[Experimental studies on hemolysis in transurethral interventions].

The question whether distilled water can be used as irrigating fluid for the transurethral electroresection has repeatedly given occasion for discussion. Price and better visibility when resecting speak in favour of its application. Haemolytic alterations, however, might occur and seem to make its application dangerous. Our own experiments in animals show that especially blood haemolised through distilled water in the bladder (a mixture of aqua dest. and haemolised erythrocytes) has a strong haemolytic effect. Intravenous instillation of distilled water leads to haemolytic processes as well, which, however, are weaker than those occurring after instillation of erythrocytes haemolised through distilled water. Solutions of Sorbitol and Manitol (e.g. Purisole) had no measurable haemolytic effect. On the basis of our examinations we can retain the fact that with massive absorption of already haemolised erythrocytes, dangerous alterations, occur. Such alterations depend on duration of resection, technique of resection and pressure conditions (use of low pressure irrigation resectoscopes or trocar irrigation). For the experienced urologic surgeon these factors might be less relevant, for the beginner, however, who might not take into consideration the facts mentioned, results could be disastrous. The use of Sorbitol- and Manitol-fluids during TUR, prevents haemolytic incidents with certainty.

[Local mitomycin-C therapy of infiltrating bladder cancer].

Adjuvant local chemotherapy for superficial cancer of the urinary bladder (TA-Tis) is meanwhile performed on a nearly standardized level. The indication for infiltrating tumours (T1-T4) however is controversial. The prevention of tumour cell implantation which is promoted by transurethral resections is advantageous. On the other hand a curative effect cannot be expected from local chemotherapy in infiltrating tumours. The long-term results of adjuvant local chemotherapy in 86 patients with infiltrating urinary bladder carcinomas are reported, who received mitomycin instillations after transurethral resections.

Human teratocarcinomas cultured in vitro produce unique retrovirus-like viruses.

We have previously reported that among a series of human tumours investigated, only human teratocarcinoma cell lines derived from testicular tumours or pulmonary metastases of patients in Germany and the U.S.A. produced retrovirus-like particles spontaneously, albeit in low amounts. In a recent publication electron microscopical data suggested that the human teratocarcinoma-derived ( HTD ) particles were morphologically closely related, but not identical, to the type C retroviruses of animals. In this communication, the explantation of three human teratocarcinoma cell lines is briefly described. Evidence is presented that HTD particles (i) are synthesized only in a fraction of the epithelioid and differentiating cells; (ii) can be induced biochemically in a manner characteristic of retroviruses; (iii) either are not infectious or possess a peculiar host range; (iv) are immunologically unrelated to animal retrovirus strains; (v) possess an endogenous RNA-dependent DNA polymerase activity that can be banded at 1.16 g/ml in linear sucrose gradients. These results may be taken as suggestive evidence that HTD particles represent a novel group of unique retroviruses.

Influences of experimental uremia on the intra- and extracellular acid-base status of the rat.

In an experimental study the influences of beginning uremia were studied in nephrectomized rats. It was observed: (1) Though there was found a continuous decrease of extracellular pH due to accumulation of fixed acids there was a slight increase of intracellular pH resulting from concomitant hypocapnia. (2) There was found a constant loss of bicarbonate from the extra- and intracellular body compartment which was much more pronounced for the extracellular space. This loss of bicarbonate reflects the progressing metabolic acidosis and demonstrates that the intracellular compartment is more protected against an increase of hydrogen ion concentration than the extracellular space is. (3) The stability of the intracellular acid-base status during uremic metabolic acidosis arises the question whether our clinical practice with alkali substitution according to the Mellemgard-Astrup equation is still adequate in these cases. (4) Blood-gas analysis can only give information about the acid-base status of the extracellular body compartment. The large intracellular space which is the aim of our clinical therapy is excluded from these measurements. Simple reliance of blood-gas analysis may lead to wrong conclusions and mistakes in therapy.

Morphology of the urinary bladder following long-term experimental irritation of the urothelium.

10 female beagle whelps received an artificial urinary calculus (1.5 ml of a rapidly polymerizing methyl-methacrylate) by means of a transurethral procedure. The X-ray, cytological, endoscopic and bioptic findings were followed in a long-term experiment. After a period of 61 months, precancerous urothelial changes (n = 1) and invasive urothelial microcarcinomas were found (n = 2). Typical changes from earlier controls were pseudopapillary follicular cystites. Chronic irritation of the urothelium is shown to be an important factor in the pathogenesis of urinary bladder carcinoma. Decisive is the length of the period of irritation rather than the actual material used in making the pellet.

[Blood and tissue levels of mitomycin C after intravesical instillation].

Local mitomycin C (MMC) bladder instillation immediately after transurethral resection of superficial transitional cell carcinoma of the bladder caused an MMC serum concentration up to 24.5 ng/ml of an average. Neither bladder perforation by TUR nor vesicorenal reflux are contraindications for local instillations MMC. Systemic side effects are not to be expected. The highest MMC concentrations were found in the cancer tissue. There were measurable concentrations of MMC down to the adventitia. The indication for immediate postoperative local MMC-chemotherapy is discussed.

New aspects of acid-base balance.

Diagnostic and therapy of acid-base disturbances are based today on so called blood-gas measurements. Recent research shows that these data can only give exact information about the acid-base status of the extracellular body compartment and that the more important intracellular space which is the aim of our clinical efforts is excluded from these measurements and still remains obscure for the clinician. Recent results could demonstrate that the arterial PO2 does not necessarily correlate with intracellular O2 tension. pH another parameter of the blood-gas analysis seems to behave in a similar way. Results, obtained with the DMO-method, an indicator distribution method for determination of the intracellular pH, are presented which show that simple interpretation of blood-gas analysis can lead to wrong conclusions and mistakes in therapy.

