[Clinical and immunologic studies of the use of thymus polypeptide factor (thymostimulin) in chronic mucocutaneous candidiasis].

T-cell lymphocyte subpopulations were successfully converted by means of thymus polypeptide factor in a 17-year-old boy with chronic mucocutaneous candidiasis and combined immunodeficiency. It was demonstrated that the injections caused normalization of the peripheral T-lymphocytes subpopulations and a better state of general health. Normalization was due to a minimal increase in total peripheral T-lymphocytes and T-helper cells and a significant decrease in T-suppressor cells.

[Aseptic necroses of the carpal bones].

The most frequent aseptic necrosis in the carpal area is Kienböck's disease, followed by necrosis of the scaphoid and capitate. Necrosis of carpal bones are seen after different conditions as chronic traumatism, cerebral palsy, chemotherapy, as well as deficient vascularisation of the carpus. Varietys of the wrist joint anatomy may contribute to development of Kienböck's disease. The natural course of the disease shows progressive necrosis and bone destruction in adults. However the clinical symptomatology may show a higher degree of variation. Prevention of carpal dissociation seems to be important. Early surgery by vascularisation techniques and retention of the necrotic bone are indicated. In stages III and IV with collapsing deformity, resection-interposition arthroplasty using autologous material, or silastic spacers, have proven to be successful.

[Initial manifestation of eruptive exanthematous psoriasis vulgaris caused by captopril medication].

Angiotensin-converting enzyme (ACE) is identical to the dipeptidyl carboxypeptidase kininase II, which inactivates bradykinin. That is why captopril, an orally active ACE inhibitor, alters the kallikrein-kinin-arachidonic acid system. It is suggested that this mechanism could be responsible for the dosage-dependent incidence of polymorph cutaneous eruptions during therapy with captopril. A 42-year-old female patient with no previous personal or family history of psoriasis developed an eruptive-exanthematous type of psoriasis vulgaris 1-2 weeks after the beginning of captopril therapy. It is suggested that the onset of the dermatosis was produced by a blockade of bradykinin inactivation. This might have increased the production of arachidonic acid and led to elevated concentrations of inflammatory mediators.

[Effect of azelaic acid on the growth of melanoma cell cultures in comparison with fibroblast cultures].

Azelaic acid, a linear C9 dicarbonic acid, can effect a reduction in the growth of melanoma cells. This was shown in a comparison of two human melanoma cell cultures and cultured human skin fibroblasts. The cultures were observed for 24 days. The reduction in growth effected by azelaic acid showed significant differences concerning the dose-effect ratio in both melanoma cell lines observed. There was no inhibition of cell proliferation in the fibroblast cultures. Morphological and final trypan blue tests suggest that azelaic acid is not cytotoxic but cytostatic in a selective way.

General pharmacology of brotizolam in animals.

Brotizolam (2-bromo-4-(2-chlorophenyl)-9-methyl-6H-thieno[3,2-f]-1,2,4-triazolo [4,3-a]-1,4-diazepine, We 941, Lendormin) is a thienotriazolo-diazepine with profound sedative and hypnogenic properties. The side effects of the drug on general behavior, motocoordination, feeding pattern, body temperature, uropoietic and gastrointestinal functions, cardiovascular system, and respiration, as well as interactions with some biogenic amines are reported and discussed. The findings correlate with those known for other diazepines. Accordingly, effects on motocoordination were prominent, but were limited to an ataxia, whereas even extremely high doses scarcely eliminated the postural reflexes. Sleeping animals could invariably be woken and were capable of locomotion; thus, no comatose condition developed. The cardiovascular functions were not appreciably altered by brotizolam in anesthetized cats, while in conscious dogs minor fluctuations of blood pressure and heart rate occurred. Respiration was clearly inhibited when brotizolam was given intravenously. The cardiovascular effects of acetylcholine, norepinephrine, epinephrine, isoprenaline, and histamine were only slightly modulated. The orexigenic and hypothermic effects equalled those of other diazepines. The functions of kidney, stomach, and intestines were not affected. The entirety of the observations procured in ten different species suggest that brotizolam is well tolerated when given orally.

[Striated lupus erythematosus following the Blaschko lines].

