[Results of local transurethral high frequency hyperthermia in bladder carcinoma (author's transl)].

On the basis of controversial results from clinical use of hyperthermal irrigation of the bladder lumen published by various authors a procedure was developed for transurethral local high frequency hyperthermia using phantom experiments and experimental bladder tumors. Encouraging results from experiments on animals led to clinical use of this technique with pretreated recurring bladder tumors (n = 64) at various stages. Poorly differentiated tumors at higher stages responded especially well. Typical pathohistological findings induced by hyperthermia were in these cases extensive tumor-necroses, and as late finding stromahyalinoses. Permanent changes of tumorfree urothelium have not been observed. During the observation period up to the present (maximum 48 month) the 3-year-survival rate has been 92.5% (T1G2-3), 59.4% (T2G3) and 22.0% (T3G3).

[Advances in transurethral resection of bladder carcinoma (author's transl)].

Improved technical conditions and progress in the standardization of staging with consistent consideration of the pathohistological grading have produced conditions which allow to adapt the transurethral resection of bladder carcinoma to the principles of tumor surgery. Therefore a prospect of cure in spite of avoiding mutilating measures can be offered to the patient with low T-category of the tumor. Precondition is the consequent application of a strict resection strategy which is adapted to the tumor and provides exact statements of the area of tumor spread and its infiltration. In a period of 5 years 207 patients with stage T3--T3 tumors were treated primarily by means of differentiated, several times repeated TUR. The corrected 3 years survival rate of altogether 146 patients was 96% for stage Ta, 70% for stage T1, 54% for T2 and 48% for T3. Proceeding from our own results the methodology, indications and limitations of the transurethral resection technique are outlined and progressive measures discussed.

Experimental data on the application of a stoma prosthesis in cutaneous ureterostomy.

The main problem with urinary diversion via cutaneous ureterostomy is stomal stenosis. Results with experimental and clinical implants of carbon polymer stoma prostheses (max. clinical observation period: 31 months) for vesicostomies have encouraged us to find out whether implants of this material would be suitable for cutaneous ureterostomies as well. The first step was dilation of the ureters in 16 mongrel dogs, 4 mini pigs and 4 sheep. This was done by knotting a thread over a splint which had been introduced into the ureter. After 7 days the ureter was ligated prevesically and a carbon polymer stoma was implanted into the ureter. 37 of the 48 stoma implants were well tolerated and provided water tight urinary drainage; slight encrustation occurred, but, radiologically, a smooth flow of contrast medium was seen. Ten of these 37 cases had transient urinary leakage. Eleven of the 48 stoma implants were unsuccessful because of insufficient healing, urinary extravasation, parastomal inflammation or severe encrustations. The results of these experiments on animals would seem to justify initial clinical use. It is conceiveable that this way stomal stenosis of the cutaneous ureterostomy can be avoided.

[New development of a one-way-system for percutaneous nephrostomy (author's transl)].

Since a surgical nephrostomy is not satisfying as either a temporary or permanent means of urinary diversion and puts a heavy burden on the patient as well, the percutaneous nephrostomy is gaining significance. Broad application of ultrasonically or radiologically controlled punctures of the renal pelvis have been til now limited due to the lack of a one-time system which is easy to manipulate. A new puncture system was thus developed with which a percutaneous nephrostomy can be performed with an up to 14 FU catheter drainage in a taechnically simple fashion. Use with a total of 41 punctures proved to be unproblematic. In three cases the puncture was unsuccessful. One case of post puncture bleeding made a surgical revision necessary. The drainage system was well tolerated. The polyurethane fistel catheter was checked at six-week intervals and showed only a few shell-shaped incrustations identified by scanning electron microscopy. Any loss of function of the fistel catheter caused by these incrustations was not observed. Catheter change, which is necessary at regular intervals, was possible in all cases without difficulties.

[Detection of ureolytic bacteria in urine in urinary calculi].

Detection of ureolytic bacteria in the urine of stone patients: using a very sensitive and selective indicator medium we tested the urine of 308 stone patients for ureolytic bacteria. Urease-producing bacteria were found in 41 patients. In the urine of 13 of these patients we found more than 10(4) bacteria/ml and in 28 patients 10(4) or less. 75% of the patients with a positive urease test had infection stones. We believe, that the test for urease is a convenient and necessary completion of the bacteriologic-diagnostic measures.

[Leakage of irrigating solution during transurethral prostatectomy. An impedance cardiography study (author's transl)].