Synthesis of retrovirus-like particles in testicular teratocarcinomas.

Two lines of evidence led to the investigation of human teratocarcinoma cells in vitro for oncogenic retroviruses: the observation by electron microscopy of retrovirus-like particles budding from the syncytial trophoblasts of human placentas, and the demonstration that teratocarcinoma patients before treatment show a high serum antibody reactivity against envelope proteins of mammalian retroviruses. In all 5 teratocarcinoma cell lines studied so far, retrovirus-like particles have been detected by electron microscopy. The production of these human teratocarcinoma-derived (HTD)-particles is enhanced by induction procedures known to be effective in animal virus model systems. In parallel, virus induction also increases the level of chorionic gonadotropin in the culture supernatant, demonstrating syncytial trophoblast-like cells in the heterogeneous cell population of teratocarcinomas which could be responsible for HTD-particle production. The origin of these virus-like particles--endogenous or exogenous--as well as their role in the pathogenesis of teratocarcinomas is not known so far. The potential value of serum antibody reactivity in teratocarcinoma patients as a diagnostic marker is discussed.

[Clinical trials with localized mitomycin instillation therapy for superficial urothelial bladder carcinoma].

105 patients with transitional cell carcinoma of the bladder, stage Tis, TA, T1 were treated by a local cytostatica therapy with Mitomycin C (MMC) after transurethral resection (TUR) of the bladder with the following purposes: 1. curative therapy of the carcinoma in situ 2. avoidance of tumor cell implantation from the TUR. After a follow up of 22,3 months this therapeutic concept seems to be of great promise because of its clearly reduced recurrence rate of a total of 12,4%. Side effects are slight. Nevertheless it must be pointed out, that any definitive statement can only be made after a longterm observation, especially by multicentric, controlled, randomized studies. MMC serum resorption studies with 19 patients, partly with reflux and TUR caused perforation of the bladder, showed that MMC instillation therapy (20 mg MMC/40 ml NaCl) immediately after resection did not lead to a systemic toxic concentration.

Clinical value of different methods for determination of acid phosphatase in prostatic cancer.

The availability of a radioimmunoassay (RIA) and an enzyme immunoassay (EIA) for the prostate specific acid phosphatase required a study to compare these techniques with the conventional colorimetric assay. Our study is based on examinations of 188 normal persons and 136 patients with carcinoma of the prostate. The advantage of the immunologic methods - RIA and EIA - lies in their stable immunologic activity and their high specificity. However, RIA and EIA are not screening methods for incidental carcinoma because of their low sensitivity for stage-A tumors. Their good sensitivity at lower ranges of concentration makes them suitable for checking the course of a prostatic carcinoma during therapy. The level of prostatic acid phosphatase may allow conclusions about intra-or extracapsular growth of the prostatic carcinoma.

Perioperative noninvasive recognition of the TUR-syndrome in transurethral resections.

Easily manipulable, noninvasive techniques for the immediate recognition of absorption of irrigating solution in transurethral prostate resections have not previously been available. For this reason, a way was sought to bring about an improvement in this situation: changes in the thoracic ground impedance were studied perioperatively by use of impedance cardiography during 16 transurethral resections. Furthermore, measurement of central venous pressure and arterial blood gases was done simultaneously as well as registration of myocardial function by systolic time intervals. Most important among the changes caused by absorption of irrigating solution which were registered with impedance cardiography are a significant increase in cardiac output and a considerable decrease in arterial oxygen pressure. Furthermore, a strong correlation was found between the increase of central venous pressure and the decrease of thoracic ground impedance. The relation of the preejection period to the left ventricular ejection time showed a significant increase due to impairment of myocardial function. The results show that impedance cardiography can monitor the time and extent of absorption of irrigating solution exactly and immediately and thus allows immediate therapy. This noninvasive technique can thus be used for perioperative supervision of the patient during transurethral prostate resections.

[Local chemotherapy of bladder carcinoma with mitomycin (author's transl)].

40 patients with transitional cell carcinoma of the bladder, stage Tis, Ta und Tl, were treated by a topical cytostatica-therapy with Mitomycin C (MMC) after transurethral resection of the bladder in order to investigate: 1. The curative influence on the carcinoma in situ, 2. the prevention of tumor cell implantations. After an average observation time of up to the 23.4 months this concept of therapy seems to be promising because of clearly reduced recurrence rate and the lack of systemic toxicity. MMC absorption studies with 19 patients, partly with reflux and TUR-caused perforation of the bladder, showed that MMC instillation therapy (20 mg MMC in 40 ml NaCl) immediately after resection does not result inn systemically toxic serum concentration.

[Urological diseases during pregnancy].

Pyelonephritis and urinary calculi are the most frequent urological complications during pregnancy. They are followed by injuries of the urinary system during birth or in connection with multiple traumas in traffic accidents. Kidney transplantation or permanent urinary diversion (Ileum- or Colon-Conduit) are not an absolute obstacle for pregnancies when closely followed. Problematic for mother and child is the coincidence of pregnancy and malignant tumours. In that case good cooperation among the different specialities is necessary. This study provides a concept for the diagnostic and therapeutic procedures. Knowledge of adequate methods of examination and modern techniques and a timely treatment meeting the demands of the situation prevent complications and sequelae of the pregnancy.
