A case of advanced gastric cancer with long-term survival as the results of combined radiation therapy and chemotherapy.

A case report on a long-term surviving patient with advanced gastric cancer with supraclavicular lymph node metastasis treated by radiation and chemotherapy is presented. The Borrman type 2 of advanced gastric cancer was found on the greater curvature of the antrum at the first radiological examination. Radiation was administered to the supraclavicular lymph node at 60 Gy and to the stomach at 64 Gy. The patient received mitomycin C (24 mg) and Tegafur (230 g). After completion of the combined therapy the endoscopy revealed an irregular mucosal change with erosion and hemorrhage. Radiological examination revealed atrophic and hyperplastic areas throughout the stomach. These findings lasted more than six years. The patient died of unknown cause in February 1983. She had survived nine years and seven months after her initial diagnosis. Radiotherapy may play a role as a means of radical treatment in certain cases of advanced gastric cancer.

Increased urine level of amino-terminal peptide derivatives of type III procollagen in patients with liver diseases.

The amino-terminal peptides of type III procollagen (PIIIP) in the urine of 40 patients with various liver diseases were determined with a commercial radioimmunoassay kit. The level of urinary PIIIP (uPIIIP) was correlated well with serum PIIIP (sPIIIP) in 9 patients, the coefficient of correlation being r = 0.836 (p less than 0.01) and the regression line being y = 1.42x + 24. Urinary PIIIP consisted of at least 4 different molecular species with molecular weights of 49 k, 18 k, 10 k and 4.6 k as estimated by column chromatography on Sephadex G-100. Furthermore. uPIIIP was found to be significantly elevated in acute hepatitis, chronic hepatitis, liver cirrhosis, hepatocellular carcinoma and other liver diseases, in which the elevation of sPIIIP has been reported by others. The mean values +/- standard deviations of uPIIIP were 44.0 +/- 32.0, 60.4 +/- 32.0, 62.0 +/- 46.5, 53.0 +/- 27.1 and 48.1 +/- 22.8 ng/ml for the respective liver diseases, and 13.2 +/- 4.5 for the non-hepatic disease group.

Anti-HBs antibody of normal human subjects predominantly binds 54 K and 60 K dalton HBs polypeptides.

The structure of hepatitis B virus surface antigen (HBs) recognized by anti-HBs antibody was analyzed by western blotting using anti-HBs sera obtained from normal subjects, from rabbits immunized with purified HBs and commercially available goat serum. The HBs used had 7 components of 24 K, 27 K, 33 K, 36 K, 39 K, 43 K and 67-72 K daltons. Goat anti-HBs serum bound all of these components, while human and rabbit anti-HBs sera bound only two components (60 K and 54 K daltons), which were hardly visible in the gel even by silver staining. Mixing the 24 K and 27 K components, and the 24 K and 43 K components without reducing reagent produced several polymerized forms of HBs components including 60 K and 54 K polypeptides, which were recognized by anti-HBs rabbit serum. Other combinations of HBs components did not yield any new polymeric forms. Thus, it was concluded that the formation of anti-HBs antibody in normal subjects might predominantly require an antigenic structure of polymeric forms of specific combinations of HBs polypeptides, other than previously known antigenic determinants.

Long-term survival in patients with small cell lung cancer.

Of the 66 patients with small cell lung cancer (SCLC) who were treated by combination chemotherapy with or without radiation therapy from July 1978 to December 1983 at the National Cancer Center Hospital, Tokyo, 12 (18%) survived over two years and nine (14%) have remained disease-free over three years. The two-year survival rates were compared according to the patient characteristics of sex, performance status (PS), extent of disease, histologic subtype, regimen of the initial chemotherapy and response to treatment. Sex, extent of disease and response to the initial chemotherapy were the most important prognostic factors. The prognosis for patients with liver or bone metastasis was poor. All disease-free survivors, except for two patients who were treated by surgical resection after chemotherapy, achieved complete response (CR) with chemotherapy with or without radiation therapy. Eleven of the 12 two-year survivors achieved CR. Because of the small number of patients in our study, it will be necessary to evaluate further the influence of prognostic factors in patients with SCLC in future studies.

[A new TV-bronchoscopy method using a video endoscope and digital processing of the endoscopic color image].

