Effects and side effects of radiosynovectomy with Yttrium 90 on rheumatic joint cartilage.

8 and 12 weeks after Synovectomy with Yttrium 90, cartilage specimens from the femoral condyle of 8 patients with R.A. were examined. Cellular and matrix damage could be detected. Intracellular particles containing Circonium, an Yttrium decay-product could be found using microprobe X-ray analysis. This was verified in experimentally induced arthritis in the rat.

The transgluteal approach to the hip joint.

A modified transgluteal approach to the hip joint without osteotomy of the trochanter is described. The advantage of this approach is a better view of the acetabular rim, prevention of gluteal muscle damage and protection of the superior gluteal nerve.

The Chiari pelvic osteotomy--indications and results.

Chiaris pelvic osteotomy with or without intertrochanteric osteotomy was performed in more than 100 cases during the past ten years. An evaluation of 97 operations showed good results in younger patients (age groups from 5-17 years). In adults with evident signs of arthrosis a higher rate of failures was obvious and therefore the indication for the pelvic osteotomy in this group of patients should be restricted.

[The LE cell phenomenon. New aspects in molecular biology and immunocytology].

1. The effects of antinuclear antibodies on nuclear material can be observed directly in free leucocyte nuclei in the supravital preparation according to Engel. 2. The antinuclear antibodies prevent the lysis of free nuclei, they obstruct the normal degeneration of nuclei. 3. The nuclear blockade is recognizable in the dark-clodded, rigid nuclei which remain small. These blocked nuclei are the visible expression of a development of complexes between chromatin and antinuclear antibodies. 4. It is supposed that the protective effect of antinuclear antibodies consists of the obstruction of the DNases and histon-proteases bound to chromatin. 5. The effects of antinuclear antibodies is in agreement with the structure of the nucleosomes of chromatin. 6. The nuclear blockade caused a persistence of antigenic material, which can create an increase of antibody production. This means that in vivo the antinuclear blockade is an effect which sustains illness.

Intracellular electrolyte concentrations in epithelial tissue during various functional states.

The scanning electron microscope together with an energy dispersive system was used to quantify intracellular elemental concentrations of transporting epithelial cells under a variety of experimental conditions. In the frog skin, the Na transport pool is comprised of the intracellular compartments of all vital cells in the different cell layers, except the mitochondria rich cells. The Na content of this transport pool exchanges easily with the epithelial (outer) bathing solution. Vasopressin increases the Na permeability of the corial cell barrier. Proximal and distal tubular cells of the rat kidney show differences in the pattern of intracellular element concentrations. The distal tubular cell is more resistant to 20 min of ischemia than the proximal cell. The changes in intracellular electrolyte concentrations following 60 min of ischemia are reversible after reperfusing the kidney with blood for 60 min. In the cells of the frog skin and rat kidney, Na and K were equally distributed between the cytoplasm and the nucleus. Differences exist for P, Cl and dry weight.

[Critical findings of captitate osteotomy in the treatment of lunatomalacia].

Five patients with Kienböck's disease had been treated by an osteotomy of the capitate (GRANER) with three poor results. In two cases necrosis of the capitate bone and resorption of the cancellous bone graft were obvious, perhaps due to damage of important vascular structures. Anatomical studies which have been carried out, showed that the main vessel of the capitate arises from the deep palmar arch and joints the bone on its ulnar-palmar and distal aspect. In order to achieve more satisfactory results with this operative procedure, the authors feel that it is important to protect the above mentioned vessel and indicate a more proximal osteotomy-site of the capitate bone.

[Nucleophagocytosis in monocytes--an immunocytologic phenomenon of anti-dna-antibodies].

In SLE-sera antinuclear antibodies pervent normal nuclear degeneration, i.e. lysis of free nuclei. In sera with high titers, small rigid LE-bodies develop rapidly, which are soon phagocytized in toto by neutrophil granulocytes and monocytes. Classic LE-cells as well as Tart-cells are formed. The foreign appearance of the phagosomes in the monocytes is a result of the different digestive processes occuring in these phagocytes. With decreasing serum activity, the LE-bodies expand somewhat more and become viscous. From these nuclei, relatively small portions are phagocytized fairly late. Sjögren-cells of the granulocytic as well as the monocytic type are generated. Nucleophagocytosis only occurs in the presence of antinuclear antibodies, regardless of whether it leads to LE-cell, Tart-cell or partial phagocytosis, i.e. Sjögren-cells of the granulocytic or monocytic type. A non-specific nucleophagocytosis does not exist.

[The operative treatment of scoliosis and kyphosis (author's transl)].

The first part of this paper consists of a discussion of the prognosis of the main forms of scoliosis and the operative treatment of this condition. The various modes of pre-operative curvature correction are detailed, as well as the main operative techniques from the dorsal (mainly the Harrington procedure) and ventral spinal approach (dwyer procedure, wedge resection). The second part describes the operative treatment of kyphosis. The most important complications are described.
