[Post-operative surveillance and intensive care after orthopedic procedures of the vertebral column (author's transl)].

113 corrective procedures of the vertebral column by ventral and/or dorsal approach have been analysed. The post-operative findings have been reported, including blood-gas analysis, hemoglobin and hematocrit, electrolytes, and fluid balance. A correlative-statistical analysis of the scoliosis group was carried out according to number of involved segments, degree of curvature, age, length of operative time and total blood loss. The post-operative complications were related to preexisting risk factors.

[Innominate osteotomy (Chiari) for the treatment of coxa magna--preliminary results of 17 patients (author's transl)].

This is a report on the validity of treatment of coxa magna hips resulting from Legg-Calvé-Perthes-disease or hip dislocation by Chiari's pelvic osteotomy. Only severely deformed, incongruous and subluxated hips were treated by this operation. The aim of treatment is the development of congruency with a well covered femoral head to prevent an early occurence of osteoarthritis of the hip joint. Our results were compared with the results of nonoperative treatment of comparable patient groups.

[The operative treatment of kyphose (author's transl)].

Report is given on 17 patients with operative treatment of their kyphoses of various etiology. The correction itself has been carried out following diverse preoperative methods of correction using an anterior and/or dorsal approach to the spine. Altogether an average correction of 22.3 degree, that is 48 per cent could be obtained.

[Problems and results of Arthroplasty of the Knee Joint].

Since 1970 43 total knee arthroplasties were performed and the results of 34 operations reviewed. The average follow-up period was 2.4 years with a minimum of 6 months. We used 2 McIntosh, 8 Sledge, 9 Shiers, 8 Guepar, 3 Geomedic, 4 GSB prosthesis. The clinical results were very good in 56.2%. The failure and the high rate of loosening of the Shiers-Joint are stressed.

The treatment of chronic knee joint synovitis with radioactive isotops (author's transl).

On the basis of 77 cases report is given on the treatment of chronic synovitis of the knee in rheumatoid arthritis with radioactive isotopes. Gold 198 and Yttrium90 have been applicated. During an observation period up to 6 years a follow-up on 46 knee joints has been carried out. Very good and good results could be achieved in 50% of these cases.

Low and high pH form of cadmium carbonic anhydrase determined by nuclear quadrupole interaction.

The pH dependence of the nuclear quadrupole interaction between the excited 247-keV state in 111Cd bound to the active site in human carbonic anhydrase B and the nearest protein surroundings has been studied by means of the nuclear spectroscopic technique of perturbed angular correlation of gamma rays. The enzyme has been studied in the pH region 5.6-11.0 at 22 and -196 degrees C. The results show that the Cd enzyme changes from one form at low pH to another form at high pH both at 22 and -196 degrees C. The pK of the transition is 8.9 +/- 0.2 at -196 degrees C and close to 9 at 22 degrees C. Parallel to this transformation, the esterase activity of the Cd enzyme for the hydration of p-nitrophenyl acetate exhibits a pH dependency with a pH of 9.1 +/- 0.2. The sulfonamide inhibitor acetazolamide completely inhibits this activity of the Cd enzyme. The quadrupole interaction parameters for the Cd enzyme are not significantly different at -196 degrees C from those obtained at 22 degrees C. A measurement at 0 degrees C pH 5.7 shows, however, a form different from those at 22 degrees C pH 5.6 and -196 degrees C pH 5.7. The change in the quadrupole interaction with pH is, in a simple model, consistent with an ionization of a metal-bound water molecule.

Learning theory and gestalt therapy.

This article discusses the theory and operations of Gestalt Therapy from the viewpoint of learning theory. General comparative issues are elaborated as well as the concepts of introjection, retroflextion, confluence, and projection. Principles and techniques of Gestalt Therapy are discussed in terms of learning theory paradigm. Practical implications of the various Gestalt techniques are presented.

[Cholinergic contraction of the dog urinary bladder by electrical stimulation of the n. pelvicus and the ligamenta lateralia nerves and its affection by atropine and scopolaminebutylbromide (author's transl)].

As is well known contraction of the urinary bladder induced by electrical stimulation of the N. pelvicus-hypogastric plexus in the dog can only be diminished by atropine but not completely abolished. The result of stimulation can, however, be quantitatively abolished by the quaternary ammonium base hyoscine-n-butylbromide (scopolaminebutylbromide, Buscopan). Apart from antimuscarine-like peripheral effects, Buscopan also possesses, in contrast to atropine, antinicotine-like ganglion-blocking properties. Since Buscopan possesses only anticholinergic properties the contraction triggered off by stimulation of the nerve can be considered as being purely cholinergic. In the collateral ligaments of the urinary bladder of the dog there is another nervous system which when electrically stimulated contracts the bladder. Depending on the dosage the effect of stimulation can be diminished or abolished by atropine as well as by Buscopan. The result of stimulation can be diminished by hexamethonium and increased by eserine (physostigmine). Because of the effect of atropine-Buscopan (inhibition) and eserine (increase) this nervous system can be characterised pharmacologically as cholinergic.

Motilin effects on gastrointestinal motility and lower esophageal sphincter (LES) pressure in dogs.

Natural porcine motilin induces phasic contractions of the lower esophageal sphincter in dogs. These contractions are related to gastric contractions, which have the characteristics of interdigestive-motility patterns. Duodenal alkalinization produces an insignificant LES pressure increase after 5 minutes. No effect of motilin was observed when using isolated canine muscle strips.

Continuous Fetal Scalp and Carotid Artery Oxygen Tension Monitoring in the Sheep.

Fetal scalp and carotid PO2 were recorded continuously by polargraphic oxygen electrodes at several levels of maternal oxygenation. The results were compared to measurements obtained by intermittent blood samples using the radiometer blood gas analyzer. There was a good correlation between the PO2 values measured by the implanted oxygen electrodes and by conventional analyses of blood samples collected anaerobically. Electrode measurements differed from the control radiometer values by a mean of +/- 1.2 mmHg; 2 SD= +/- 2.50. Scalp PO2 recordings were lower but paralleled the carotid recordings under varying degrees of maternally inspired O2 concentrations. Thus scalp PO2 levels reflect closely those in the central fetal circulation. Administration of 95% O2 to the mother resulted in a 27% increase of scalp PO2 and a 29% increase in the carotid artery PO2 when compare to room air.

[General pharmacology and secretion inhibitory action of (8r)-3alpha-hydroxy-8-isopropyl-1alphaH,5alphaH-tropanium bromide-(+/-)-tropate (ipratropiumbromide) (author's transl)].

(8r)-3alpha-Hydroxy-8-isopropyl-1alphaH,5alphaH-tropaniumbromide-(+/-)-tropate (ipratropiumbromide, Sch 1000) is a quaternary tropic acid tropane ester with pronounced anticholinergic activities (inhibition of secretion and spasmolysis). The effect of the substance of all parasympathetically innervated organs ist 1.4 to 2 times stronger than that of atropine. As an inhibitor of the secretion of free hydrochloric acid in the stomach it proved to be 5 times more effective than atropine. Central activities were not observed. The duration of action of Sch 1000 exceeds that of atropine by far.
