[The Carraro syndrome (author's transl)].

The report concerns a girl aged 9 1/2 years who was deaf and dumb and had marked shortening of the calves with deformities of the feet and bilateral, congenital hypoplasia of the tibiae. This syndrome was first described by Carraro in 1931, but there have been no further reports since then.

Opiate receptor binding and analgesic effects of the tetrahydroisoquinolines salsolinol and tetrahydropapaveroline.

Salosolinol and tetrahydropapaveroline bind to opiate receptors in rat brain with affinities of 6.2 and 1.95 x 10(-5)M respectively. Their ability to displace 3H-naloxone is decreased about 4-fold by 100 mM sodium ion. Both of these agents have antinociceptive effects when given to rats intraventricularly. Their potency is comparable to the enkephalins, and their effect is blocked by naloxone.

[Results of treatment of cervical carcinoma at Gynaecological Hospital of Rostock University, 1959 through 1972 (author's transl)].

Results obtained in the treatment of cervical carcinoma at the Gynaecological Hospital of Rostock University, between 1959 and 1972, are reported and compared with the figures given in the Annual Report. The results obtained in Rostock were found to be in key with that international standard. Surgical and radiological therapies yielded somewhat identical results, when applied under optimum conditions to the operable stages I and II.

Evaluation Studies as Topic↗

[Value of routine skull radiography for etiology of simple febrile seizures (author's transl)].

The dramatic episode of a febrile seizure, happening short and generalized with normal children, has a good prognosis. The examiner has to perform a very exact differential diagnosis in which an exact history and physical examination have to be on the first place, followed by serological examinations, the EEG and occasionally a lumbar punction. Skull radiography, as we wanted to show in the paper, has lost its place in general examination and should be reserved for special cases.

The prognosis of idiopathic nephrotic syndrome: a comparative study between the index of selectivity of proteinuria and the findings in renal biopsies.

In 20 children with idiopathic nephrotic syndrome, the morphological changes in the glomeruli were compared with the index of selectivity of proteinuria. For the determination of the index of selectivity, the clearances of IgG and transferrin were used. A high correlation was found between the morphological changes and the index values. An index of selectivity of proteinuria of 0.20 proved to be the critical value for differentiating between a steroid sensitive and a steroid resistant INS.

[Experience obtained from use of endoprotheses in treatment of ureterostenosis (author's transl)].

Reported in this paper is the authors' own experience obtained from treatment of 25 cases of severe ureterostenosis, using plastic endoprotheses (splints), according to a technique proposed by Schmitz and Hegemann, 1966. Ascending infection did not occur at all. The disputed method may be used in exceptional situations to get to grips with postsurgical or postradiological ureterostenosis, a condition not fully avoidable yet. Such exceptions may be claimed for patients to whom uretero-surgery would be an intolerable stress or for those on whom pharmacotherapy, using glucocorticoids and antibiotics, has failed.

[Behaviour of cardiovascular system during obstetric neuroleptanalgesia, using droperidol/fentanyl (author's transl)].

Intravenous injection of 2.5 mg of droperidol and 0,05 mg of fentanyl to ten women with clinically intact cardiovascular system in advanced pregnancy and the cardiovascular effects of such injection were studied by means of quantitative sphygmometry. - Mean arterial blood pressure was found to drop slightly but with significance, while the blood pressure amplitude went up. Heart rates remained largely constant. Significant increase in cardiac output, consequently, resulted solely from increase in stroke volume. Total peripheral resistance was reduced with significance. Medication, using the above combination of drugs, may be recommended for obstetric analgesisa, because of its favourable impact in terms of hemodynamics.

[Circulatory effects of xanthinol nicotinate in advanced pregnancy (author's transl)].

Xanthinolnicotinate is a vasostimulant substance which is recommended to improve blood flow in the treatment of placental insufficiency. Quantitative sphygmometry with non-invasive graphic recording of arterial blood pressure was used in an attempt to study the effects of intravenous infusion of 300 mg of xanthinolnicotinate (Jupal) on the cardiovascular systems of ten women in advanced pregnancy with suspicion of intra-uterine foetal retardation. Their circulation remained basically unaffected, except for transient initial rise in pulse rate and cardiac output as well as decline of total peripheral resistance. However, additional blood supply to the skin and resulting shift in blood volumes may well provide a possibility of uteroplacental hypoperfusion, at least in cases in which perfusion had been restricted before. Administration of xanthinolnicotinate during pregnancy, therefore, cannot be recommended against the background of present knowledge.

[Surgical treatment of cervical carcinoma (author's transl)].

At the Gynaecological Hospital of Rostock University radical abdominal surgery was performed on 321 cases of cervical carcinoma and radical vaginal surgery on 268 cases, between 1950 and 1972. The five-year survival rates were 73.5 or 83.8 per cent, respectively. Indications as well as morbidity and lethality of either approach are discussed, and the authors' personal experience is reported. - The value of lymphography has remained to be disputed. Reasons are given for general renunciation of presurgical irradiation and for sole use of radiological techniques and their limited applicability in postsurgical treatment.

[Primary hyperparathyroidism--bone turnover and osteitis fibrosa assessed by x-ray (author's transl)].

