Prevention of bacteriuria after transurethral prostatectomy with nitrofurantoin macrocrystals.

Of 40 patients who underwent transurethral prostatectomy without receiving antimicrobial chemoprophylaxis 13 had significant bacteriuria at some time in the postoperative course. Of 47 patients given nitrofurantoin none of the 42 for whom culture results were available had bacteriuria. This difference between the 2 groups is statistically significant (p less than 0.01). Nitrofurantoin prevents post-prostatectomy bacteriuria and has a theoretical advantage over agents such as the cephalosporins in that its action is limited to the urinary tract.

Determination of fetal maturity from the measurement of the sedimentary fraction of the amniotic fluid (amniocrit).

A simple technique is described for evaluating foetal maturity. This consists in measuring the sedimentary fraction of the amniotic fluid (amniocrit). The amount of sediment, expressed in percent of the total liquid volume varies between 0.33% at 36 weeks of amenorrhea and about 2.22% at 40 weeks. These values are correspond to arithmetical means. The differences between pre-term and term pregnancy are highly significant (p less than 0.001). The mean at 32--36 weeks of amenorrhea is 0.27, and at 37--41 weeks is 1.45. A close correlation between gestational age and the fraction of sediment in amniotic fluid is shown in Fig. 1. The correlation coefficient was 0.726. This technique of measuring the sediment combines simplicity and reliability and can be employed where minimal resources are available, and in rural hospitals. The feasibility of using this procedure in institutions with limited resources is emphasized.

Sterol synthesis in cultured human renal cell cancer.

Studies on sterol synthesis in cultured human renal cell carcinoma were conducted in an attempt to elucidate the mechanism by which clear cell tumors accumulate cholesterol in the cytoplasm. Cultured renal cell carcinoma requires serum lipoproteins for optimal growth. In some renal cell carcinomas cellular cholesterol content was significantly higher than that present in cultured kidney cells under similar conditions. Renal cell carcinoma synthesizes sterols from labeled acetate. The rate of sterol synthesis is inversely proportional to the levels of exogenous cholesterol in the growth medium, suggesting the presence of a negative feedback mechanism. In addition, the conversion of 27-carbon-atom sterol precursors to cholesterol appears to be slow and inefficient under the presnt conditions. We conclude that an aberrant regulation of cholesterol synthesis cannot be invoked necessarily to explain cholesterol accumulation in renal cell carcinoma.

Regional services for children and youth with diabetes.

A regional diabetes program for children and youth comprising outreach clinics, frequent routine and emergency telephone contact, and a camping program was developed within the State Children's Medical Services (Crippled Children's) Program. The pediatric nurse specialist in diabetes served as the pivotal professional in the education and supervision of patients. Cost benefit was calculated from estimated hospital days prevented based on average length of admissions at diagnosis (14 days) and hospital days in the year prior to referral of those with established diabetes. During the first year 369 hospital days were prevented, for an estimated savings of $65,000; the second year 547 hospital days were avoided, a value of $96,000. Total annual program costs to the state and private patients (including camp fees) were $36,000. Although hospital costs are an apparent and useful reference to justify funding, the main value of the program must be in the prevention of secondary physical and emotional disability. This requires continuous commitment and availability of persons who know the patient and family, who can interpret information related to metabolic control of diabetes, and who are able to influence the family and community to respond appropriately to the youngster's needs.

Method for resection and prosthetic replacement of aneurysm of aortic arch.

We shall describe a case of aneurysm of the transverse aortic arch. The patient was operated upon successuflly. Severe respiratory distress owing to tracheal compression was the indication for emergency surgical therapy. A method is described comprising total cardiopulmonary bypass, hypothermia (28 C.) and local deep myocardial hypothermia. Catheters to cerebral vessels are brought off the main arterial line beyond the pump. Cerebral vascular resistance regulates local blood flow. Coronary perfusion is omitted.

Multilocular cystic kidney.

Two cases of multilocular cystic kidney are reported. Current concepts of pathogenesis are reviewed. Treatment by total nephrectomy is advocated when the contralateral kidney is normal; partial nephrectomy should be reserved for those cases in which the contralateral kidney has reduced function or is absent.

Prophylactic antibiotics in transurethral prostatectomy.

In a prospective randomized study involving 90 uninfected patients undergoing transurethral prostatectomy it was found that routine postoperative use of prophylactic cephalosporins had no beneficial effect on the incidence of fever, hospital stay or major complications. Patients receiving cephalosporins had a significantly lower incidence of postoperative bacteriuria immediately after catheter removal and 1 month postoperatively.

Torsion of scrotal testis in a persistent processus vaginalis.

A case of an unusual association between hernia and testicular infarction, namely torsion of a normally descended testis within a persistent processus vaginalis, is presented. The advantages of the inguinal ingulinal approach for the exploration of suspected torsion are discussed. Although an operation was performed more than 30 hours after the onset of symptoms the testicle was viable and recovered.

Surgical repair of post-hysterectomy vesicovaginal fistulas.

Herein we describe in detail the preoperative evaluation, surgical technique and postoperative care used in repairing 13 post-hysterectomy vesicovaginal fistulas by the transabdominal transvesical approach. We were successful in all 13 cases, although 1 required 2 operations. Excellent results can be obtained in these patients if a complete urological diagnostic study is done, a careful technique is used during the operation and specialized postoperative care is given. The transabdominal transvesical approach to this problem is safer and gives better results than the transvaginal operation.

Emphysematous pyelonephritis: report of 3 cases treated by nephrectomy.

Three cases of emphysematous pyelonephritis are presented. All 3 patients were diabetics, they all had Escherichia coli and there was evidence of ureteral obstruction in 1 case only. All 3 patients underwent nephrectomy and survived. Initial evaluation should be performed to rule out ureteral obstruction. Treatment should consist of a brief trial of high dose antibiotic therapy with serious consideration of nephrectomy in the face of persistent symptoms or gas. Incision and drainage are reserved for poor surgical risk patients. All patients should remain on antibacterial therapy and have frequent followup examinations.
