Hemodynamic adjusted treatment of hypertension in pregnancy. Part II: Hypotensive drugs.

If in late pregnancy hypertension persists after hypovolemia and disturbances of the microcirculation have been corrected, treatment with hypotensive drugs may be indicated. Because the mode of action of these drugs varies, we tried to put this treatment on a rational basis. Due to the different effects on uteroplacental blood flow and the fetus, only those substances should be used which cause a decrease in total peripheral resistance. In our view the uteroplacental flow increases in spite of the lower pressure level. We could prove these beneficial effects for magnesium sulfate, dihydralazine, diazoxide, and nylidrin. Thus during pregnancy these are the drugs of choice.

[Molestation of patients and personnel in noise-exposed gynaecological hospital (author's transl)].

Noise levels were measured in the Gynaecological Hospital of the University of Rostock. Patients, medical doctors, and nurses were interviewed on their own molestation. The equivalent permanent sound level, Leq, was 63.5 dB (AI) on average in patient rooms on daytime, while the average night value was 59.8 dB (AI). Maximum sound pressures, Lmax, varied between 55 and 80 dB (AI) in patient rooms. An Lmax of 85 dB (AI) was measured on a ward corridor, when instruments were cleaned in the wash-basin of a service room. Molestation by outdoor noise was reported by 88 per cent of 85 patients interviewed, while 29 per cent felt disturbed by indoor noise. The noise was perceived as disturbing also by the personnel. While the gravest role for the hospital investigated by played by outdoor noise, greatest attention should be given also to the control of indoor noise.

The effect of long-term HCG or TP administration on reproductive functions in adult male rabbits. I. Histomorphological changes in the scrotum.

The effect of long-term treatment with various doses of HCG or TP on the scrotum was studied in 36 adult rabbits of pure Bouscat breed. The epidermis of the scrotum exhibited a more significant dose-related increase in thickness in animals injected with TP than those treated with HCG. The hair follicles in animals injected with higher doses of HCG or TP were thicker in diameter and extended deeper within the dermis where they curved to grow parallel to the surface of the skin. The high dose of HCG was more effective in demonstrating higher glandular activity than that seen in rabbits treated with high doses of TP. The vascularization in the scrotum was more prominent in rabbits treated with higher doses of HCG. The fibers of the Tunica dartos were much thicker in animals treated with high doses of HCG or TP.

A simple method of realizing the deconvolution of ultrasonic images.

A readily implementable procedure for improving the lateral resolution of ultrasonic images is proposed. The starting point is the approximation of the sound-beam profile by model functions which arise from the convolution of rectangular functions of varying width. From these model functions it is possible to state analytically the Fourier transforms and their reciprocals. These reciprocals can be allocated to a chain circuit of recursive space frequency filters of the first degree, whereby a single basic element corresponds to the rectangular approximation and still produces a resolution improvement by a factor of 1.5. For noise suppression a low-pass limitation function is introduced whose inverse Fourier transform is mathematically similar to the sound-beam approximation; however, as compared with this, it is compressed by a specific factor and thus is optimally matched to the expected resolution improvement.

[Cardiovascular action of dipyridamole in advanced pregnancy (author's transl)].

Quantitative sphygmometry was used to check the cardiovascular action of an intravenous injection of 30 mg of dipyridamole in ten women, between the 31st and 38th weeks of pregnancy. Circulation was intact in all of them, but there was suspicion of intra-uterine foetal retardation. Moderate decline in arterial mean blood pressure was paralleled over a short period of time by increase in heart rate, stroke volume, and cardiac output. Overall peripheral resistance dropped with significance and caused, with some probability, increase in uterine perfusion. Hence, dipyridamole proved suitable for intra-uterine reanimation.

[Hypertension and hormonal contraceptives].

After several years the intake of hormonal contraceptives causes a slight increase of the arterial blood pressure in the non-hypertensive region in all women. The incidence rate of real hypertensions is 1.8 to 3 times as high as users, compared with the non-users. The cause of the increase of blood pressure is not exactly known. The estrogen-induced changes of the renin-angiotensin-aldosterone-system are most probable. Conclusions for the first prescription, the frequency of controls of blood pressure and the behaviour in the appearance of a hypertensive reaction are reported. It is particularly warned of antihypertensive therapy and permanent intake of the "pill".

[The cardiovascular effect of diisopropylaminhydrochloride (Disotat) in non-pregnant hypertensive women].

With the help of the quantitative sphygmometry and the unbloody graphic registration of the arterial pressure the hemodynamic effect of diisopropylamine hydrochloride (disotat) was investigated in 10 non-pregnant hypertensive patients. After a rapid intravenous application of 100 mg diisopropylamine hydrochloride (DIIPA) within 10 minutes after injection a statistically significant decrease of the systolic pressure by 15%, of the diastolic pressure by 9% and of the arterial medium pressure by 15% developed. Thus the blood pressure amplitude was restricted by 25%. Connected with the decrease of the pressure was a significant decrease of the stroke volume by 22% and of the cardiac output by 19%. The total peripheral resistance increased statistically uncharacteristically by 13% in comparison to the initial value. The courses of the curves show that in all parameters during the 45-minute observation duration the initial values are striven for, however, without reaching them in every case. Thus the decrease of pressure induced by bolus injection of DIIPA independent on the existence of pregnancy is always accompanied by a decrease of the output volumes of the heart and the increase of the total peripheral resistance.

[Initial aspects of the changes in diabetic proteinuria].

Proteinuria was analysed quantitatively and qualitatively in 156 diabetics and 63 matched controls. The study was limited to patients with a proteinuria of less than 100 ng/min in the recumbency. The urinary proteins were analysed using cellulose acetate electrophoresis, immunoelectrophoresis and sodium dodecylsulfate poly-acrylamide gel electrophoresis. Abnormal urinary protein patterns were more frequent in diabetics than in the controls (p less than 0.01). However, when the subjects were divided into 2 groups according to their age, the limit being arbitrarily chosen at 60 years, the differences were statistically not significant in the older group. By contrast, in the younger group, the proportion of perfectly normal patterns was significantly decreased in diabetics as compared to the controls (p less than 0.005). In our diabetic population, chosen on the basis of a quantitatively normal proteinuria, no clear relation could be found between the abnormalities of the urinary protein electrophoretic patterns and the patient's clinical data, with the exception of vascular and cardiac complications. Our results suggest that qualitative changes of urinary proteins might be the first signs of renal complications in diabetic patients and that diabetes might constitute an additional cause of aging for the kidneys.

[On the diagnosis of schizophrenia (author's transl)].

After a review of terms used in the description of schizophrenia, mainly based on Bleuler's and Kurt Schneider's terminology, attempts to operationalize the diagnosis of schizophrenia are discussed. First rank symptoms according to Kurt Schneider have been most extensively used as criteria. Classification based on these symptoms is, however, subject to certain restrictions from cross-sectional as well as longitudinal points of view. Additional criteria are necessary to describe the whole spectrum of what is called schizophrenia in clinical psychiatry with regard to better therapeutic measures. Symptomatology is also the leading criterion for the diagnosis of schizophrenia in the discussed multidimensional schemes. With an increasing number of criteria the diagnosis of schizophrenia will be confined according to the applied variables.
