Total external ophthalmoplegia was observed in a patient following oral administration of phenytoin, although the state of consciousness was relatively unimpaired and the plasma phenytoin level was within the recommended therapeutic range. This report demonstrates the potency of phenytoin to act selectively on the vestibulo-ocular system. Recognition of such association may be important as regards further investigations aimed at excluding structural brainstem lesions.
A 47-year-old man of Irish descent was severely disabled by idiopathic spasmodic torticollis. Sodium valproate (Epilim) and baclofen (Lioresal) in combination provided marked relief maintained for over 6 months. No adverse or toxic effects were recorded during treatment. This is believed to be the first report in the literature on the efficacy of this drug combination in the treatment of idiopathic spasmodic torticollis.

A 50-year-old man presented with bilateral deafness and ataxia of sudden onset and without constitutional symptoms or signs of meningeal irritation. He was subsequently proved to have meningococcal meningitis, and the deafness and ataxia resolved following appropriate antibiotic therapy.

A man with typical symptoms of lithium intoxication had, in addition, severe and persistent oculogyric crises, which cleared completely during recovery. Previous reports have demonstrated that lithium may impair basal ganglia activity, but this is the first reported case of oculogyric crises induced by lithium carbonate intoxication.
Internuclear ophthalmoplegia (INO) is characteristic of lesions of the medial longitudinal fasciculus and usually indicates primary brain stem pathology. Here we describe 2 cases of tuberculous meningitis (TBM) in which INO was a prominent presenting sign. The mechanism of INO in TBM is presumed to be ischemia secondary to involvement of small basilar vessels supplying the MLF in the inflammatory process. In both cases the INO resolved on antituberculous therapy suggesting that the cause of the ischemia is reversible. INO in a patient with meningitis must suggest a basal process probably TBM.
A 58-year-old woman with long-standing hemifacial spasm experienced dramatic relief of symptomatology while receiving baclofen (Lioresal) in a daily dosage of 37.5 mg. Clinical response was noted within 48 h and she remained asymptomatic during 12 months of treatment. No toxic or side-effects were observed. Controlled studies are needed to establish the role of baclofen in hemifacial spasm.
