Transitional cell carcinoma of the bladder in children and adolescents.

A review of our records between 1950 and 1980 identified 12 patients with transitional cell carcinoma of the bladder who were less than 21 years old. The tumors were low grade and low stage, and were associated with an excellent prognosis. Only 1 patient had a solitary recurrence. This study supports the contention that transitional cell carcinoma of the bladder is a less aggressive disease in patients who are in the first 2 decades of life than in older patients.

The Nesbit procedure for Peyronie's disease.

We treated 8 patients with severe Peyronie's disease with the Nesbit procedure. All patients had had partial to full erections and all had been unable to achieve vaginal penetration because of pronounced curvature of the penis. After penile straightening all patients were able to resume normal coitus. We recommend the Nesbit procedure as the first step in the surgical treatment of patients with Peyronie's disease who have severe penile deformity but good erections.

The Jonas prosthesis--technical considerations and results.

We analyzed 100 patients who underwent implantation of the Jonas penile prosthesis because of impotence, complications, and patient and partner satisfaction. Data were gathered by direct patient contact and mailed questionnaires, and at followup ranging from 4 to 18 months. Complications occurred in 7 patients. Of the 100 patients 90 were satisfied with the results and would choose the same operation again, although 20 claimed to have some difficulty with concealment of the penis. A total of 56 patients underwent implantation of the prosthesis under local anesthesia, which has resulted in diminished patient risk as well as reduced cost and duration of hospitalization.

Treatment of transitional cell carcinoma of the bladder with hematoporphyrin derivative phototherapy.

The demonstration that intravenously administered hematoporphyrin derivative concentrates preferentially in dysplastic and neoplastic transitional cells in the human bladder prompted a trial of hematoporphyrin derivative phototherapy in patients with transitional cell cancer of the bladder. A dose of 2.5 mg. per kg. was given intravenously 3 to 48 hours before treatment with light of a wavelength of 630 nm. from an argon ion pumped dye laser. Total light dose approximated 150 joules per cm.2. Four patients with resistant carcinoma in situ have been treated and all have had disappearance of the tumors proved by biopsy. Although there are many technical problems we believe that hematoporphyrin derivative phototherapy holds promise as an effective treatment modality, primarily for patients with resistant, recurrent in situ transitional cell carcinoma of the bladder.

Primary realignment of posterior urethral injuries.

The records of 34 patients with posterior urethral disruptions secondary to blunt pelvic trauma who were treated with primary realignment were reviewed. Among the 29 evaluable patients there was a 38 per cent incidence of stricture (11 of 29), a 3 per cent incidence of incontinence (1 of 29) and a 15 per cent incidence of impotence (4 of 27). Primary realignment has a low morbidity and an acceptable rate of incontinence, impotence and stricture formation, and is recommended in the treatment of posterior urethral injuries.

Relationship between grade and stage of adenocarcinoma of the prostate and regional pelvic lymph node metastases.

The relationship of grade and local tumor extent to pelvic nodal involvement was assessed surgically in 469 patients who underwent pelvic lymphadenectomy and radical retropubic prostatectomy for adenocarcinoma of the prostate. Grade and local stage alone and combined score were associated in a linear fashion with nodal involvement. Gleason sum and Mayo grade alone were not predictive of nodal metastases. Some patients with a Gleason score of 4 had positive nodes and some with a score of 8 to 10 had negative nodes. Mayo sum (grade plus stage) consistently was related to negative nodal involvement.

External beam radiotherapy for palliation of pain from metastatic carcinoma of the prostate.

Radiotherapy often is used for palliation of bone pain from metastatic cancer of the prostate but an objective evaluation of its efficacy in a large series of patients is unavailable. We report the results of external beam irradiation in 62 patients who had bone pain secondary to stage D carcinoma of the prostate. The variables used to judge pain before and after radiotherapy included subjective evaluation of pain, status of activity and quantitation of analgesic use. Complete relief of pain was achieved in 26 patients (42 per cent), partial relief in 22 (35 per cent) and no relief in 14 (23 per cent). On the basis of our experience external beam irradiation is useful palliative therapy for pain from metastatic cancer of the prostate.

Detection and localization of In situ carcinoma of the bladder with hematoporphyrin derivative.

The extent and localization of in situ carcinoma of the urinary bladder are frequently difficult to assess. We have studied this problem by employing intravenous hematoporphyrin derivative, an endoscopic detection device, and complete pathologic bladder mapping. These preliminary investigations indicate that hematoporphyrin derivative localizes in dysplastic and neoplastic transitional cell epithelium and that these abnormal areas can be detected during a cytoscopic examination with the use of our detection device.