The TV-bronchoscope dose not have an image fiber bundle, which is instead replaced by a monochromatic C.C.D. (charge-coupled device). This system consists of a bronchofiberscope, a computer, a TV camera, a frame memory and a color CRT display. The enhancement of an endoscopic color image is one typical example of color image processing. Firstly a color image is divided into luminance information and color information, and only the former is processed for enhancement. The luminance information is filtered so as to eliminate the fiber bundle structure, and the spatial frequency band characterizing the structure is then emphasized. Finally the processed luminance information and the non-processed color information are combined to reconstruct the color image. In this way, a processed color image can be obtained without changing the color balance of the original image. Using this process, blood vessels and surface irregularities are emphasized by the newly developed digital processing method. We have applied it for localization of hilar-type early lung cancer, which progresses on the surface, and have obtained good results.

Evaluation of Talbot's safety zone of infusion volume and osmolality in infusion therapy for decompensated liver cirrhosis.

Problems with infusion therapy for correcting fluid and sodium imbalance in decompensated liver cirrhosis (DLC) were investigated by establishing the safety zone of Talbot et al. for parenteral fluid therapy in 4 DLC patients infused with over 900 ml of fluid each day for at least 9 days. The safety zone was different in each case. The safe infusion volume decreased and the safe electrolyte concentration shifted to a lower osmolality when there was ascites with renal failure than ascites without renal failure. Infusion therapy was performed without deterioration of the water and sodium balance in those patients whose infusion volume and fluid osmolality were in the safety zone. In contrast, ascites retention increased and peripheral edema appeared in patients whose infusion volume and osmolality were out of the safety zone. Therefore, the safety zone should be determined repeatedly during infusion therapy.

[Postoperative prophylactic irradiation of lung cancer].

Eighty-three patients with resected lung cancer were studied to determine whether postoperative prophylactic irradiation improves survival. All patients had mediastinal lymph node metastases and underwent relative curative surgery. Thirty patients received postoperative irradiation, and another 52 were followed without further treatment. The 5-year survival rate was 19.2% in the surgery alone group and 20% in the postoperative irradiation group. No increase in survival time was noticed in the postoperative irradiation group. The 5-year survival rate was not influenced by histologic types, presence or absence of subcarinal lymph node metastases and right or left thoracotomy in either group.

[Laser acupuncture therapy for pain and vague complaints in neurosurgery].

The value of diode laser acupuncture for pain and other vague neurological complaints has been studied in chronic patients of head trauma and of cerebrovascular disease or neuralgia in head and neck regions. Gallium-Aluminium-Arsenide (Ga-Al-As) diode laser pain attenuator PANALAS-4000 manufactured by Japan Medical Laser Laboratory was used in the following specifications; wave length: 830 nm, power: 20-40 mW, frequency: 5-50 c.p.s., pulse duty: 0.9. The laser acupuncture was applied to some of the representative Chinese meridian points, local pain points and related nerves for 1 to 7 times in a week. It was given in 310 patients in our neurosurgical department. 175 of 227 patients who complained of pain and 12 of 39 patients who complained of numbness showed improvement within 24 hours, another 24 patients with pain and 16 patients with numbness showed improvement after 24 hours. No side effect was noticed. The duration of the effectiveness was about 2 or 3 days. It is very useful to apply the laser acupuncture for pain and vague complaints in neurosurgery.

[Cases of head and neck carcinoma detected by sputum cytology--lung cancer screening and head and neck cancer].

Seven cases of head and neck carcinoma are presented. They were first noticed by sputum cytology, which was ordered to check lung cancer because of hemosputum (three cases), or in a general check-up of asymptomatic individuals. None of them had lung cancer, and three were found to have piriform sinus carcinoma, two mesopharynx carcinoma, and two vocal cord carcinoma. All seven cases were histologically diagnosed as squamous cell carcinoma. Their clinical stages were as follows; T 1 N 0, three cases; T 1 N 1, one case; T 2 N 0, two cases; T 2 N 1, one case (UICC 1978). These cases remind us that positive sputum cytology is not only a sign of lung cancer, but also of other upper aerodigestive tract malignancies. Sputum cytology may be useful for checking asymptomatic head and neck cancer patients, though it is still too costly and time-consuming to be applied to mass screening.