1. In primary hyperparathyroidism an increased bone turnover is seen, accompanied by osteitis fibrosa (= fibroosteoclasia, FO) in severe forms of the disease. Both types of bone reaction may be detected by microradioscopy X-rays of the hand, extensive striation of metacarpal cortical bone indicating increased bone turnover and subperiosteal resorption of phalanges pointing to FO. 2. In the present study 65 patients with proven PHPT were evaluated before and 39 after operation. Microradioscopy was combined with biochemical assessment of hyperparathyroidism including alkaline serum phosphatase (aPh) as an index of osteoblastic activity, hydroxyprolin excretion (HyPro) reflecting bone turnover, immunoreactive parathyroid hormone levels (PTH), serum calcium (SCa), urinary calcium (UCa), serum inorganic phosphorus (SP) and clearance of phosphate (Cp). A comparison was made with the incidence of renal stone disease and the degrees of metacarpal striation (StG) and subperiosteal resorption (UG) were followed after operation. 3. Preoperative X-rays of 60% of the PHPT subjects showed increased StG and/or UG, and in 41,5% the diagnosis of PHPT was possible from the X-ray findings only. There existed a significant correlation between StG and UG on one hand and aPh, HyPro and PTH on the other. No correlation, either positive or negative, was seen between FO and the incidence of renal stones. After surgery, subperiosteal bone lesions disappeared in all patients, while intracortical striations persisted in half of the subjects despite the normalised bone turnover. Thus, primary hyperparathyroidism may not only lead to endosteal bone loss but to an irreversible intracortical bone deficit as well.

[Clinical Investigations about the behaviour of pulmonary respiration and cardiovascular circulation after the application of Rohypnol for basis-sedation for microsurgical operations in otolaryngology in local anesthesia (author's transl)].

The influence of analog-sedation on pulmonary respiration and cardiovascular circulation caused by the action of pentazocine/flunitrazepam respectively in microsurgical operations under local anesthesia in otolaryngology is investigated in two collectives of patients. The results are presented and the hazards, especially the hypoxemia and its avoidance are discussed.

[The haemodynamic principle of action of diazoxide (hypertonalum) in the treatment of acute hypertensive crises in late pregnant women (author's transl)].

In ten hypertensive late pregnant women with passing crises of hypertension cardiovascular analysis with the method of quantitative sphygmometry, the unbloody recording of the blood pressure and the direct electronic measuring of the pulse wave velocity were made before and after the intravenous injection of 300 mg Diazoxid (Hypertonalum). This drug is recommended to the treatment of severe preeclampsia and eclampsia because of lessening the arterial blood pressure and the total peripheral resistance, increasing the heart rate, stroke volume and cardiac output.

[The value of magnesium sulfate in intensive care of severe toxemias of pregnancy (author's transl)].

A view on the employment of magnesium sulfate in pregnancy is given and the statement of the cardiovascular changes before, during and after the intravenous infusion of 4,0 g magnesium sulfate in ten hypertensive late pregnant women. The drug induces a mild decrease of arterial blood pressure with a distinct increase of stroke volume, cardiac output, heart rate, heart work and the heart's power. The pathologically increased total peripheral resistance is reduced. These hemodynamic effects are very favourable concerning intensive care of severe toxemias of pregnancy.

Observations on the Leydig cells in the male East African spring hare (Pedetes surdaster larvalis).

The morphology of Leydig cells of the testis of sexually mature and sexually immature spring hares was studied. The cytoplasm of the Leydig of cells the sexually immature spring hares was packed with large lipid droplets leaving little space for the other organelles. Smooth endoplasmic reticulum was poorly developed and occasionally formed concentric layers of fenestrated cisterns around the large lipid droplets. The Leydig of cells the sexually mature spring hares were almost devoid of lipid droplets and their cytoplasm was occupied by abundant tubular smooth endoplasmic reticulum. Cells which shared characteristics with both immature Leydig cells and undifferentiated mesenchymal cells were observed in the limiting membrane of the seminiferous tubulus. These Leydig-like cells may play a role in the differentiation of Leydig cells in the spring hare.

[Dilatol for hypertension therapy in pregnancy (author's transl)].

Dilatol (Nylidrin), a betamimetic used for tocolysis, was applied to ten hypertensive women in advanced pregnancy and produced in them characteristic effects with favourable impact upon oxygen supply to the foetuses, such as significant lowering of diastolic pressure, decrease in total peripheral resistance which reflected increase in uteroplacental blood flow, as well as rises in heart rate, stroke volume, and cardiac output. Nylidrin, therefore, may be recommended as a hypotensive drug in the treatment of arterial hypertension in pregnancy.

Micromorphology of the skin (epidermis, dermis, subcutis) of the dog.

Grossly, the skin of the dog is characterized by varying thicknesses, density of the hair coat and the implantation angle of the hairs. Multiple folds varying in shape are formed by the epidermis and the papillary layer of the corium. The epidermis consists of a stratum profundum with a continuous layer of basal cells, a stratum spinosum consisting of 3-6 cell layers, a stratum superficiale with single granulosa and "lucidum" cells and a well-developed stratum corneum. The corium shows a narrow stratum papillare corli containing delicate fibres and a broad stratum reticulare corii with mainly coarse fibre bundles. The subcutis is divided into adipose and fibrous layers. The broad adipose layer displays breed and individual differences which vary with the region of the body. The fibrous layer corresponds to the connective tissue layer, formerly described as fascia trunci superficialis. The morphological and functional unity of the corium and subcutis is discussed.
