[Byler's disease. Ultrastructural study. Apropos of a case in an infant].

A case of familial cholestasis with cirrhosis is described in a 8 months old boy, presenting with hepatosplenomegaly, portal hypertension, dramatic pruritus, and fluctuating icterus of early post-natal onset. Biological data include positive hepatocyte retention test, with mild hepatocyte cytolysis, without patent hepatocyte insufficiency. The discrepancy between the clinical symptoms and a slight elevation of bilirubin partially conjugated, the absence of elevated blood cholesterol, the absence of evidence of antigen or antibody of virus A or B, the marked elevation of blood biliary acid lead to the suspicion of Byler disease. A liver biopsy with ultrastructural study shows a thickening of the ectoplasm, and the presence of microfilament material in the lumen of partially broken villi. Comparisons are made with the 4 other cases of Byler disease with E.M. study documented in the literature.

[Recto-sigmoid cancer and polyps in children. Comments apropos of 2 cases of atypical juvenile polyps].

The writers present a report of 2 observations of colo-rectal carcinoma, on a 19 month old infant with a mixed juvenile and adenomatous polyp situated at the recto-sigmoid junction and treated successfully by segmental colectomy, and on a 10 years old girl presenting a large colloid rectal carcinoma which occurred 6 years after resection of an atypical rectal juvenile polyp. After recalling the rarity and the gravity of colo-rectal carcinoma in childhood, the authors underline the importance of histological investigations in the juvenile polyposis which is usually benign; the possibility of histological aspects on separate polyps and sometimes on the same polyp explains the degeneration of some multiple juvenile polyps (several cases in the literature), or isolated polyp (first case described).

Intravesical instillation of mitomycin C in the prophylactic treatment of recurring superficial transitional cell carcinoma of the bladder.

Twenty-six patients with an elevated recurrence rate of stage O or A bladder tumours were treated prophylactically after transurethral resection (TUR) with short-term intravesical instillations of mitomycin C in a non-randomised study. The data before intravesical chemotherapy (IVC) were taken as the internal control for each patient. Mitomycin C was administered in one course of 10 daily doses of 40 mg, starting 24 to 48 hours after TUR. The average follow-up was 13.9 months before and 28.5 months after IVC. Before IVC, the average number of recurrences was 2.6, with an average recurrence rate of 1 every 5.3 months. After IVC there was no tumour recurrence in 4 patients, with a mean of 12.2 months' follow-up, but there was tumour recurrence in 22 patients. For 17 patients (77%) the free interval averaged 2.3 times longer than the mean interval of recurrence before IVC and for 18 patients (82%) the recurrence rate averaged 3.3 times less than the recurrence rate before IVC.

Correlation between cytology and cystoscopy in the follow-up of patients with bladder tumours.

The correlation between cytology and cystoscopy was established using data from 450 cytologic and cystoscopic examinations performed on 401 urology outpatients. Cytology specimens were interpreted by cytopathologists, without access to any clinical information. 311 examinations were performed on patients with present or past bladder tumour history and 139 on patients who were examined for other urological causes. Cytology specificity was good, with a low rate of false-positive results, and revealed 36% of cystoscopically visible tumors; sensitivity increased with tumour grade. In some patients, positive cytology preceded the appearance of tumoral lesions by several months. Cytology and cystoscopy are complementary methods for use in the management of patients with bladder tumours.

[Congenital epulis of the newborn (granulous Abrikosov tumor). Remarks apropos of a personal case].

The authors report a case of middle and superior gingival tumor in a newborn patient. Macroscopic and topographic aspects are a "congenital epulis". The surgical treatment is easy. Histologically finding shows a benign tumor with granulous cells called "Abrikossoff Tumor". The literature is reviewed to study histological aspect and pathogeny of this tumor. They try to distinguish the macroscopic aspect of the "congenital epulis" with the histologically finding. It is not systematically a granulous cells tumor.

[Quantitative histology of papillary tumors of the bladder. Prognostic value].

