[Treatment of acute idiopathic vestibular paresis (author's transl)].

In 31 patients with sudden isolated unilateral idiopathic lesion of the vestibular function the effect of therapeutic application of low molecular weight dextran and cortison was examined. 15 out of 31 patients received no drugs, 16 patients were treated with dextran-infusion and cortison for 10 days. Without drug application the vestibular function was restored in only 4 out of 15 patients after about 9 months, but after the therapeutic application of low molecular weight dextran and cortison a normal reaction of the vestibular system was seen in 11 out of 16 patients.

[Amylase activity and isoamylase patterns in human parotid saliva (author's transl)].

For the determination of amylase activity and number of isoamylases in the parotid saliva of persons without diseases of the salivary glands, we investigated the saliva of 90 subjects who had been divided into 3 groups of 30 individuals each (up to 30 years, 31 to 60 years, 61 years and older). Within each group the sex ratio was 1:1. Although the amylase activities reveal no age-dependent differences, the number of isoamylases increases with age, the highest number being found only in the age group over 61 years. The possible diagnostic value of the determination of amylase activity and distribution of isoamylases is discussed.

[Spontaneously occuring and experimentally induced enzymatic changes in the rat parotid gland: a possible experimental model of ageing (author's transl)].

The activity, specificity and stability of many enzymes decreases in ageing organisms. This fact is attributed to the accumulation of errors in protein biosynthesis. Due to its very active protein metabolism, the parotid gland is a suitable experimental model for the investigation of such age-dependent processes. By administration of 5-bromodeoxyuridine to young rats, changes of isoamylase.activity were induced in the parotid glands which were strikingly similar to those found earlier in control normally-aged animals.

'Fast isoamylases' in parotid saliva of heterozygous carriers of cystic fibrosis.

An increased number of 'fast isoamylases' is found in the parotid saliva of children with cystic fibrosis and their heterozygous parents who are clinically healthy. 'Fast isoamylases' are generated, at least in part, from the 6 isoamylases of the basic pattern by deamidation of asparagine and glutamine residues. This basic pattern is present in the parotid saliva of all subjects we have tested so far. A shift in the equilibrium between amidation and deamidation processes is discussed in terms of its possible significance for the pathogenesis of cystic fibrosis and for ageing.

Age-dependent distribution of isoamylases in human parotid saliva.

The number of isoamylases present in parotid saliva from 53 healthy men and women of different ages was determined. A basic pattern of 6 isoamylases was evident in all samples. With increasing age, an increasing number of additional isoenzymes was observed which migrated faster towards the anode. These 'fast isoamylases' are derived from the basic isoenzymes by deamidation. The significance of deamidation of proteins as age increases as well as the increasing number of 'fast isoamylases' in saliva from mucoviscidosis patients are discussed.

Effect of cytostatic agents on salivary gland tissue of the rat. The influence of 5-fluorouracil.

Application of 5-fluorouracil (5FU) for 3 weeks considerably decreased submaxillary gland weight but had no apparent influence on amylase and protein concentrations in gland extracts and isoamylase distribution patterns as revealed by electrophoresis. After another 3 weeks during which no 5FU was administered, an overproduction of protein synthesis (especially amylase) was observed.

Pharmacological inhibition of salivary glands: a possible therapy for sialosis and sialoadenitis. Effect of experimentally induced beta-receptor block on the rat parotid gland.

An increase of alpha-amylase concentration and specific amylase activity (correlated to protein content) was evident already after 3 days in the parotid glands of rats treated with 2 X 2 mg/day of propranolol hydrochloride. After separation of amylase into isoenzymes, only the specific isoamylase activities were enhanced. In contrast to treatment with a guanethidine derivative (guanoxane), these changes have persisted after 3 weeks. On the strength of these results, functional inhibition of the salivary glands (i.e. blockade of secretion by beta-sympatholytic agnets) is discussed as a therapeutical possibility for the management of sialosis and sialoadenitis.

[The short-time-intubation as a cause of morphological and functional disorders of the larynx (author's transl)].

In the Göttingen ORL-hospital 100 anaesthetics by endotracheal intubation were carried out within 3 months. All patients were examined phoniatrically and laryngoscopically before and after the anaesthetic. 31% of the patients exhibited no morphological or functional disorders of the larynx. The other patients exhibited minimal (26%), flight (24%) or moderate (19%) postintubation complications. The incidence of postintubation compications is independent of intubation-time, age and sex of the patients, and the size of the tube. An asymmetry of the larynx however favoured the incidence of moderate postintubation complications.

[The arthritis of the cricoarytenoid joint, a rare postintubation complication and its novel treatment (author's transl)].

A case of aphonia following a long-time endotracheal intubation is presented. The paramedian position of the left and the incomplete adduction of the right vocal cord were caused by a complete, respectively partial ankylosis of the cricoarytenoid joints. The ankylosis was the result of an arthritis during the intubation period. A nearly normal mobility of the vocal cords and a normal voice was achieved by translaryngeal injections of Fludrocortisone and Hyaluronidase into the cricoarytenoid joints and by their passive mobilisation.

[Penicillin levels in tonsillar tissue and serum after intramuscular and oral application (author's transl)].

Penicillin levels were determined in both serum and tonsillar tissue 1-2 h (29 patients) and 3-4 h (16 patients) after intramuscular application of Penicillin-G resp. oral doses of Penicillin-V. All tonsils were found to contain different amounts of Penicillin. Concentrations of antibiotic in the tonsillar tissues are dependent on the respective serum levels. The serum-tonsil-difference of the Penicillin levels increases significantly in adults. The efficiency of oral and intramuscular Penicillin application and the occasionally described failure of oral Penicillin medication are discussed with regard to the minimal inhibitory concentrations (MIC) for group A streptococci.

[Endo-ecto-laryngeal dysproportion: a frequent cause of dysplastic dysphonia (author's transl)].

A post-mutational laryngeal dysplasia is described which results in rapid vocal fatigue and chronic dysphonia. Pathognostically important in the respiratory position is a profile-deficient endolarynx, combined with hypoplastic Morgagni ventricle and distinct vocal cord contour in the phonatory position. We interpret this dysplasia as the consequence of a dysproportionate growth process during which the growth of the endolaryngeal soft tissue fails to match that of the laryngeal frame.

[Combined treatment of a cicatrised post-intubation complication with surgery and medication summary (author's transl)].

A case of postintubation laryngotracheal injury is described. The paramedian position of the vocal cords was found to be a consequence of cicatrix in the area of the posterior commissure, and by a luxation with ankylosis of the right cricoarytenoid joint. Paresis of the recurrent laryngeal nerve was excluded by electromyography. Multiple incisions of the cicatrix failed to correct the problem, whereas subsequent incision followed by intensive therapy with fludrocortisone and hyaluronidase was successful in management. In administering the medication, a new simplifed method for translaryngeal injection is described.

[The Göttingen audiometric speech test for children. I. Speech audiometry of the young and retarded child by a picture-test (author's transl)].

The Göttingen audiometric speech test was developed for testing 3 to 4-years-old and retarded children. The test presents the following advantages: 1. The child is tested by one-syllable nouns. Thus more information by speech-rhythm is prevented. 2. For each word there are given four different pictures, which present words containing the same vowel, therefore a) the possibility of choice remains constant, b) the child won't be overtaxed by too many pictures, c) the possibility of understanding the test-word by hearing the vowel alone is avoided. The test was applied to 122 3 to 6-years-old children in order to get normal values.
