Facial myokymia--a false localizing sign in obstructive communicating hydrocephalus. A case report.

Facial myokymia was evident in a 57-year-old man exhibiting both clinical and radiological signs of obstructive communicating hydrocephalus. Insertion of a ventriculoatrial shunt resulted in resolution of clinical symptomatology including the facial myokymia. Since there was no evidence of a structural brain stem pathology, it is believed that in this case, facial myokymia represented a false localizing sign secondary to obstructive communication hydrocephalus.

Nitrous oxide ameliorates spasmodic torticollis.

In a case of therapy-resistant spasmodic torticollis administration of nitrous oxide (N2O) resulted in a dramatic transient amelioration of the dystonic movements. We suggest that manipulations of the opioid system may be useful in the therapy of this condition. Furthermore, our results with N2O may provide insight into the pathogenesis of the disease.

Efficacy of trazodone in narcolepsy.

A 25-year-old man with narcolepsy and cataplexy experienced partial relief of symptomatology following administration of methylphenidate. Moreover, the latter caused extreme agitation and aggression. However, administration of the antidepressant agent trazodone resulted in almost complete alleviation of the narcoleptic and cataplectic attacks within 48 h after initiation of therapy. Trazodone, a novel antidepressant agent may be useful in the long-term management of individuals suffering from narcolepsy.
