Carcinoembryonic (CEA)--like antigen in adenomas and cancers of the breast in human patients.

By using specific anti-CEA serum and indirect immunofluorescence, a substance was detected in breast cancers which reacted similarly to CEA demonstrable by the same method in cancers of gastrointestinal tract. However, the double diffusion test in agar gel and additional absorption analysis showed that it was a glycoprotein different from CEA, although possessing common or similar antigenic determinants with it. This antigen also differs from another antigen called "nonspecific cross-reacting antigen NCA" and from alpha 1-acid glycoprotein of human serum.

Distribution of the crossed olivocochlear bundle in the chinchilla's cochlea.

The efferent crossed olivocochlear bundle (COCB) was transected in the brain stem of the chinchilla, and the animals sacrificed 7 to 96 days later. Electron microscopy revealed that all the large efferent nerve endings on outer hair cells in the basalmost 2 mm (round window region) of the cochlea had degenerated, 87.5% in the remainder of the first turn, 70% in the second turn and 43% in the third turn. Only a few degenerating nerve fibers were seen in the medial spiral tract (inner spiral and tunnel bundles) of the experimental animals. Nerve fibers were counted in the medial spiral tracts of the cochleas of control animals as well as in those animals whose COCB had been transected. There were considerable individual variations in the fiber numbers, and statistical analysis showed no significant difference between the numbers of nerve fibers in normal and experimental animals. The cochlear microphonics (CM) and nerve action potentials (AP) of acute animals were assessed before and after COCB section. The CM and AP of the chronic experimental animals were compared with responses from normals. Overall, no changes in a physiological response of the anesthetized chinchilla could be attributed to complete section of the COCB.

[Effectiveness of prolonged uterine relaxation by betamimetic drugs for threatened premature labour (author's transl)].

It is statistically proven that long-term administered betamimetic drugs inhibit excessive uterine activity, but for the individual subject the extent of this effect can hardly be predicted. By studying 167 pregnancies treated for threatened premature labour it is confirmed that the success rate of the tocolytic treatment depends strictly on the clinical feature of the condition as observed on admission. By use of a simple scheme for classification of success, correlations with the importance of the syndrome at start of the treatment as well as with the neonatal outcome are established. These correlations thus allow one to determine individually the neonatal chances even at onset of the tocolytic treatment.

[Determination of the severity of mitral stenosis by hemodynamic and echocardiographic parameters].

M-Mode and two-dimensional echocardiographic examinations were performed in 70 patients with pure or prevailing mitral stenosis. Mitral valve excursion, mitral valve opening area (MVOAe) and diastolic E-F slope were determined and compared with the gradient across the valve and the opening area obtained during cardiac catheterization. Mitral valve excursion and E-F slope showed mean values of 1.71 cm and 1.98 cm/sec. respectively and were indicative of a stenosed mitral valve. Correlation between E-F slope and gradient with MVOAe was poor. The correlation coefficient was r = +0.56 and r = 0.34 resp. MVOAe compared favorably to the mitral valve area determined at cardiac catheterization (r = + 0.96) and the gradient across the mitral valve (r = 0.90)9 We conclude: 1. Determination of the mitral valve opening area by means of two-dimensional echocardiography represents a valuable addition in the assessment of the severity of mitral stenosis. 2. M Mode echocardiography indicates the presence, but not the severity of mitral stenosis. 3. Computerized planimetry is superior to the manually planimetered opening area and represents a reproducible, exact and time-saving procedure.

[Routine EEG examinations accompanying lithium therapy over two years (author's transl)].

In 15 lithium-treated psychotic patients, the following variables were examined over two years at 6-monthly intervals; seru lithium levels, serum electrolytes, thyroid function, urinary creatinine and urea, and clinical EEG. The mean serum lithium level was 0.64 mval/l and did not change during the two years. When compared to the pretreatment EEG, visual analysis did not indicate any specific potentials, focal signs, or asymmetrical phenomena such as have been reported in the literature. Only at the beginning of treatment could a small increase in unspecific, abnormal changes be demonstrated, which, however, decreased during the following 18-month period of investigation.

[Parturition under peridural anesthesia (author's transl)].

