Falciparum malaria in seamen.

Three cases of Plasmodium falciparum malaria in seamen, all acquired while working off tropical West Africa, and all in patients coming in for treatment at a New Orleans hospital during one six-week period, are described in the context of the importance of considering recent travel history for arrival at the correct diagnosis and treatment. Two of the three patients whose cases are reported had taken some form of malarial chemoprophylaxis during their voyage.

Imperforate anus and colon calcification in association with the prune belly syndrome.

Two patients with the prune belly syndrome demonstrated colon calcifications and anorectal malformations. Bladder outlet obstruction was present in both cases. Calcifications were also found in the renal collecting system and bladder of one patient. No fistula was demonstrated between the genitourinary tract and bowel in either infant at autopsy. The calcification in the colon and urinary tract is probably secondary to stasis.

Focal sclerosing glomerulopathy with adverse effects during pregnancy.

Exacerbation of focal sclerosing glomerulopathy (FSGN) during pregnancy was noted in the three patients. All had antecedent asymptomatic proteinuria. Toxemia developed in two of the women during pregnancy and progressed rapidly to renal failure. Severe nephrotic syndrome developed in one patient with pregnancy and remitted after delivery. These cases suggest a deleterious effect of pregnancy on the course of FSGN and indicate the necessity for doing renal biopsy in women of childbearing age with asymptomatic fixed proteinuria or nephrotic syndrome so that those patients with FSGN can be properly counseled about future pregnancies.