Pleomorphic microvilli and low-grade human urothelial cancer: need to correlate diagnostic modalities.

It is technically feasible to examine human urothelial biopsy specimens by light microscopy after the same specimen has been examined by scanning electron microscopy. This combined microscopic procedure demonstrates that surface microfeatures of human urothelial cells, including pleomorphic microvilli, are variably present in atypical and in malignant vesical tissues. Scanning electron microscopy of a urothelial specimen requires direct light microscopic correlation of the pathologic disorder which is present in a particular area of the specimen which has been examined by scanning electron microscopy.

Embolization for genitourinary disorders.

Fourteen patients with various genitourinary disorders underwent arterial embolization. The procedure was useful in facilitating surgical removal of hypernephromas, in decreasing tumor-induced hypercalcemia, and in diminishing proteinuria secondary to renal failure. Patients with priapism due to excessive arterial inflow responded well. Results of embolization performed because of bleeding varied. When bleeding was localized to a single vessel or to an area supplied by such a vessel, embolization was successful. However, when hemorrhage was diffuse or due to multiple vessel inflow, arterial embolization was not entirely effective.

Treatment of pain of diffuse metastatic cancer by stereotactic chemical hypophysectomy: long term results and observations on mechanism of action.

Stereotactic instillation of absolute alcohol into the sella turcica for pituitary destruction was carried out in 29 patients divided into two groups. Seventeen with prostatic carcinoma underwent a total of 19 injections with 94% good to excellent results that persisted throughout the remainder of the patient's life-span. The longest survival was 9 months. Brief relapses did occur, but spontaneous remissions were the rule. A second group of mixed cancers contained 12 patients who received a total of 13 injections. Eleven patients had good to excellent results that persisted in all but 1 patient. The longest survival was 7 months. Hormonal levels and prolactin stimulation tests failed to show any correlation between hormonal changes and pain relief. Naloxone reversal of analgesia did not occur. There was no loss of cognitive function shown on psychological testing. Pathological studies showed destruction of the pituitary gland, which was subtotal in some patients despite good pain relief. All examinations showed that the pituitary stalk was destroyed. Patients who survived longer also showed degeneration of the supraoptic and paraventricular nuclei of the hypothalamus and the median eminence. All but 1 patient with pain relief exhibited a lack of antidiuretic hormone (ADH) production. Interpretation of the data indicates that ADH or its associated neurophysins act as central pain transmitters. The production of these transmitters is decreased or abolished by chemical hypophysectomy through the destruction of hypothalamic nuclei.

Undetected genito-urinary dysfunction in vertebral fractures.

Five of fourteen patients admitted with fractures of the thoracic and lumbar vertebrae who had normal rectal tone and sacral sensation were subsequently found to have genito-urinary dysfunction. Four patients had a hypotonic bladder documented by cystometrograms. The other patient was impotent from the time of fracture. Since the genito-urinary dysfunction was not detectable by physical examination, thorough urological evaluations are recommended for all patients with fractures of the lower thoracic and upper lumbar vertebrae, even when rectal tone and sacral sensation are normal.

Multifocal fibrous pseudotumor of testicular tunics. Possible clinical dilemma.

A morphologic characterization of a tunical multifocal fibrous pseudotumor is presented because of the clinical dilemma this rare entity causes urologists and pathologists. This report demonstrates the necessity for familiarity with testicular pseudotumors in order to avoid an unnecessary orchiectomy. If histopathologic examination of biopsies from every pseudotumor nodule fails to demonstrate malignancy, excision of the entire tunics including the pseudotumors is appropriate therapy.

Treatment of stage D hormone-resistant carcinoma of the prostate with estramustine phosphate.

We report on 51 patients with hormone-resistant, stage D prostatic carcinoma who were treated with estramustine phosphate and followed for at least 6 months. Of the 51 patients 5 (10 per cent) had a partial objective response, 30 (59 per cent) remained stable and 16 (31 per cent) had progression of the disease. All of those patients who had a partial response or remained stable also experienced subjective improvement as judged by relief of pain and performance status. Approximately 8 per cent of the patients will be unable to take estramustine phosphate because of intolerable gastrointestinal side effects.

Cellular autofluorescence--is it due to flavins?

For many cell types, cellular autofluorescence in the 500-600 nm spectral region can result in a significant background signal for measurements of weakly fluorescent probes. Measurements of fluorescence intensity and spectra of a variety of cell types and assays of neuronal homogenates are presented to demonstrate that this autofluorescence is most likely due to endogenous flavoproteins.
