Increased occurrence of ovarian pregnancy.

Forty-one cases of ovarian pregnancies diagnosed during 1946 and 1980 were analysed clinically and histologically. All of the pregnancies were terminated during the first trimester. The average duration of amenorrhea was 36.4 +/- 28.6 days; fifteen of the patients (37%) had no amenorrhea. The gestational sac was implanted at the ovarian surface in 30 cases and deeper in the cortex in 11 cases. A close connection with the corpus luteum was noted in 26 cases, with 8 cases showing placental tissue inside the corpus luteum. Endometriosis was found in the affected ovary in 2 cases and acute or chronic inflammatory changes in 4. During the 1970's the number of ovarian pregnancies diagnosed was four times as great as in the 1960's. The share of ovarian pregnancies out of all ectopic pregnancies in this decade was 5.4% and that for the 1960's was 1.5%. Of the patients with an ovarian pregnancy diagnosed in the 1970's, 61% had an IUD in situ at the time of the operation. In addition 61% of these patients had a history of a gynecological operation during a previous pregnancy.

Tuberculous peritonitis simulating peritoneal carcinosis.

Two cases are reported exemplifying the difficulties faced in the clinical diagnosis of peritoneal tuberculosis. Two fertile-aged nulliparous females were admitted with symptoms and signs of an acute abdomen. Both showed a relative intestinal obstruction, abdominal mass and ascitic fluid. A malignant disease was suspected and laparotomy was performed. Tuberculous peritonitis was demonstrated histologically in biopsy and later confirmed by positive culture for tubercle bacilli. In the first case the correct diagnosis was disclosed during operation by frozen section, although the histological picture also indicated possible carcinosis because of a heavy mesothelial hyperplasia. A 9-month chemotherapy with isoniazide and rifampicin, supplemented during the first 2 months by streptomycin or ethambutol, was successful in both cases.

Bilateral oophorectomy and plasma renin substrate concentration.

Plasma renin substrate concentration was studied in 11 premenopausal women subjected to bilateral oophorectomy and hysterectomy. Preoperative plasma renin substrate concentration was 1573 +/- 477 ng angiotensin I/ml (mean +/- SD). Plasma renin substrate concentration did not change significantly after operation (1632 +/- 459 ng angiotensin I/ml) or after 3 months of postoperative estradiol valerate treatment (2 mg/day) (1762 +2- 467 ng angiotensin I/ml). It is concluded that non-pregnant levels of endogenous estrogens are of no significance in the regulation of plasma renin substrate.

Serum concentrations of oestrone, oestradiol and oestriol during various oestrogen treatments.

Serum oestradiol/oestrone ratios were measured during various oestrogen treatments in castrated women. Oral oestriol succinate therapy (8 mg/day) caused little change in the pre-treatment oestradiol/oestrone ratio. During oestradiol valerianate therapy (2 mg/day) serum total oestrogens and the E2/I1 ratio were considerably increased. One day after the injection of 10 mg of oestradiol valerianate and 2.5 mg of oestradiol benzoate + 10 mg of oestradiol phenylpropionate the E2/E1 ratio was similar to the ratio in middle of the normal ovulatory cycle. The change in serum oestriol was rather small after oral doses of 8 mg og oestriol succinate 15, 30 and 120 min after the application.

Conservative treatment of urinary incontinence in women with special reference to the use of oestrogens.

Urinary incontinence was treated conservatively in 100 patients. The follow-up period was 12-24 mth. For post-menopausal women, the oestrogen therapy consisted of oral oestradiol valerate or vaginal oestrone sulphate combined with emepronium bromide. In post-menopausal patients the best results were noted when incontinence had begun at the menopause and when the duration of the complaint was not more than 3 yr. For pre-menopausal patients, the treatment given was generally emepronium bromide. During the follow-up period 15 of the patients, 11 of whom were post-menopausal, became symptomless and 77 improved; that is, the incontinence was only slight and occasional. The treatment was without any effect in 8 of the patients. Oestrogen therapy was successful in most post-menopausal women. In these patients, the best results were obtained when the duration of the incontinence was not more than 3 yr.

The use of ultrasonography in the diagnosis of ectopic pregnancy.

Ultrasonography was used to examine 196 cases of suspected ectopic pregnancy. The final diagnosis was established in 185 cases by means of surgical intervention or follow-up. The actual number of ectopic pregnancies was found to be 42, of which 30 (71%) had been correctly diagnosed by ultrasound. There was a false negative rate of 29%. Of the group of 185 patients followed up, a false positive diagnosis had been made in six cases (4.2%). On the basis of these results, we conclude that, while ultrasonography offers significant help when a non-invasive diagnostic method for ectopic is desired, it is not yet, in spite of the present level of technical development, a completely reliable method in such cases.

Binding of estrogen and progestin in the human fallopian tube.

The estrogen and progestin receptor concentrations in the human fallopian tube have been investigated. The binding of estrogen and progestin to their cytoplasmic and nuclear receptor proteins were of high affinity and specificity. The cytoplasmic and nuclear receptor concentrations in the isthmus, ampulla, and fimbria did not differ significantly. The nuclear progestin receptor concentrations in the isthmus and fimbriae were significantly higher (P less than 0.05) in the follicular phase than in the luteal phase of the menstrual cycle. In the postmenopausal tubes the receptor concentrations was low, whereas the progestin receptor concentration was quite high.

Effect of medroxyprogesterone acetate and d-norgestrel on plasma renin activity, antidiuretic hormone, and urinary aldosterone in postmenopausal women.

The effect of intramuscular injection of 1,000 mg medroxyprogesterone acetate on plasma renin activity and antidiuretic hormone was studied in 9 postmenopausal women. There were no significant differences in the plasma renin activity and antidiuretic hormone values before and after the injection. 8 postmenopausal women received d-norgestrel (0.18 mg/day) during a 3-week period. The therapy had no effect on plasma renin activity, antidiuretic hormone, or aldosterone excretion.

Von Willebrand's disease and pregnancy.

Seven new cases of pregnancy combined with von Willebrand's disease have been reported. Altogether the patients had 11 deliveries, and one abortion after 10 weeks' gestation. The diagnosis was based on the determination of the bleeding time, factor VIII coagulant activity (VIII C), factor VIII-related antigen (VIII RAg) and the ristocetin cofactor (VIII RCoF). Two of our 6 parturients showed profuse bleeding at delivery; in addition one patient showed bleeding after abortion at 10 weeks' gestation. Vaginal delivery is generally safe in patients with von Willebrand's disease. There is a great risk of copious bleeding especially in connection with early abortion, when factor VIII levels are low.

Diagnosis and treatment of primary vaginal carcinoma in situ and dysplasia.

Fifteen cases of primary carcinoma in situ or severe or moderate dysplasia of the vagina have been diagnosed and treated during the period 1966-75 at the Department of Obstetrics and Gynecology, Turku University Central Hospital. Because of abnormal vaginal cytology the lesion was localized by means of colposcopy and iodine staining. For 6 patients local estrogen treatment was applied to increase the thickness of the epidermal layer. For 6 patients, treatment consisted of simple excision. Nine patients, including both those with carcinoma in situ, were treated with intravaginal radiation therapy. The radiation dosage was 2 000 to 2 600 rad at a distance of 1 cm from the surface of the epithelium. None of the patients showed any complications, and all are now symptom-free.
