Acute evolving myocardial infarction. A surgical emergency.

This is a report of 342 cases of acute evolving myocardial infarction treated with prompt coronary artery bypass. Myocardial infarction results from a time-related sequence of ischemic pathophysiological changes. The first hours constitute a rapidly progressive event. Prompt surgical revascularization partially prevents impending myocardial necrosis; occasionally it may even prevent it completely. The results are limited infarct size, decreased mortality and morbidity, and a striking absence of the complications associated with conventional therapy (ventricular aneurysm or perforation and septal and papillary muscle rupture).

The role of surgical reperfusion in myocardial infarction.

The role of coronary artery bypass graft surgery during myocardial infarction is unclear. This article reviews worldwide results with this form of therapy. Included in the analysis are the effects of surgical reperfusion on left ventricular function, anginal classification following reperfusion, the effects of early versus late reperfusion, and an in-depth mortality analysis from multiple medical centers. It is concluded that surgery can be performed with a very low mortality in most patients with acute myocardial infarction, but the most appropriate form of therapy of acute myocardial infarction that will result in low mortality and morbidity remains a challenge for future investigation.

Anatomical studies on the cerebellum of the donkey of Egypt.

The cerebellar mass forms about 34.4 cm3. The ratio to the brain mass is 1:9.8. The cerebellar dimensions are 5.2 cm in width, 3.9 cm in height, and 4.9 cm in the rostrocaudal length. Its weight is 37.6 g, it forms a relation of 1:9.4 (10.8%) to the weight of the whole brain mass. The cerebellar lobulation of the donkey is as follows: The rostral lobe consists of median lobules (Lingula, Lobulus centralis, and Culmen) and lateral lobules (Vinculum lingulae, Ala lobuli centralis, and Lobulus quadrangularis). The caudal lobe consists of median lobules (Declive, Folium, Tuber, Pyramis, and Uvula) and lateral lobules (Lobulus simplex, Lobulus ansiformis, Lobulus paramedianus, Parafloccularis dorsalis, and Parafloccularis ventralis). The flocculonodular lobule is made up a median lobule, the Nodulus, and the lateral lobule is the Flocculus. All measurements, numbers of the superficial folia for each lobule as well as the folia which border the fissures were recorded. From the investigation, some morphological differences between donkey and horse can be concluded.

Studies on the histogenesis of the tunica mucosa of the stomach of the Egyptian water buffalo (Bos bubalus L.). V. Histogenesis of the abomasal mucosa.

The primordia of the mucosal longitudinal folds are already prominent in the fetus with 9 cm CRL. In the fetus with 9 cm CRL the epithelium is stratified and becomes mixtured of simple and pseudostratified columnar in the fetus with 16 cm CRL in which the primordia of the abomasal glands are firstly seen but finally in the fetus with 17 cm CRL the whole epithelium becomes simple columnar. Although some few parietal cells are evident in the fetus with 73 cm among some fundic glands, but the complete-establishment of their cellular elements is completed postnatally. The Lamina muscularis mucosae is firstly seen in the fetus with 16 cm in the terminal portion of the abomasum.

Anterior transmural myocardial infarction: effects of surgical coronary reperfusion on global and regional left ventricular function.

Global and regional left ventricular function were assessed before and after surgical coronary reperfusion in 54 patients surviving anterior transmural myocardial infarction. Two groups were identified. Group I (n = 34) was treated within 4.8 +/- 0.7 (mean +/- standard deviation) hours of onset of symptoms of anterior transmural myocardial infarction, and Group II (n = 20) was treated 9.2 +/- 4.8 hours from the onset of symptoms (p less than 0.01). On study entry, the two groups were similar in all characteristics except global left ventricular ejection fraction (48 +/- 9 versus 42 +/- 13%, p less than 0.05). Regional ejection fraction was obtained by computer-assisted planimetry from ventriculographic tracings at end-systole and end-diastole. The anterior wall was divided into four equal segments from the apex (area 1) to base (area 4). Areas 2 and 3 defined the midportion of the anterior wall of the left ventricle. This yielded four fractional changes expressed as ejection fraction in percent. Global and regional ejection fractions (from apex to base) of the anterior wall significantly improved in Group I (from 48 +/- 9 to 55 +/- 11%; 7 +/- 17 to 18 +/- 20%; 12 +/- 14 to 25 +/- 18%; 25 +/- 15 to 38 +/- 17%; and 39 +/- 13 to 41 +/- 12%) (p less than 0.05, except for the basal area), but only to a minor degree in Group II (from 42 +/- 13 to 45 +/- 16%; 9 +/- 10 to 13 +/- 15%; 10 +/- 10 to 17 +/- 10%; 27 +/- 16 to 32 +/- 14%; and 37 +/- 10 to 36 +/- 13%) (all p values were not significant [NS] except for region 2). These data suggest significant enhancement of global function and regional wall motion in selected patients if surgical reperfusion is performed within 6 hours from the onset of symptoms of anterior infarction. Little improvement can be expected when the procedure is instituted later than 6 hours from peak symptoms, although improvement in some patients occurs if adequate collateral perfusion or nontotal left anterior descending coronary occlusion is present. In spite of functional improvements, some contractile deficit persisted throughout the period studied even when successful reperfusion was achieved early during evolving anterior transmural myocardial infarction.

Micturition studies and sexual function in operated hypospadiacs.

Micturition patterns in 33 males, aged 15 to 34 years, were investigated after hypospadias repair in childhood. Spraying of the urinary stream was the most common complaint (13 patients). Urinary flow was low in five patients as compared with controls, but in only one of these patients was an obstruction found that required treatment. Five patients had persistent curvature of the penis. One of them was unable to perform sexual intercourse.

Castration complex. Evidence from men operated for hypospadias.

