[Detection of secretory immunoglobulin A in cervical secretion and in cervicovaginal irrigation fluids].

Secretory immunoglobulin A (S-IgA) is an essential part of local immune system of female genital tract. Accordingly the estimation of S-IgA in female genital secretions is important for judgement of immunologic problems in female genital tract. The estimation of S-IgA was carried out by means of an antiserum against human secretory component (anti SC) and S-IgA standard. It was used the single radial immunodiffusion according to Mancini and co-workers. Our results refer to changes in the quantity of S-IgA locally produced in the mucous membranes of female genital tract correlated to the menstrual cycle. The essential results are a decrease of S-IgA in midcycle in cervical mucus and vaginal fluids as well as of fertile and sterile patients.

[Value of hydrotubation in the treatment of sterility].

In the Department of Obstetrics and Gynaecology of the Wilhelm-Pieck-University Rostock 215 women were treated with altogether 510 hydrotubations because of tubar sterility. 154 patients with altogether 392 hydrotubations could be analysed. Hydrotubations were performed ambulatory, praeovulatory, using the pertubation device by means of double ballon cathether partly in combination with the Speck-test. 35 of the 154 patients treated because of tubar sterility by means of hydrotubation and conception may be suspected in 5 cases. The rate of complications following hydrotubation is 1.5%. The use of hydrotubation can be recommended because of the technically easy administration and the wide therapeutical scale (cleaning of the tube, fimbriolysis, antiphlogistic effect on the tubal mucosa, stimulation of ciliar activity, desobliteration, neuroreflective induction of ovulation) especially as an additional treatment in the field of diagnosis and therapy of the pathological tubal factor.

[Use of no-protein blood extract for infertility and sterility (author's transl)].

In vitro studies into the effects of various concentrations of Solcoseryl (Solco, Basle, Switzerland) on selected asthenozoospermia patients have shown rise in motility in 26 per cent of the probands and decline in motility in 17 per cent. Spermatozoan motility was most strongly increased by a five per cent concentration of Solcoseryl.--Fifty-one patients, from whom different spermiographic findings had been earlier obtained, received ten injections of one ampoule of Solcoseryl daily, with weekend pauses inserted. Included were 46 oligozoospermia, 31 asthenozoospermia, and eight hypospermia. Spermiographic checks were undertaken on termination of therapy, and others followed two weeks, four weeks, three months, and six months after termination of therapy. They all gave improved findings, which were most unambiguous with regard to the number of spermia.--Findings were at least temporarily normal after treatment in 58 per cent of the patients with asthenozoospermia. The wives of eleven in 44 patients (25 per cent) with Solcoseryl treatment were pregnant after three months, as established in follow-up examinations. Artificial insemination was applied to 45 patients, among them 37 with maritogenic approach and eight cases with donogenic approach, using Solcoseryl. Eleven pregnancies were obtained from maritogenic insemination. Pregnancy was achieved in 38 per cent of the probands who received cryopreserved donor semen. These findings would not justify the claim of direct conception-producing action for Solcoseryl, in the context of insemination treatment.

[Serum concentration of pregnancy-associated alpha 2-glycoprotein (alpha 2-PAG, protein of pregnancy zone, PZ) in patients with dermatological diseases (author's transl)].

Radial immunodiffusion, according to Mancini, was applied to 15 hospitalised patients, both during and after hospitalisation. They were of both sexes and all afflicted with dermatological diseases (psoriasis, eczema, mycosis). Determination of serum concentration of pregnancy-associated alpha 2-glycoprotein (alpha 2-PAG, PZ) was the purpose of the tests. While in the acute phase of the disease the PZ serum levels of the three groups of patients were higher than those recorded from a control group, conventional therapy failed to have any impact upon PZ serum concentration. Continuous decline in PZ serum concentration was recordable from psoriasis patients in response to photochemotherapy (PUVA).

Immunochemical investigations on the problem of the pregnancy zone protein. XII. Serum concentrations of pregnancy-associated alpha-2-glycoprotein in diabetic pregnancy.

