[Cardiovascular effect of infukoll 6% on normotensive late pregnancy].

By means of quantitative sphygmometry and the unbloody recording of arterial blood pressure the hemodynamic effect of an infusion of 500 ml dextrane 75 (Infukoll 6%) on the cardiovascular systeme in 10 normovolemic normotensive healthy late pregnant women is examined. Hematocrit decreases as an expression of increasing plasma volume. Arterial blood pressure and heart rate remain unchanged. Stroke volume and cardiac output increase significantly. Simultaneously total peripheral resistance decreases. Therefore dextrane 75 is indicated in pregnancy for therapy and prophylaxis of circulatory conditions with lowered plasma volume.

[Cardiovascular effect of fenoterol (Partusisten)].

8 normotensive late pregnant women in the early latent phase of premature labor were examined before, during and after the intravenous pump infusion of fenoterol (Partusisten) with the method of quantitative sphygmometry, the bloodless graphic registration of the arterial blood pressure and the direct electronic timing of pulse wave velocity. Dose levels of fenoterol were 3 microgram/min and 6 microgram/min. During the administration of fenoterol systolic blood pressure increases insignificantly, but mean and diastolic pressure decrease distinctly with broadening of amplitude. Stroke volume, cardiac output, work and power of the heart increase significantly. The fall of the total peripheral resistance is much more pronounciated than the decrease of blood pressure. Therefore the hemodynamic mode of action of fenoterol is a favourable one in regard to the perfusion of peripheral tissues. The effects on the cardiovascular system in a dose level usual for tocolysis are smaller than those of nylidrin.

[Trichomoniasis in the newborn (author's transl)].

Trichomonas vaginalis was diagnosed in two girls out of 948 newborn babies (0.21%) admitted for various reasons to the Universitäts-Kinderklinik Graz during a 21 months period. One of the patients revealed leukocyturia and vulvovaginitis, the other had both bacterial urinary tract infection and vulvovaginitis being controlled but incomplete with antibiotica therapy. The radiologic finding of the urinary tract were normal. In both cases trichomonas vaginalis was identified in a routine urine specimen. The infants mothers both had vulvovaginitis trichomonalis. After a 5 days course with metronidazole given orally, the symptoms ceased and the urinary tract infection healed. The influence of maternal hormones on the epithelium of the infantile vagina facilitating trichomoniasis in the newborn is discussed.

[Cardiovascular effect of solcoseryl with low molecular dextran in normal late pregnancy].

By means of quantitative sphygmometry and the unbloody recording of arterial blood pressure the hemodynamic effect of an infusion of Solcoseryl in combination with low molecular dextrane on the maternal cardiovascular system in 9 normotensive healthy late pregnant women is examined. Solcoseryl causes no improvement of the cardiovascular effects known for low molecular dextrane.

[Familial nephrotic syndrome with focal glomerular sclerosis (author's transl)].

This is a report about three siblings (one boy and two girls) suffering from a clinically- and morphologically-identical form of renal disease. The disease began in each case with symptomless proteinuria at the age of 3 years and proceeded after several years to the full-blown picture of idiopathic nephrotic syndrome with the rapid development of renal insufficiency. Histologically, minimal proliferative intercapillary glomerulonephritis with focal sclerosis was found in all 3 cases. This condition was resistant to steroid and immunosuppressive therapy. The incidence and the morphological, clinical and therapeutic peculiarities and the prognosis of familial nephrotic syndrome are discussed on the basis of these case reports.

[Cardiovascular effects of prostaglandin F 2 alpha in early pregnancy].

