The frequency of postmenopausal endometriosis (ovarian endometriosis and adenomyosis) was 2.2%. The mean of the menopausal ages in 11 patients with ovarian endometriosis was 50.3 yr and the average time elapsed since menopause, 7.3 yr. The corresponding values in 8 patients with adenomyosis were 52.1 and 8.8 yr. Carcinoma was a common associated finding in patients with ovarian endometriosis. Increased estrogen activity was observed more frequently in patients with adenomyosis than in those with ovarian endometriosis. Only one of the patients had received estrogen therapy. Hormone-producing tumors in the ovaries or adrenal glands were not confirmed. 70% of the patients were obese and the signs of increased estrogen activity could be explained by extraglandular estrogen formation.

Serum E1, E2 and E3 concentrations and E2/E1 ratio were measured after vaginal application of conjugated oestrogens, micronized 17 beta-oestradiol and oestriol. 2.4 mg of conjugated oestrogens caused a prompt elevation in the serum E1 concentration; the E2 level changed only slightly. After vaginal application of 2 mg micronized 17 beta-oestradiol the main serum oestrogen is E2 and the conversion of E2 to E1, as in oral administration, does not occur. A significant elevation in the serum E3 concentration was noted 2 h after the vaginal application of 0.5 mg oestriol. The E2/E1 ratio changed little after the application of conjugated oestrogens but increased considerably after the vaginal administration of 2 mg micronized 17 beta-oestradiol.
The effect of estradiol valerate (Progynova, 2 mg daily) on the renin--aldosterone system was studied in nine women after bilateral oophorectomy. Plasma renin activity (PRA) and the daily urinary excretion of aldosterone (dU-Ald) were determined 4 mth after the operation -- during which no estrogen was given -- and after 3 mth on estrogen therapy. Plasma estradiol concentration rose in 7 subjects to the normal postovulatory level. No changes were found in PRA or dU-Ald. The results suggest that an effective estradiol valerate therapy is not accompanied with activation of renin--aldosterone system, while synthetic and non-human natural estrogens (Premarin) were shown earlier to increase PRA.
Serum oestrone, oestradiol and oestriol concentrations were studied during intramuscular Primogyn Depot (10 mg of oestradiolvalerate) and Dimenformon prolongatum (2.5 mg of oestradiolbenzoate + 10 mg of oestradiolphenylpropionate) treatments. The serum oestrone concentrations were markedly elevated in both therapeutic groups 24 h after the injection. After the Primogyn Depot injection the serum oestrone concentration 10 days after the injection was still significantly higher than the pretreatment level. The serum oestradiol concentrations of both groups were markedly elevated 24 h after the injections and were significantly higher than the pretreatment concentration 10 days later. The oestriol values were low during both treatments.
Of 903 surgically treated patients with endometriosis the disease recurred in 132 cases (14.6%) during the follow-up period of 6-10 years. The re-operation was performed within 1 year after the primary operation for 7 patients, within 1-5 years for 91 patients and more than 5 years after the primary surgery for 34 patients. The most common complaints were pain in the lower abdomen and dysmenorrhoea and the most common sites for the recurrence the ovaries and retrocervix. Endometrioma of the rectum was surgically treated in 10 patients and endometrioma of the urinary bladder in 3. Gestagen therapy after the primary surgery had no effect on the recurrence rate.

The etiology of premalignant cervical lesions was investigated anamnestically in teenagers. The material comprised of 54 patients (mean age 18.0 years, range 12--19 years) treated in 1973--77. Early coital practise and frequent change of sexual partners were usual. Previous abortions and frequent genital infections were also typical. The rising frequency of dysplasias in young age groups calls for increased general knowledge and information even among teenagers and, if possible, mass screening.
Chlamydia trachomatis (C.tr.) is recognized as an aetiological agent in most cases of male nonspecific urethritis, post-gonococcal urethritis, and epididymitis. According to the recent view, C.tr. is considered capable of causing infertility in women, but up to date the significance of C.tr. in male infertility is unknown. According to our study, chlamydial genital infection seems to play a role in male infertility, but in lesser degree than in females. Male chlamydial infection seems to be less intense and more "distal" than female infection. The infection is apparently often quite symptomless. The incidence of genital chlamydial infection in male population evidently increases with age and promiscuity at least from the age of 20 to 30 years.
Oestrogens increase plasma renin activity (PRA) by inducing synthesis of renin substrate in the liver. This is proved clinically by the synthetic oestrogens of oral contraceptives and by the equine conjugated oestrogens in postmenopausal therapy. The effect of endogenous oestrogen production on the activity of the renin-aldosterone system is not clear, but progesterone is an important stimulus of renin release in the luteal phase of menstrual cycle. We studied 15 premenopausal women undergoing bilateral oophorectomy. Eight women had high mean serum oestradiol level 1.2 +/- 0.6 (SD) nmol/l for follicular cysts or for a prolonged climacteric proliferation phase. Seven women had normal follicular oestradiol level (0.2 +/- 0.1 nmol/l). In PRA or daily urinary aldosterone excretion (dU-Ald) no differences could be found between the groups. In one month after oophorectomy no changes in PRA or dU-Ald were found in either group, while the serum oestradiol fell to unmeasurable levels. The results suggest that endogenous oestrogens are not to cause activation of the renin-aldosterone axis as measured by the clinically useful PRA and dU-Ald. The result is in agreement with previous reports of postmenopausal therapy with oestriol succinate or oestradiol valerate.
During the years 1968--1972, 903 patients with endometriosis were operated on in the Turku University Central Hospital, of whom 136 were found to have adenomyosis (endometriosis interna). The diagnosis of adenomyosis was usually made post-operatively from following examination of uterus; only 15% were suspected to have adenomyosis preoperatively. 24% of the patients also had external endometriosis. The symptoms and the clinical finding were analysed, but the results were quite unclear and were often easier to explain in terms of other simultaneous diseases or of menopausal disturbances. Adenomyosis, unlike external endometriosis, did not cause sterility. A study of the aetiology showed that only 6% had had no pregnancies. Adenomyosis is a benign disease with little clinical significance. Its symptoms are unclear and it is usually underdiagnosed, but it may actually appear in more than 50% of parous women over the age of 40. In this study, the only aetiology of adenomyosis appeared to be pregnancy and/or delivery. Finally it is suggested that adenomyosis may be a more or less physiological finding in parous women, with the highest frequency of occurrence in the female population over the age 30 years when childbearing is generally over.
