Pancreatic choriocarcinoma presenting as inflammatory pseudocyst.

A primary pancreatic choriocarcinoma was discovered at surgery in a patient with a clinical diagnosis of pancreatic inflammatory pseudocyst. To our knowledge, this is the first example of an extragonadal choriocarcinoma reported as a primary at this site. We consider this neoplasm to be another example of a visceral choriocarcinoma arising from a primary carcinoma through a process of metaplasia. This case illustrates the inherent difficulties in the preoperative evaluation of cystic lesions of the pancreas, and the importance of recognizing endocrinologic manifestations of malignant disease.

Globular Mallory-like bodies in renal cell carcinoma: report of a case and review of cytoplasmic eosinophilic globules.

The case of a patient who had renal cell carcinoma with cytoplasmic eosinophilic globular hyaline bodies antigenically similar to Mallory bodies, highly suggestive of globular type III alcoholic hyalin, is presented. Mallory bodies are not found exclusively in the liver, having also been demonstrated in the lung. The renal tumor in this case represents an additional extrahepatic site.

Fibronectin-containing hyaline globules in malignant melanoma.

Hyaline globular inclusions were found in a case of malignant melanoma. The globules were both intracellular and extracellular in the primary neoplasm and in metastases to lymph nodes, liver, and lung. They were studied immunohistochemically and were found to contain fibronectin, a glycoprotein of high molecular weight. Ultrastructurally, the globules were composed of electron-dense, finely fibrillar aggregates. It is suggested that the neoplastic cells produced the fibronectin, which accumulated in globular form. Ten additional cases of malignant melanoma were studied; none of these had hyaline globules or intracellular globular fibronectin. The globules were compared to similar structures described in Spitz's nevi and other neoplasms. Immunohistochemical analysis was most useful in distinguishing the globules found in the malignant melanoma from those found in other conditions.

Relationship of ultrasound findings after cesarean section to operative morbidity.

Postcesarean section febrile morbidity remains a common problem on obstetric services. Although a few preliminary studies have reported on the use of ultrasound to assess the postcesarean section patient, they have not compared sonographic findings with either intraoperative or postoperative events. These reports have noted fluid collections around the incision site. In this report of 100 postcesarean section patients, it is noted that echo-free areas anterior to the incision site, which correspond to the described fluid collections, were present in 29% of patients and were more frequently found in patients with excess blood loss at surgery. Patients whose echo-free areas were greater than or equal to 3.5 cm were significantly more likely to have postoperative morbidity. The significance of these findings and the role of ultrasound in the evaluation of the postcesarean section patient are discussed.

The effect of deoxymannojirimycin on the processing of the influenza viral glycoproteins.

Deoxymannojirimycin (dMM) was tested as an inhibitor of the processing of the oligosaccharide portion of viral and cellular N-linked glycoproteins. The NWS strain of influenza virus was grown in MDCK cells in the presence of various amounts of dMM, and the glycoproteins were labeled by the addition of 2-[3H]mannose to the medium. At levels of 10 micrograms/ml dMM or higher, most of the viral glycopeptides became susceptible to digestion by endoglucosaminidase H, and the liberated oligosaccharide migrated mostly like a Hexose9GlcNAc on a calibrated column of Bio-Gel P-4. This oligosaccharide was characterized as a typical Man9GlcNAc by a variety of chemical and enzymatic procedures. Deoxymannojirimycin gave rise to similar oligosaccharide structures in the cellular glycoproteins. In both the viral and the cellular glycoproteins, this inhibitor caused a significant increase in the amount of [3H]mannose present in the glycoproteins. Deoxymannojirimycin did not inhibit the incorporation of [3H]leucine into protein in MDCK cells, nor did it affect the yield or infectivity of NWS virus particles. However, its effect on mannose incorporation into lipid-linked saccharides depended on the incubation time, the virus strain, and the cell line. Thus, high concentrations of dMM showed some inhibition of mannose incorporation into lipid-linked oligosaccharides with the NWS strain in a 3-h incubation, but no inhibition was observed after 48 h of incubation. On the other hand, the PR8 strain was much more sensitive to dMM inhibition, and mannose incorporation into lipid-linked oligosaccharides was strongly inhibited when the virus was raised in chick embryo cells, but less inhibition was observed when this virus was grown in MDCK cells. Nevertheless, in these cases also, the major oligosaccharide structure in the glycoproteins was the Man9GlcNAc2 species.

Social and psychological differences between cancer and noncancer patients: cause or consequence of the disease?

83 female patients with breast tumors were interviewed prior to biopsy. The interview contained a psychological instrument measuring action control as an indicator for the patients' reaction to stress together with questions about the expected diagnosis. Using multiple-regression analysis we estimated the explained variance of the variables relating to the histological result of the biopsy. Since most of the patients gave a correct prognosis of the nature of their disease-this variable proved to be very important-most of the social psychological findings have to be interpreted as consequences rather than causes of cancer.

Optically clear endometrial nuclei.

The ultrastructure of optically clear endometrial nuclei is presented. These nuclear alterations have been found in spontaneous abortion, term pregnancy, endometriosis, and uterine choriocarcinoma.

[Preventive appendectomy--pro and con].

The pros and cons of incidental appendectomy on the occasion of gynecological laparotomies are discussed. It is possible to prevent the well known morbidity and mortality of appendicitis later on by removing the appendix. The rate of complications is elevated only insignificantly. Incidental appendectomy is proposed under special preconditions.

[Significance of serum uric acid determination for the early detection of complications of hypertensive pregnancy in primigravidae].

In 100 healthy primigravidae the concentration of serum uric acid was examined in a prospective longitudinal study between the 12th and 40th gestational week. Serum uric acid increased continuously and was elevated significantly in 9 women who developed toxaemia of late pregnancy at the 16th, 32nd, and 36th week of pregnancy. If serum concentration are higher than 220 mumol/l or 241 mumol/l in the 32nd gestational week they were considered for a increased risk of developing a hypertensive complication. Sensitivity, specificity and the predictive value of a "positive test" were low in the 16th (22 or 20%) and 32nd (16 or 17%) week of pregnancy. The predictive value of a "negative test" was relative high in the 16th and 32nd week with 93 or 90% respectively 95 or 92%.