Thirty-three adult hypospadiacs and 36 matched controls were investigated by means of Rorschach's test, in a double-blind design. From psychoanalytic theory predictions were made about the effects of the influences of the genital malformation and its treatment on the psychological adjustment. The hypospadiacs differed significantly from the controls as predicted. They exhibited more neurotic constriction with less ego-strength and less utilization of resources. Their anxiety and hostility levels were judged higher and they showed less self-esteem and activity as well as less capacity for interpersonal relations. The results are compatible with psychoanalytic theory on the role of castration anxiety in the neurosis formation. The need for psychotherapeutic support for these boys and their parents is stressed.

Penile malformation, gender identity and sexual orientation.

Gender identity and sexual orientation were investigated in 34 men operated for hypospadias in childhood and in 36 matched control subjects. Independent psychiatric assessments from semi-structured interviews, and double-blind formalized psychological ratings from the Rorschach, yielded similar findings. The probands were less secure in their maleness but were similar in sexual orientation as compared to the controls. A third blind, independent measure of unconscious gender identity, the Franck Drawing Completion Test, gave further evidence of uncertain gender identity in the patients. A self-administered inventory, the Gough Femininity Scale, showed a tendency for the patients to take more feminine sex roles. The findings are discussed from psychoendocrinological as well as psycho-analytical view points. The need for early psychological preventive measures for hypospadic boys and their parents is stressed.

Androgens and personality in normal men and men operated for hypospadias in childhood.

Serum levels of LH, FSH, prolactin and androgens were assayed in 33 adult men operated for a penile malformation (hypospadias) in childhood and in 34 matched controls. Two cases of severe hypospadias had signs of hypergonadotrophic hypogonadism. Three moderately severe cases possibly had a central relative androgen receptor insensitivity. Patients had lower levels of 5 alpha-dihydrotestosterone in serum, unrelated to the severity of the hypospadias. The hypospadias patients have previously been shown to be more neurotic and inhibited both socially and sexually than the controls, which might be related to defective androgenic functioning. Relations between androgen levels and psychological variables were studied. Low testosterone levels were related to higher hostility scores in Rorschach. Relations between androgens and other personality characteristics could not be shown.

Irreversible binding of acrylonitrile to nucleic acids.

1. [2,3-14C]Acrylonitrile was incubated with rat-liver microsomes, NADPH and either DNA, RNA or bovine serum albumin. Irreversible binding occurred to the macromolecular targets. Binding was lower when incubations were performed without microsomes. 2. Most of the 14C bound to DNA, RNA or polynucleotides (poly-A, poly-C, poly-G, poly-U) after incubation of [2,3-14C]acrylonitrile with rat-liver microsomes and 'conventional' re-isolation of the nucleic acids was removed from the macromolecular target when subsequently chromatographed on hydroxyapatite. 3. Radioactivity attached to DNA after prolonged non-enzymic incubations with [2,3-14C]acrylonitrile was also removed from the DNA by chromatography on hydroxyapatite. 4. When [2,3-14C]acrylonitrile was administered to rats (i.p.), incorporation of 14C into the natural bases of hepatic RNA was observed. In contrast with previous experiments with [1,2-14C]vinyl chloride, no radioactive [1-N6]etheno-adenine could be detected in RNA. 5. After administration of [2,3-14C]acrylonitrile to rats, hepatic DNA was isolated and hydrolysed by a modified enzymic procedure. Chromatography on PEI-cellulose showed two 14C peaks which did not co-chromatograph with any known standard. The amount of 14C in these presumed alkylation products was too low to allow chemical identification. 6. It is concluded that acrylonitrile, either itself or its metabolites, can alkylate nucleic acids. However, the extent of irreversible nucleic-acid binding is quantitatively much less than that observed with vinyl halides.

The prenatal development of hair follicles and hair in the one-humped camel (Camelus dromedarius).

The development of the hair follicles started in the prescapular region of CRL 15 cm-fetuses as follicular plugs. At CRL 31 cm, they became cylindrical cell cords. In the CRL 45 cm-fetus two types of growing follicles, primary and secondary ones, could be differentiated, while the tertiary follicles were still follicular plugs. The follicles formed groups, and the primary follicles attained their complete structure at CRL 79 cm, while the secondary follicles at CRL 83 cm. The tertiary follicles were present in the full-term fetus. The latter follicles showed the phenomenon of pairing, where two, three, four or five follicles shared in a common hair canal and arose from the epidermis independently.

The development of the sweat and sebaceous glands in the skin of the one-humped camel (Camelus dromedarius).

The sweat gland primordium appeared as a solid bud from the ental side of the epithelial cords of the growing hair follicles in the CRL 36 cm-fetus from the primary hair follicles and in the CRL 64 cm-fetus from the secondary ones. In the CRL 68 cm-fetus the glands were tubular with a slight curving of their lower third, which became convoluted at the CRL 83 cm-stage. At the latter age, clear myoepithelial cells were observed in the secretory portion as also in the duct. The buds of the sebaceous glands could be observed in the CRL 45 cm-fetus from the primary hair follicles and in the CRL 68 cm-fetus from the secondary ones. At the CRL 83 cm-fetal stage, the branched glands were elongated in shape and opened into the follicles by two separated ducts, united at the hair canal. The glands were in function nearly from its early development.

[Experiences with gynecologic laparoscopy as a routine method].

Report on 631 gynecological laparoscopies performed from January 1978 till March 1982. The main indications are ectopic pregnancy, acute and chronic pelvic inflammatory diseases and tumours of the ovaries. It is necessary to be critical against the palpation findings and the clinical diagnosis acute or chronic salpingooophoritis. There was a concordance of 51% with laparoscopic diagnosis. The importance and necessity of laparoscopic tubal sterilization is commented.