The concentration in serum of the pregnancy zone protein (PZ), a high molecular pregnancy-associated alpha 2-glycoprotein, increases during pregnancy, when oral contraceptives are taken and under certain pathological conditions. PZ is probably part of a multifactorial immunological defence system which prevents the premature rejection of the fetus and placenta which together may be regarded as an allograft. Normally, the PZ serum concentration begins to rise during the 6th-12th week of pregnancy and reaches its peak at about the 24th week. The mean values then remain stable and afterwards diminish slightly toward the end of the pregnancy. In contrast, the mean values in mothers with insulin-dependent diabetes mellitus continue to rise. Excessively high serum PZ concentrations are significantly more frequent among pregnant diabetic patients, especially where there is also rhesus D antigen incompatibility. No correlation was found between excessively high values and the risk features used for the Priscilla White classification. A few patients with progressive proliferative retinopathy even had unusually low values.

[Use of endometrial biopsy on first day of cycle, CO2-hysterosalpingography, and PSP-hydrotubation in ambulatory diagnosis of female sterility (author's transl)].

Three techniques for ambulatory diagnosis of female sterility are compared and critically assessed for their potential benefits and drawbacks. They are endometrial biopsy on the first day of cycle, hysterosalpingography in combination with CO2 pertubation, and hydrotubation, using 0.06 per cent phenolphthalein (Speck's test). There were many more inherent benefits than drawbacks, primarily with regard to informative value and avoidance of undesirable side effects. Good economy, convenient technical applicability, and relief of ward and operation theatre personnel are additional factors in favour of the above ambulatory approaches.

Fibrinogen: agreement of experimental and calculated hydrodynamic data with electron-microscopic models.

There is no general agreement on the size and shape of the fibrinogen molecule. We have studied the diffusion of the fibrinogen in solution by means of dynamic light scattering, nanosecond fluorescence depolarization and analytical ultracentrifugation. The results obtained under physiological concentration, pH an ionic strength are DT = 2.0 X 10(7) cm2sec-1, DR perpendicular = 40'000 sec 1. Nanosecond fluorescence depolarization yielded DR parallel = 1.6 x 10(6) sec-1. Tentatively this value is interpreted as DR parallel, namely rotational diffusion about the major axis of the molecule. The sedimentation coefficient is 8.1 S. The hydrodynamic parameters derived from our measurements were compared with those calculated on the basis of the models proposed by Hall and Slayter, Hudry-Clergeon and Marguerie et al., Bachmann and Lederer, and Köppel. The agreement is poor even if the degree of hydration is varied within wide limits. However, satisfactory agreement can be achieved by assuming a flexible molecule of about 900 A length corresponding to two end-to-end bound trinodular structures of the Hall and Slayter type, with a nodule diameter of about 45 A. Experimental evidence indicates that the discrepancies between the different models might be due to different techniques of sample preparation leading to different conformations of the molecule.

Follow-up study of chronic gastric erosions.

Chronic gastric erosions were detected with a frequency of 4.4% in 8,468 patients over the six year period from 1971 to 1977. The lesions were usually multiple, chain-like or clustered, along the greater curvature. The age group most often afflicted was the 50-60 year olds, and male predominance were three to one. In one-third of the cases, chronic erosions were found in combination with gastric or duodenal ulceration. Longterm observation of 64 patients over a period of four and a half years revealed no change in the condition in 50%, increase, decrease or complete disappearance of the lesions were found in the remaining 50%. In 10% chain-like multiple erosions developed into a persisting fold in the gastric mucosa. No evidence suggesting that erosive lesions lead to chronic ulceration or development of either polyps or malignancy within 4 1/2 years was recorded, nor was there any association with gastric protein loss.

[The specificity of peptide-PABA-test (author's transl)].

Exocrine pancreatic function was determined by oral administration of N-benzoyl-L-tyrosyl-p-aminobenzoic acid (peptic-PABA-test) in 120 controls, 74 patients with chronic pancreatitis, 35 patients with acute pancreatitis 2--6 weeks after recovery, 201 patients with a variety of gastro-intestinal diseases and in 10 patients with anorexia nervosa. In the control group, 70% +/- 18% of the oral administered dose of PABA was found within 6 hours in the urine. In contrast the group of chronic pancreatic patients excreted only 40% +/- 13% over the same period. "False negative" PABA excretion was found in 11 (9%) of the 120 persons with no pancreas disease. "False positive" PABA excretion was found in 13 (17,5%) of the 74 patients with chronic pancreatitis. The test was not influenced by age or sex. After stomach resection or cholecystectomy and in patients with ulcus duodeni, chronic hepatitis, functional diarrhea, Crohn's disease, colitis ulcerosa and acute pancreatitis 2--6 weeks after recovery the peptide-PABA-test was not distored. Diminished PABA excretion was encountered in some patients with toxic liver disease, inflammatory disease of the small intensine like M. Whipple, celiac disease and unspecific enteritis and in a few patients with cholelithiasis. Low PABA excretion was found in early all patients with partial small intestinal resection, terminal liver cirrhosis or liver metastasis with ascites and in all patients with anorexia nervosa.