In 10 normotensive healthy early pregnant women cardiovascular studies were done before, during and after the intravenous administration of prostaglandin F2alpha with the method of quantitative sphygmometry. Arterial blood pressure was measured graphically with an automatic sphygmomanometer unit. Velocity of aortic pulse wave was determined directly on the principle of exact electronic timing. Prostaglandin F2alpha was infused with electric pump in the dosage of 6, 5, 13 and 26 mug per minute for 30 minutes in each case. Arterial blood pressure is nearly constant. Heart rate, the elasticity coefficient of the arteries E' and total peripheral resistance decreases significantly. Stroke volume, cardiac output, work and power of the heart increases significantly. Nevertheless there are no contra-indications on the part of cardiovascular system for using prostaglandin F2alpha for induction of abortion

[Cardiovascular effect of prostaglandin E 2 in early pregnancy].

In 10 normotensive healthy women between the 8th and 12th week of pregnancy complete cardiovascular studies were done before, during and after the intravenous administration of Prostaglandin E2 with the method of quantitative sphygmometry, unbloody measuring of the arterial blood pressure and direct electronical timing of the pulse wave velocity in the aorta. Prostaglandin E2 was infused with electric pump in the dosage of 0.65, 1.3 and 2.6 mug per minute for 30 minutes in each case. Arterial blood pressure and heart rate remains constant. Elasticity coefficient E' of the arteries and total peripheral resistance decreases, stroke volume, cardiac output, work and power of the heart increases significantly. These reactions of the cardiovascular system are much more pronounced than using Prostaglandin F2-alpha. Therefore prostaglandin E2 is characterized in relation to prostaglandin F2-alpha not only by a stronger uterine effectiveness, but also at the same time a pronounced influence on the cardiovascular system.

[Influence of low molecular dextran on the acid-base balance in mother and fetus].

9 normal pregnant women in the active phase of labor were treated with 500 ml low molecular dextrane with a pH of 4,5 under standardised external conditions. Before and after this infusion simultaneous estimations of the maternal and fetal acid-base status were made. There was neither a maternal nor a fetal acidosis.

[Cardiovascular effects of nylidrin (Dilatol) in pregnancy].

8 normotensive late pregnant women were examined before, during and after the intravenous administration of nylidrin (Dilatol) with the method of quantitative sphygmometry, the bloodless graphic registration of the arterial blood pressure and the direct electronic timing of pulse wave velocity. Systolic blood pressure remains unchanged; mean and diastolic blood pressure and total peripheral resistance decrease. Simultaneously heart rate, stroke volume and above all cardiac output increase. The hemodynamic mode of action of nylidrin is a favourable one in regard to the perfusion of tissues in pregnancy, because the fall of total peripheral resistance is much more pronunciated than the decrease of blood pressure. Therefore there are no objections to a tocolytic treatment with nylidrin in normotensive pregnant women on the part of cardiovascular system.

[Vaginal radical surgery using the Schauta-Amreich method. Results from the years 1959 to 1970 and consequences for determining indications].

Clinical report on 234 vaginal radical hysterectomies carried out in the years 1959 to 1970 at the department of obstetrics and gynecology of the university of Rostock. 84% 5-year recoveries. No primary operative mortality, low morbidity. 5-year recovery is 76%, if cancer does exceed 20 X 20 mm at the surface and/or 10 mm extent at the depth. If cancer does not exceed this three-dimensional measurement 5-year recovery is 91%. There was concluded firstly that the possibility of metastases in the pelvic lymph nodes is dependent from the size of the primary tumor and secondly that before treatment a subtile diagnostic procedure secures that only good cases of cervical cancer FIGO stage I b are operated on by radical vaginal hysterectomy (Schauta-Amreich). Also in some cases of microcarcinoma a Schauta-operation is indicated. Our proposal is a reduced radicality with regard to the removal of the parametrium in these caces.

[Telemetric ECG control of the heart during the rehabilitation of hemiplegics].

In hemiplegic patients of advanced age the incidence of cardiac disorders, frequently combined with heart failure, is 80%. To assess the effect of rehabilitation on cardiac status 25 patients underwent period telemetric ECG monitoring during the rehabilitation period. The rest forced upon the patient in the initial period following the stroke economizes cardiac effort. The increasing load in rehabilitation thus has a favourable effect on the heart and affords appropriate cardiac training in addition to its immediate purpose.