The aim of this study was to investigate the influence of the incorporation of irrigating solution during transurethral prostatectomy on the pulmonary and cardiovascular system. In 16 patients the changes of thoracic ground impedance were compared with those of the central venous pressure. Myocardial function was evaluated semiquantitatively by use of the systolic time intervals and pulmonary gas exchange by arterial blood gas analysis. There was a significant correlation between the increase of central venous pressure and the decrease of thoracic ground impedance. There was also a significant increase of the relation of pre-ejection period (PEP) to left ventricular ejection time (LVET) meaning an impairment of myocardial function, an increase of cardiac output, and a severe decrease of arterial oxygen pressure. The influence of the incorporation of irrigating solution on the pulmonary and cardiovascular system is discussed, the noninvasive character and simplicity of handling the impedance cardiography is pointed out, and the possibilities of monitoring critically ill patients undergoing transurethral prostatectomy by this method is demonstrated.

[Chemically induced transplantable carcinomas of the urinary bladder in dogs (author's transl)].

It takes 24 months to induce preinvasive carcinomas in the urinary bladder of dogs as reported in the literature. The transplantation of chemically induced carcinomas has never been successful. Therefore, it was looked for methods which enable a shortening of the latency period and to transplant such tumors. 12 of 22 female beagle whelps were injected transurethrally with fast polymerizing plastics and then they have been treated subcutaneously with ortho-aminodiphenyl, formylaminonitrofurylthiazole or with the two carcinogens together. The dogs have been controlled by endoscopy, cytology and histology every 4 weeks. The first invasive urothelial carcinomas were observed 13 months after starting the treatment. Totally 7 carcinomas of the bladder and 3 in the pelvis of the kidney were found. It was possible to cultivate tumor material in tissue culture, gained by biopsy transurethrally. The transplantation of tissue culture material into nude mice was successful in 19 of 23 animals. In 2 of 10 beagles tumors started to grow transiently after transplantation of tissue culture cells. In the bacterial mutagenicity test (Ames) and in animals ortho-aminodiphenyl was neither mutagenic nor carcinogenic. However, it was cocarcinogenic if used together with formylaminonitrofurylthiazole.

[Detection of ureolytic bacteria in the urine of stone patients (author's transl)].

Using a very sensitive and selective indicator medium we tested the urine of 308 stone patients for ureolytic bacteria. Urease-producing bacteria were found in 41 patients. In the urine of 13 of these patients we found more than 10(4) bacteria/ml and in 28 patients 10(4) or less. 75% of the patients with a positive urease test had infection stones. We believe that the test for urease is a convenient and necessary completion of the bacteriologic-diagnostic measures.

Induction of a transplantable urinary bladder carcinoma in dogs.

We attempted to reduce the carcinoma induction latency period and to produce a transplantable urinary bladder cancer in dogs. After transurethral pellet application, 12 of 22 female beagles were given subcutaneous injections of orthoaminodiphenyl, FANFT, or both. Monthly cytologic and histologic checks yielded the earliest evidence yet obtained for invasive urothelial urinary bladder carcinoma (15 months); seven urothelial carcinomas of the urinary bladder and three of the renal pelvis were established. Tissue cultures of the urothelial carcinomas in vitro were successful. Transplantation of tumor cells to 23 nude mice resulted in tumor growth in 19. Transplantation to 10 beagles yielded 2 tumors very quickly, but the tumors were rejected within 4 weeks.

[Carbon polymer stomata in vesicostomy and other urinary diversion procedures (author's transl)].

Implants of pure carbon are ideal for prosteses because of their tissue acceptability and lack of incrustation. Animal experiments with biocarbon implants were tested as subfascial implantations, coeco-, ileo-, and vesicostomies, (n = 37). The results showed lack of reaction and good healing of the stomata, lack of incrustation, and water tight urinary diversion. Based on these animal experiments biocarbon vesicostomies were carried out successfully in six patients. The follow up period was 3--16 months.

Urinary hydroxyproline in healthy patients and in prostate patients with and without bone metastases.

An essential part of the classification of prostate carcinoma is the diagnosis of bone metastases. This was done with 70 patients using x-ray analysis, scintography, determination of the acid and alkaline phosphatase, and pelvic crest biopsy, as well as aspiration of the pelvic and sternal bone marrow. In addition, the hydroxyproline concentration was determined in the 24-hour-urine. The study, which was initially undertaken on a sample group (n = 145), yielded a high correlation between age and sex and hydroxyproline values. Women before menopause show significantly lower values than do men of the same age. The data on patients with prostata cancer (n = 70) showed that patients with and without bone metastases, who had been treated with estrogens, had a significantly lower quantity of hydroxyproline than did patients who had not received estrogen therapy. Patients with skeletal metastases (n = 24) showed significantly higher hydroxyproline excretion in the urine than did those with prostate cancer without metastases, or healthy men of the same age (n = 35). Comparison of the results of hydroxyproline determination with the other diagnostic methods for demonstrating bone metastases showed that hydroxyproline determination was diagnostically on par with the scintigram. Pelvic crest biopsy, pelvic and sternal marrow aspiration can be considered valuable supplementary diagnostic procedures.