[Sperma- and HLA-antibody demonstration in sterility].

Sperma antibodies were detected by means of passive haemagglutination, the immobilisation test and immunofluorescence in the serum of 16 females from among 150 infertile married couples. These serums (experimental group) and 16 serum samples containing no sperma antibodies (control group) were investigated for the presence of cytotoxic HLA-antibodies against lymphocytes from their married partners and from 20 selected donors by means of the micro-lymphocyte toxicity test. HLA-antibodies against the lymphocytes of the husbands involved were found in four women from the experimental group. Two of these four serums also reacted with 20% of the test lymphocytes. In the control group, lymphocyte-toxic antibodies against test subjects were also found in all three serums which cross-reacted with the lymphocytes of the husbands. No new diagnostic or therapeutic criteria for the treatment of sterility can be derived from the combined humoral sperma and cytotoxic HLA-antibodies in the serum.

[Immunology of pregnancy -- more recent aspects of immunological mother-child relations (author's transl)].

An account is given of some topical aspects relating to immunology of pregnancy, with reference being made to more recent literature. Included are hypothetical considerations on undisturbed embryonic development, the barrier function of the placenta, the ontogenesis of the immune system, immunosuppressive factors of pregnancy serum, the macrophage function of placental cells, pregnancy proteins, and immunological peculiarities of EPH gestosis.

[Measurement of basal temperature for calculation of duration of pregnancy (author's transl)].

The duration of pregnancy post ovulationem (p.o.), post hyperthermiam (p.h.), and post menstruationem (p.m.) was calculated for single births with birth weights around 2,500 g against the background of basal temperatures which had been measured by 172 patients. - Average pregnancy durations, p.o. (n = 159) were 263 +/- 11,3 days [boys (n = 81) 262,2 +/- 12,3 days; girls (n = 78) 263,9 +/- 10,1 days]. Average pregnancy durations, p.h. (n = 171) were 260,4 +/- 11,5 days [boys (n = 90) 259,7 +/- 12,2 days; girls (n = 81) 261,2 +/- 10.8 days]. Differences for p.o. and p.h. mean values between girls and boys could not be statistically secured. The average duration of pregnancy, p.m. (n = 166) was 281,7 +/- 11,2 days, with boys being born (n = 89) after 280,8 +/- 12,4 days and girls (n = 77) after 282,7 +/- 9,7 days.

[Effect of behavior changing drugs, imipramine and tranylcypromine, on the composition of the blood and activity of the alkaline leukocytophosphatase of emotive and non-emotive adult male Wistar rats].

The corpuscular composition of the blood and the alcalic leucocytophosphatase (ALP) activity was measured for a total of 40 emotive and non-emotive adult male Wistar rats in order to test the influence of a single dose of imipramine (IMI) and tranylcypromine (TRAN) as well as of IMI and TRAN given in a period of 8 days. Special attention was paid to the reaction type of the animals. Changes in the red and white components of the blood were found due to drug application and to reaction type. The relationship between reaction type and drug application we assumed to exist was confirmed. The ALP activity also changed depending on the type of drug dosage. The assumption that ALP activity is higher for emotive rats than for non-emotive ones was confirmed as well.

[Artificial marital (homologous) insemination of women with sterility and retroflexion of uterus (author's transl)].

Successful use of artificial marital (homologous) insemination is described in this paper by examples of eight sterility patients (women aged 31 on average) with mobile retroflexion of uterus and procreative husbands. After an average period of 4.7 years had elapsed with desires for children unaccomplished in spite of special treatment, the above approach led to positive success in al cases. Five women turned pregnant in the first insemination cycle, one in the second, and two in the third. There were five spontaneous births of six intact infants, while two patients still are pregnant. Laparotomy was necessary in one case for ectopic gravidity. The authors are inclined to conclude from their own experience that homologous insemination may be used as one of the therapies applicable to childlessness, if no causative factors are detectable in addition to mobile retorflexion of uterus.
