Hysterography in evaluating the uterine cavity in patients with endometrial carcinoma.

In 49 patients with endometrial carcinoma hysterographic study was performed. This finding was correlated with fractional curettage, hysteroscopy and surgery. The best correlation was noted between hysterography and hysteroscopy. The flow into the tubes or abdominal cavity using low pressure was found in 20% of the cases. In our opinion hysterography is a useful aid when examining the uterine cavity and cervical canal, and, thus, important when planning treatment and determining clinical stage.

Chlamydial serology in infertile women by immunofluorescence.

The chlamydial serum immunoglobulin G antibodies of 128 infertile women were determined by a single-antigen (L2) immunofluorescence test. The antibody titers were compared with those of female contacts of men with nonspecific urethritis and with those of "normal" pregnant women. Among infertile women, 43% lacked chlamydial antibodies (titer less than or equal to 8). The corresponding proportion for contacts of men with nonspecific urethritis was 14% and that for pregnant women 71%. A very high titer (greater than or equal to 256) was recorded in 25 infertile women and in 7% of pregnant women. Among women with positive chlamydial isolation, 46% had titers greater than or equal to 256. Bilateral tubal obstruction was demonstrated in 23 infertile women (18%). The geometric mean titer of these patients was higher than that of patients with normal hysterosalpingograms. In nine cases with bilateral tubal obstruction and very high chlamydial antibody titers, there was no history of pelvis infections. The operative findings were typical of previous salpingo-oophoritis.

Methods for evaluating the intrauterine location of carcinoma.

The intrauterine location of endometrial carcinoma was investigated with curettage, hysterography and hysteroscopy in 83 patients. These three methods are complementary. Curettage provides important information about the differentiation of the tumor. Hysterography is suitable especially for elucidating the anatomy of the cervical canal and uterine cavity. Direct examination by hysteroscopy gives a reliable picture when defining the boundary between stages I and II.

Pregnancy and delivery after conization of the cervix.

Of 327 patients who had undergone conization of the cervix in 1968-74 in the Department of Obstetrics and Gynecology, Turku University Central Hospital, 249 replied to a questionnaire. Eighty-nine of these had had total of 112 pregnancies after conization. Conization had only minimal influence on the pregnancies and none on the deliveries. Over 90 per cent of the newborns delivered were full term and without anomalies. The incidence of spontaneous abortions also did not differ from normal.

Treatment of vulval epithelial lesions by pulsed high-frequency therapy.

The effect of pulsed high-frequency therapy has been investigated in 25 patients, who had various chronic vulval lesions resulting in continuous, annoying pruritus, smarting or pain, resistant to conventional therapy. The beneficial effect of pulsed high-frequency therapy was either definite or good in 80% of all the cases.

Uterine and ovarian estrogen receptor levels in climacteric women.

High affinity cytoplasmic estrogen receptors in the endometrium, myometrium and ovary of 15 climacteric women were studied. In addition, the concurrent serum estradiol and progesterone level of each woman was estimated and the endometrium examined histologically. The cytoplasmic estrogen receptor level of the endometrium and myometrium had remained extremely high in some cases several years after the menopause and in the presence of a completely atrophied endometrium. The lowest endometrial and myometrial estrogen receptor levels in pre-menopausal women were measured towards the end of the menstrual cycle. The eodometrial estrogen receptor level was roughly 2--3 times the comparable myometrial level. Estrogen receptors were also encountered in all cases in the cervical myometrium. The estrogen receptor levels of the ovary were low in all cases.

Pregnancy and small cell carcinoma of the uterine cervix.

A fatal case of a 22-year old pregnant patient with small cell cervical cancer is reported. The fulminant course of the disease was attributable more to the histological type of the tumour than to the pregnancy. Delay in diagnosis as a result of the pregnancy created a problem emphasized, by difficulty of cytological examination due to the very small size of the cancer cells.

Abdominal gynaecological emergencies in the surgical unit.