Basing on a study of 205 "matched pairs", it is shown to what extent peridural anesthesia (PA) influences the course of parturition, the type of termination of birth, and the intrapartal and post-partal condition of the child. The method of PA application is accurately described in the section "Material and Methods". The first stage of labor, or period of dilatation (PD) is not in any way effected by PA. However, PA will almost double the expulsion period (EP). Hence, elective termination of birth via technically simple, complication-free outlet forceps is usually propagated and employed significantly more often. Emergency sections or delivery by forceps due to mandatory indication, are not more frequent under PA than otherwise. The intrapartal condition of the child, measured with the cardiotocogram employing in the score according to Hammacher et al. (6) before and after application of PA, as well as during the last 30 minutes of birth (final score), is influenced only irrelevantly. The Apgar scores one minute and five minutes post partum, as well as the arterial umbilical cord pH values, do not differ significantly between the peridural and the control groups. The authors agree with the widespread opinion that, at the present moment, PA is the best obstetric analgesic method with simultaneous maximum possible safety for the fetus.

[Value of the abdominal electrocardiogram for cardiotocography (author's transl)].

The value of abdominal Electrocardiogram (abd. ECG) for cardiotocography has been studied prospectively in 183 ante- and 93 subpartum patients. The indications for using abd. ECG have been worked out and the influence of various factors on the quality of registration were tested. It was found that neither location of the placenta nor obesity nor the state of the membranes influenced the quality of abd. ECG-records. Furthermore abd. ECG was used successfully in cases with advanced labor.

[Use of a computer for the preparation of medical history screening in cardiovascular diseases].

Questionable data may be regarded as significant indicators for a screening. It is the aim of our investigations to develop a screening standard anamnesis in form of a questionaire which allows on the scale of larger groups of population to establish the number of endangered persons and patients who probably suffer from cardiovascular diseases but of the quantity of apparantly healthy persons. In order to recognize regularities of the diagnostics coordination of the individual patient by his answers on the questionnaire the data of 400 diagnostically clarified persons (model experiment) underwent a discriminancy analysis. When the data of unknown test persons are given in, on account of the well-known functions of discriminancy a classification in the groups persons with heart and circulatory diseases or with a healthy heart and circulation or a group diagnosis can be performed, respectively. The evaluation of the data of unknown test persons of a pilot study in 96% resulted in a correct classification in patients with heart and circulatory diseases or persons with healthy heart and circulation, the classification in the above mentioned groups of diagnosis was performed on an average to 57%.

[Successful treatment of a supraventricular re-entry tachycardia in a patient with WPW and sick-sinus syndrome with permanent rapid atrial and QRS-inhibited ventricular pacemaker stimulation (author's transl)].

A supraventricular re-entry tachycardia was successfully treated in a patient with WPW and sick-sinus syndrome by permanent rapid atrial pacing and ventricular demand pacing. The use of a demand pacemaker with bipolar electrode avoided slow ventricular rates resulting from second degree A-V block during rapid atrial pacing. The trigger system of this pacemaker was not disturbed by unipolar atrial stimulation. Ventricular rates responded satisfactorily to physical and to emotional stress as a result of improved A-V conduction in these circumstances. Rapid atrial pacing has prevented the re-entry tachycardia. In an emergency the atrial pacemaker can be inhibited by a magnet, but otherwise discharges permanently at a constant rapid rate.

Evaluation of success in treatment of threatening premature labor by betamimetic drugs.

For prevention of premature labor the betasympathicomimetic drugs buphenin, fenoterol, and ritodrine were administered to 135 patients intravenously for 1 week and afterward orally up to the end of week 37 of gestation. No satisfactory evaluation of success could be obtained by analyzing only the time gained by the treatment. Two supplemental parameters were needed. In addition to the previously introduced Tocolysis Index (TI) which describes the obstetric situation at the beginning of the treatment, a Prolongation Index (PI) was defined transforming the lag time gained by the treatment into a relative measure. By means of this PI, extremely differing cases could be compared, though the gestational ages at the start of treatment and the lag times between admission and delivery showed wide variation. In addition, satisfactory statistical correlations between the PI and the clinical signs as well as the neonatal outcome could be computed, indicating the PI to be a suitable parameter for the estimation of success.