We describe a striated cutaneous lupus erythematosus following the lines of Blaschko. These are lines along which cell populations having a common stem cell spread. If this stem cell is genetically altered, the lines of Blaschko are made visible by linear skin diseases. In the case of striated lupus erythematosus described here this genetic alteration remained unnoticed until, due to the influence of possible injury, the affected cells became antigenic, provoking a localized autoimmune disease.

[Fractionated afterloading irradiation as a new procedure for treating inoperable brain tumors].

A method is shown for fractionated afterloading therapy of inoperable cerebral tumors. A 3 mm thick, tube-form applicator of noble metal which is closed at the front side is implanted into the tumor by a stereotaxic technique and firmly screwed together with the osseous calotte. It remains there up to two weeks, giving full mobility to the patient. The afterloading therapy with a moving iridium source is performed one or two times per day with individual doses of 2 Gray. Thus, the chronological and--to a certain extent--also the spatial dose distribution is variable when applying this method.

Vascular resistance, metabolism and EEG within cerebral grey and white matter during hypoxia in neonatal piglets.

To study unsolved problems of the causal chain of neonatal hypoxic brain damage in 31 hypoxic newborn non-anesthetized piglets cerebro-vascular, metabolic and EEG reactions were investigated (FiO2: 0.06-0.10, 1 h). Only in artificially ventilated newborn piglets this acute hypoxic hypoxia provoked a vital decompensation by critical depression of mean arterial blood pressure (less than or equal to 4.67 kPa [35 mmHg]) and/or a critical increase of vascular resistance (Rc) in the cerebral white matter (greater than 50%). Spontaneously breathing piglets survived always showing hyperventilation, higher pHa and an increase of cerebral O2-consumption in cerebral grey matter, partly also in white matter. This critical increase of Rc was related to a critical decrease of O2-consumption in the white matter and an insufficient decrease of Rc of the grey matter. The observed strong metabolic and hemodynamic differences between these two brain compartments can explain the evaluated special morphological vulnerability of cerebral white matter in ventilated animals.

Differences in direction specificity of receptive fields in upper and lower layers of the cat's prestriate area 18.

Direction specificity was measured for neurons in area 18 (A18) in penetrations approximately perpendicular to the layers in awake cats. A majority of lower layer cells (below 1.0 mm) preferred stimuli moving at rather orthogonal directions with respect to the preferred directions of the neurons in the upper layers (0-1.0 mm). The results suggest more functional autonomy of the corresponding cortical layers.

Polyacryl prosthesis for cranioplasty--their production in silicon rubber casts.

The frequently observed osteolysis after reimplantation of deep-frozen skullcap bone have led us to use polyacryl as the material of choice if a secondary covering of a skull bone defect becomes necessary. Polyacryl has an excellent biological compatibility but often yields unsatisfactory cosmetic results when the modelling is done at the site of the intervention, especially in the case of defects in the forehead region. A method is described that allows the preparation of a prosthesis before the actual operation identical with the original bone in every detail. The surgery is then confined to the anchoring of the prosthesis to the bone edges.

Use of gut cyclic motor activity to evaluate a stimulant (narcotic) and inhibitor (anticholinergic) of gastrointestinal-tract activity in the unanesthetized dog.

The use of gut cyclic motor activity to evaluate both a stimulant and inhibitor of gastrointestinal contractile activity in the unanesthetized dog is described. In the interdigestive (fasted) state, there is a cyclic alternation of intense periods of contractions (burst) with quiescent periods (basal). Codeine phosphate, a narcotic agent, caused a dose-dependent increase in the number and duration of circular muscle contractions when administered in the basal phase. Hyoscine N-butyl-bromide, an anticholinergic drug, caused a dose-dependent reduction in both amplitude and frequency of burst-phase contractions. When a combination of codeine phosphate and hyoscine N-butylbromide was administered simultaneously in the basal or burst phase, the hyoscine N-butylbromide prevailed and had the dominant effect. This is a useful in vivo model to evaluate the effects of drugs on the gastrointestinal tract.

First magnetoencephalographic recordings of the brain activity of a human fetus.

Using a one-channel neuromagnetometer adjusted to a special site on the mother's abdomen, we succeeded in recording prenatally, for the for the time, human fetal brain activity in late pregnancy. It was possible to record both the fetal auditory-evoked neuromagnetic field and to detect fetal brain activity by analysis of the frequency spectrum. Such measurements may soon prove valuable for the noninvasive investigation of human brain function.