Uromucoid excretion in normal individuals and stone formers.

A quantitative immunological determination of uromucoid according to Laurell's method has been established. The average excretion rate of this glycoprotein in normal individuals was found to be 51.1 +/- 5 mg/24 hours. The uromucoid excretion in patients with calcium-oxalate, calcium-phosphate and cystine stones showed no significant change. In patients with urate stones and those with renal tubular acidosis a significant decrease in uromucoid excretion was found. Localization of uromucoid was found to be in the cells of the distal tubular section and the collecting tubules which was verified by immunincubation with FITC conjugated antihumanuromucoid globuline.

A transplantable tumor of the urinary bladder in rabbits.

The latency period for induction of urinary bladder carcinoma is at least 2 years in animals that can be followed endoscopically. We studied the possibility of producing a model of malignant urniary bladder tumor in rabbits in less time by transplanting tumor cells. Each of 80 male mixed bred rabbits received 1 ml of tumor cell suspension of Brown-Pearce carcinoma. Transplantation was done subcutaneously, intratesticularly, transurethrally, or aftet cystotomy via injection into the bladder submucosa. Within 2 to 3 weeks malignant tumor growth in the bladder could be shown. Superficial scarification of the mucosa, performed at the same time as transplantation, led to exulceration of the tumor into the bladder lumen. Tumor incidence reached a 80 to 95 per cent level. Metastases in these animals were analogous to human urothelial bladder carcinoma. This malignant tumor model seems especially suitable to study new methods of transurethral therapy for cancer of the urinary bladder.

The use of pure carbon stomata (Biocarbon) in urinary diversion.

A new synthetic material, Biocarbon, has the advantages of tissue compatibility and chemical inactivity, so that there is minimal salt sedimentation. Experiments with animals extending over a period of more than 2 years showed good healing after various surgical and urological operations. Biocarbon stomata were then used for vesicostomies in 6 patients. The period of follow-up has been months. We conclude that Biocarbon implants used for ureterocutaneostomies and for various conduit operations can improve tthe care of stomapatients.

Experimental and clinical investigations on the transfusion of autologous blood in urological operations.

In view of the known risks in homologous blood transfusions, the procedures for transfusion of endogenous blood are gaining increasing importance. Using this fact as a basis, the feasibility of direct intraoperative autotransfusion in the area of urology was investigated. 6 mongrel dogs received blood-urine autotransfusions after cystotomy and cavotomy (n = 2) as well as after left-side nephrotomy (n = 4). Coagulation and hemolysis parameters were examined 1, 2, 24 h and 7 days after the autotransfusion, and showed transitory pathological changes which were, without exception, reversible within 7 days. Clinical effects were not observed. On the basis of favorable experience, the intraoperative, machine autotransfusion was also used clinically for renal traumas (n = 13), urinary bladder traumas (n = 4) and nephrotomies (n = 3). Postoperative checks showed pathological changes in the hemolysis and coagulation values only during the first 3 postoperative days. The blood-urine autotransfusion was well tolerated without exception. Indications and contraindications for direct intraoperative autotransfusions in the area of urological operations are discussed.

[A transurethrally transplanted model of a carcinoma in the area of the urinary bladder of the rabbit (author's transl)].

Thirty-nine rabbits were injected intratesticularly (n = 4), in the neck (n = 6), or in the submucosal area of the bladder (n = 29), with a 1-ml suspension of cells from a continuously passaged Brown-Pearce carcinoma. Actual submucosal transplantation (n = 9) caused, in addition to bladder tumors, massive metastases in the peritoneal cavity, the pleura, and the eyes. Animals receiving mucous membrane cornification prior to the submucosal infiltration (n = 15) developed exulcerated tumors in the bladder lumen and a primary bladder carcinoma like metastasis. Animals in which the tumor cell suspension was transplanted transurethrally (n = 5) likewise showed metastases in para-aortal lymph nodes and in the liver. The experiments indicate that the Brown-Pearce carcinoma, which is not indigenous to the bladder, will, with appropriate methodology, behave in a manner similar to that of a primary bladder carcinoma and can in addition be transplanted transurethrally. Tumors thus induced appear to be suitable models for studying different therapeutic approaches to bladder carcinoma.