We have studied two types of emergency cases that occurred in the surgical unit who were found to have gynaecological aetiology: 1) patients with the so-called "morbus acutum dexter", i.e. appendicitis, or a gynaecological disease and 2) late complications in patients who had earlier benign gynaecological disease, usually such as tumour and surgery for it. The material with seven years follow-up consisted of 97 patients, who were treated during the periods 1959--60 and 1969--70. The relative numbers in both groups increased during the latter period. Of the patients in the reproductive age with suspected appendicitis, gynaecological diseases--mostly ovarian tumours with complications--constituted 7.6% of patients who had undergone appendicectomy in the surgical unit during the former period and 9.6% during the latter. The late complications consisted mainly of small bowel obstruction, resulting from adhesions caused by previous gynaecological surgery. The time interval between initial surgery and the obstruction was rather long, 3.5 years on the average. In the surgical unit the surgery was carried out in about two thirds of these patients. Our analysis demonstrates the necessity of recognizing cases with gynaecological aetiology as a significant factor in surgical practice; it has to be taken into consideration both in surgical and gynaecological education.

Appendicectomy during pregnancy.

Twenty-four appendicectomies were performed during intra-uterine pregnancy at the Department of Surgery of the University Central Hospital, Turku, in the period 1963--77. Six out of 14 cases of appendicitis occurred in the third trimester. The incidence of appendicitis during pregnancy was 1 : 1 445. In 9 cases the appendices removed were normal. The diagnostic difficulties were greatest during the final phase of pregnancy. The location of palpation tenderness in these cases differed from that in the cases with a normal appendix. Fever was generally slight and was sometimes completely absent. The appendix had perforated in 2 cases in both of which premature delivery occurred on the third postoperative day. The infant died in one of these two cases because of immaturity. The pregnancy of the other patients continued uneventfully to term. Appendicitis and appendecectomy performed in good time generally do not involve risk for the continued progress of pregnancy. Progesterone or isoxuprine may have beneficial effect on the course of the pregnancy.

Total serum cholesterol, triglycerides and phospholipids during the normal menstrual cycle.

Ten healthy women (mean age, 29 years) with regular menstrual cycles of 26 to 29 days were tested for levels of total serum cholesterol, triglycerides, phospholipids, and estradiol three times during one menstrual cycle-during menstruation, at ovulation, and in the luteal phase. All studies were performed simultaneously. Lipid variations during the menstrual cycle were minimal. Total serum cholesterol, triglycerides, and phospholipids did not correlate with changes in the serum estradiol levels.

Carcinoma in situ cervicis uteri: diagnosis, treatment and prognosis.

A total of 252 patients with carcinoma in situ colli uteri were treated at the Women's Clinic at Turku University between 1964 and 1971. Seventy-seven percent of the patients came to the clinic for treatment after they had participated in a mass screening program (cytodiagnosis, colposcopy). Extrafascial hysterectomy was performed most often on women in the older age groups, while most of the younger patients were treated by conization. Patients were followed up for a minimum of 5 years. There was a 1.0% recurrence of invasive carcinoma and a 4.0% recurrence of noninvasive carcinoma. All invasive recurrences occurred after hysterectomy. None of the patients died from complications of carcinoma.

The effect of estriol succinate therapy on plasma renin activity and urinary aldosterone in postmenopausal women.

The effects of estriol succinate (Synapause, 2 mg daily) on the renin-aldosterone system and blood pressure (RR) were studied in 14 postmenopausal women after bilateral oophorectomy. Plasma renin activity (PRA) and daily urinary aldosterone excretion (dU-Ald) were determined 1 mth after the operation and before estrogen treatment, at the end of 2 mth therapy, and, for the third time, 2 mth after the termination of treatment with the drug. No changes in PRA, dU-Ald or RR were found in normotensive women, or in 3 women with hypertension in this group. Another group of 11 postmenopausal women was investigated after long-term estriol succinate therapy, which had lasted for 5-8 yr after oophorectomy. PRA, dU-Ald and RR were measured during treatment and 2 mth after terminating the therapy. No changes were found either in hormone differences in the mean levels of PRA or dU-Ald. The results suggest that estriol succinate is devoid of general harmful effects on the renin-aldosterone system during postmenopausal therapy for climacteric symptoms.

Serum oestriol, oestrone and oestradiol concentrations during oral oestriol succinate treatment in ovariectomized women.

Serum oestriol, oestrone and 17 beta-oestradiol concentrations during oral oestriol succinate treatment were investigated in ovariectomized women. Either a single dose of 8 mg or two doses of 4 mg were given daily. With the second divided dosage the serum oestriol levels remained uniform. The oestriol concentrations were clearly higher than in the beginning or in the middle of the normal menstrual cycle in the fertile woman. With both treatment schemes the ratio E3/(E2 + E1) was clearly higher than before treatment and during the normal menstrual cycle. Oestriol succinate treatment lowered the ratio E1/(E2 + E3), which was rather similar to the one during normal menstrual cycle.
