated between the cortical and medullary levels of calcium, zinc and cadmium. For the West of Scotland the values for the latter are similar to those found in Scandinavia. There are significantly higher cadmium contents in subjects who smoke and who have evidence of ischaemic heart disease.

Short course netilmicin prophylaxis in renal stone surgery.

Following surgery for the removal of intrarenal calculi 30% of patients will develop severe wound complications. This study assesses the role of a short course of netilmicin given over the operative period, in 20 consecutive patients. Preoperative studies demonstrated that whilst 20% of patients had bacterial growth in the urine, culture of the removed calculus demonstrated that 50% of the stones exhibited a significant bacterial growth. Of the 20 patients, only one developed a wound complication. Following the preoperative dose, satisfactory netilmicin levels were obtained and no patient had levels greater than 1 microgram ml-1 by 7 h. Despite some degree of renal impairment, serum urea and creatinine values were unaffected.

A prospective study of the development of hypertension and renal stone disease in subjects with increased blood urate.

The results of a prospective study of the development of hypertension and renal stone disease in subjects with increased blood urate are reported and compared with matched controls. None of the 14 patients on Allopurinol developed a renal stone but one untreated control did. For male patients there was a significant difference (p less than 0.01) in diastolic blood pressure between the 2 groups over the 5-year study period, with treated patients having higher blood pressures than untreated controls. This suggests that Allopurinol has had no effect on lowering diastolic blood pressure. Long-term therapy with Allopurinol was effective in reducing mean blood urate levels. It is suggested that blood urate levels are more relevant in predicting renal stone formation and of less value in assessing the therapy and prognosis in hypertension.

The characteristics of post tur blood loss: a preliminary study.

Peroperative and postoperative blood loss was studied in 20 patients. Both peroperative and postoperative blood loss was found to correlate together (p less than 0.05) and with the weight of resected tissue (p less than 0.05). The characteristics of the postoperative blood loss demonstrated a strong negative correlation with time, and calculation of the t50 showed that a 50% reduction in postoperative blood loss occurred within 16 h.

An ultrastructural study of experimental high velocity penetrating head injury.

Experimental high velocity missile brain injury in the rhesus monkey produces widespread swelling of perivascular astrocytes within 30 min of injury. Possible mechanisms for this lesion include a direct effect of force, chemical mediation secondary to the extravasation of blood, alterations in the permeability of the blood brain barrier and ischaemia. The implications of this findings for the function of the blood brain barrier, for neurotransmission and for neuronal survival are discussed.

An unusual complication after excision of a recurrent mucocele of the anterior lingual gland.

A case report of a recurrent mucous extravasation phenomenon in the anterior lingual gland is presented. Multiple surgical procedures were necessary to eradicate the lesion. Alginate impression material was used during one of the surgical procedures to aid in defining the margins of the lesion. A foreign-body reaction arose in the tongue as a result of residual alginate within the tissues, necessitating further surgery. The use of this technique cannot be recommended in areas where extension into surrounding tissue planes is possible.

Co-trimoxazole and trimethoprim in complicated urinary tract infection.

Previous work has suggested that co-trimoxazole may be superior to trimethoprim in the treatment of complicated urinary tract infection. A prospective study has been done to assess the relative value of the drugs in this situation using trimethoprim at higher than normal dosage. Fifty three patients (33 women and 20 men) were randomly allocated to either a fourteen-day course of co-trimoxazole tabs, 2 twice a day (27 patients) or trimethoprim 250 mg twice a day (26 patients). After patient withdrawals from the study, 17 (77%) of the co-trimoxazole group achieved a sterile urine three weeks after starting treatment compared with 15 (65%) in the trimethoprim group (X2 = 0.80). When those patients with sterile urine at three weeks who could be reassessed four weeks later were analyzed, 8 (89%) of the co-trimoxazole patients maintained a sterile urine against 7 (58%) in the trimethoprim group (X2 = 1.09). Although statistical significance was not attained, the results suggest that even at increased dosage, trimethoprim would not appear to be as efficient as co-trimoxazole in complicated urinary tract infection.

Immunocytoma of prostate.

Prostatic involvement by non-Hodgkin lymphoma is rare. Infiltration of the gland by the uncommon plasmocytoid cells sufficient to cause clinical signs and symptoms of prostatic obstruction is described.

Acrosoxacin in the treatment of uncomplicated gonorrhoea.

Acrosoxacin was given as a single 300 mg oral dose to 105 patients with acute gonorrhoea. Of the 100 patients followed completely there was a 93% cure rate and 33% of men developed postgonococcal urethritis (PGU). This compared with a 97% cure and 30% PGU with 2 megaunits penicillin. The in vitro activity of acrosoxacin in Nottingham against strains of Neisseria gonorrhoeae showed that it was a highly active agent with 90% of strains inhibited by a concentration of 0.03 mg/l.

Relative proportions of human hemoglobins A, S, C in individual erythrocytes of heterozygotes.

Inherited electrophoretic variants of hemoglobin in individual erythrocytes were separated by electrophoresis in ultrathin acid agar gels. The electropherograms were stained under a hood with hemoglobin specific chromogen, benzidine. Photomicrographs of the electropherograms were scanned with a recording densitometer. Relative quantities of the respective hemoglobins in individual erythrocytes were estimated by weighing the areas under the densitograph curves. The findings confirm that the intracellular proportions of Hb A and Hb S or Hb A and Hb C in heterozygotes are of heterogeneous distributions within a given population of cells. The tool of single cell electrophoresis may be useful for investigating relative quantities of other electrophoretically separable protein variants in individual erythrocytes.