Morphometry was applied to quantitate characteristic microscopical features of the removed non invasive papillary bladder tumours in 60 patients and of the normal urothelium in 13 cases. The measurements of nuclear areas were made on routine HES section with a microscopic digitiser (ASM-Leitz). There was a significant difference, concerning the average nuclear area, between normal urothelium and bladder tumours and also a correlation between the number of relapses at two years and this parameter. A significant difference existed for the average nuclear area of tumour without any relapse and the tumours with one or several relapses (p less than 0,02). A difference existed also between the tumours that caused cystectomie or early death and other tumours (p less than 0,01). The anisocaryosis (evaluated by the mean of the coefficient of variation of the average nuclear area) was greater in cases of tumours with bad prognosis (death). These results confirm the prognostic value of morphometric parameters in the histological analysis of papillary bladder tumours.

[Klinefelter's syndrome associated with precocious puberty due to tumoral secretion of chorionic gonadotropins].

A 8 and a half year-old boy presented with precocious puberty related to a malignant thoracic teratoma. He was also shown to have a Klinefelter syndrome. Precocious puberty related mainly to the liver, intracranial or thoracic tumors is rare. It seems to be exclusively observed in boys. The slight testicular enlargement is the main clinical sign. The contrast between high LH and low FSH levels is the most striking biological data. The diagnosis is proved by plasma HCG, beta-HCG and alpha-foetoprotein determination. Our patient is the third one with Klinefelter syndrome; this this association is certainly not fortuitous.

[Bilateral parotid enlargement from malignant lymphoma (author's transl)].

A rare case is reported of a 32-year-old man presenting with bilateral enlargement of the parotids only, later found to be due to a malignant lymphoma of the non-Hodgkin's type. Diagnosis was impossible at this stage because of the absence of other clinical signs, except for asthenia, and the negative results of complete investigation. Diagnosis became possible after the appearance of cutaneous lesions followed by a parotid nodule, which were shown by biopsy, over a period of two years, to be extensions of a non-Hodgkin's type 2 malignant lymphoma, according to the classification of Bryon et al. from Lyon. The initially prescribed corticoid treatment was replaced by multiple chemotherapy, and parotid enlargement, asthenia, and skin lesions were no longer present eight months later. The authors emphasize that no lesions occurred in the hematopoietic organs during the course of the disease, which clearly differentiates this case from the parotid lesions seen fairly frequently in blood diseases and Gougerot-Sjögren syndromes complicated by lymphoid leukemia.

[Tumors of the mandibular condyle (author's transl)].

Three cases of tumors of the mandibular condyle are reported. One patient had metastases from a cutaneous melanoblastoma, another a benign osteochondroma, and the third a plasmocytosarcoma revealing the presence of Kahler disease. The authors emphasize the rare nature of these lesions, in spite of the technical advances made in the radiological and surgical exploration of the temporomandibular joint, and also the large variety of histological types reported in the literature. They discuss the main diagnostic features, especially in isolated lesions in patients with no relevant past history, and stress the importance of surgical biopsy. Therapy varies from surgical treatment in benign tumors, the prognosis being excellent, to usually palliative therapy in malignant lesions.

[Meckel syndrome. Difficulties in prenatal diagnosis caused by variability of expression (observations on two families)].

The Meckel syndrome is inherited as an autosomal recessive monogenic trait. It associates abnormalities of the neural tube (encephalo or occipital meningocele more seldom hydrocephaly) and of the extremities (post axial polydactyly) with multicystic dysplasic kidneys. When diagnosed in a conception product, it allows one's to provide an appropriate genetic counseling as well as an early and followed-up surveillance of the next pregnancies in the affected sibship. Taking the example of two families, the authors emphasize the difficulties in diagnosing this affection (mainly due to its various expressions) and the various methods available for its pre-natal detection.

[Mesoblastic nephroma. Apropos of 6 cases].

6 cases of congenital mesoblastic nephroma removed surgically between 5 days and 2 months 1/2 of age are reported. The authors point out the peculiar histological aspect of this tumor : myomatous or fibromyomatous proliferation which surround in some places normal or dysplastic glomerular or tubular structures and penetrates by digitations in parenchymatous elements without destroying them. At the periphery of the tumor there are some lymphatic capsular ectasia or lymphoid infiltration or cartilaginous islets. In case of a renal tumor of the newborn or infant they emphasize the necessity to search for a congenital mesoblastic nephroma in order to avoid all types of radio therapic or chimiotherapic treatment without histological diagnosis. These forms of therapy are responsible for severe complications heavily impairing a prognosis which in most cases is an excellent one when nephrectomy alone is carried out. In some exceptional cases it seems useful to add other forms of therapy when the tumor proves itself to be of a peculiar aggressivity macro or microscopically.
