Urinary calculous disease in childhood.

Reviewed are 56 cases of children suffering from urinary calculous disease between 1970 and 1977; 27 were female and 29 male. The age distribution was from one month to fourteen years. In 34 per cent of the children urodynamic disorders were responsible for stone formation, 23 per cent had identifiable metabolic disorders, and in 43 per cent the cause of stone formation was unknown. Urinary infection was present in 60 per cent of the patients. Forty-five of the 56 children (80 per cent) had to undergo surgery. After surgery 78 per cent remained stone free, while 16 per cent had recurrent stones, only half requiring a second operation. In cases of active stone disease, especially in the group with underlying metabolic disorders, specific therapeutic measures have been applied and the patients regularly investigated afterward.

Vesicoperineal accessory urethra.

A previously undescribed form of accessory urethra was found in 2 boys. In each the supernumerary urethra originated from the bladder, just anterior to the vesical neck, passed anterior to the prostate and crossed the normal urethra. In 1 case the channel ended blindly and in the other it opened on the posterior aspect of the scrotum.

The persisting mesonephric duct: high junction of vas deferens and ureter.

We describe 9 examples of a rare congenital anomaly in which the unduplicated ureter joined the vas and exhibited long persisting mesonephric ducts that resembled the ureter rather than the vas. Some of the kidneys were unexpectedly well developed. It is proposed that a cranial ectopic ureteral bud may have induced a metanephric kidney in the region of the mesonephros or the ectopic bud and the metanephric blastema were displaced cranially.

A new type of congenital nephrotic syndrome.

Description of a newborn male suffering from hydrops fetalis of unknown cause. Placenta showed multiple chorioangiomata. Coincidence of chorioangiomata of the placenta and hydrops fetalis has not been previously reported. The authors suggest a new kind of glomerulopathy as a cause of congenital nephrotic syndrome.

[New findings in the study of schizophrenia].

The paper gives a survey on the results of research on etiology and course of schizophrenia. The results of etiological research have not, as yet, progressed so far as to provide measures of primary prevention, whereas the results of research into the course of schizophrenia have already led to practical consequences for therapy resp. rehabilitation of schizophrenic patients. In future, with a decrease in the number of long-stay hospitalizations, the number of schizophrenic out-patients, resp. of prolonged periods of out-patient treatments will increase accordingly. Thus hitherto known influencing factors and treatment strategies will also be due to change. According to present knowledge, etiology, psychopathology, course and outcome of schizophrenia represent a very complex and variable, multifactorially initiated process, in which illness-related and environment-related factors concur. Drafts of new models and instruments for the assessment of this process and subsequent disabilities as well as methodological difficulties herewith are reported.

[Suitability of mephentermine for the management of arterial hypotension in pregnancy].

10 late pregnant women with hypotension were examined before and after the intravenous injection of 30 mg mephentermin with the method of quantitative sphygmometry, the bloodless graphic registration of the arterial blood pressure and direct electronic timing of pulse wave velocity. Systolic and diastolic blood pressure increase significantly. The cause of this rise in arterial blood pressure is the increase of stroke volume exclusively. Heart rate and total peripheral resistance remain constant. This hemodynamic mode of action is a favourable one in regard to protection of uteroplacental blood flow. Therefore mephentermin is suitable for treatment of hypotension in pregnancy.

[Experiences with the preventive use of heparin in septic abortion].

Report on 62 cases of septic abortion with prophylactic use of heparin. In two cases during this prophylactic therapy shock develops with simultaneous decrease of blood level of fibrinogen as sign of disseminated intravascular coagulation. The generally accepted value of prophylactic medication of heparin in cases of septic abortion is not diminished by these casuistic observations. Examinations of the coagulation system are necessary at intervals.

[Cardiovascular effect of 15(S)-15-methyl-prostaglandin F2alpha].

The reaction of the cardiovascular system on one intramuscular injection of 250 microgram 15-methyl-prostaglandin F2alpha was examined in 14 normotensive healthy pregnant women between 7th and 11th weeks of gestation with the method of quantitative sphygmometry with unbloody graphic recording of arterial blood pressure and direct electronic determination of velocity of aortic pulse wave. The tests were done in 10 patients in intervals of 5 minutes for one hour and in 4 subjects for 12 hours in intervals of one hour. Systolic blood pressure remains nearly constant, but diastolic blood pressure increases and then decreases significantly. Heart rate decreases significantly. Aortic pulse wave velocity decreases in a characteristic manner. Analogous to the biphasic behaviour of blood pressure cardiac output decreases significantly, but then increases insignificantly. The inverse changes of total peripheral resistance are insignificant. Comparing these reactions with the cardiovascular effects of prostaglandin F2alpha or E2, 15-methyl-prostaglandin F2alpha shows the smallest circulatory alterations.

[Effect on the cardiovascular system of a 3.5% gelatin infusion solution Gelafusal in hypertensive women in late pregnancy].

Using the method of quantitative sphygmometry with bloodless graphic recording of the arterial blood pressure and direct electronic timing of the pulse wave velocity the behaviour of the cardiovascular system in 10 hypertensive late pregnant women was examined before, during and after the intravenous infusion of 500 ml of a 3,5% gelatinous infusion solution (Gelafusal) given within 30 minutes. There is a mild and transient expansion of the plasma volume. Arterial blood pressure, stroke volume, cardiac output and total peripheral resistance remain constant nearly. Consequently the employment of Gelafusal in hypertensive late pregnant women is possible without making worse the arterial hypertension.

[Cardiovascular changes during ethanol-tocolysis].

The cardiovascular effects of an intravenous infusion of a 5% ethanol-10% fructose-solution was examined in 10 healthy late pregnant women with the method of quantitative sphygmometry. There are insignificant changes of the arterial pressure, but increase of stroke volume, cardiac output, work and power and a decrease of total peripheral resistance. In spite of these favourable hemodynamic alterations, particularly the constant heart rate, ethanol should be used only for a short-time-tocolysis.
