[Use of process and turning point in psychotherapy--significance of the theatrical aspect as a cooperative mode of behavior].

The paper presents observations respecting the contrast of playful and grim attitudes in the behavior of patients undergoing psychotherapeutic treatment. Playful correspondence to Erikson's Cooperative Stage of development and Freud's mature Genitality, while grim corresponds to Erikson's Autonomous and Assertive Stages and Freud's Anal and Phallic Stages. To be effective, theatrical performances have to be produced in a cooperative way; psychotherapy too must assist the patient to achieve a greater degree of cooperativeness and mature Genitality. Identifying with the therapist and adapting his style can enable the patient to acquire self-composure through playful behavior, and to overcome protracted grimnes. Play-matured Genitality Cooperative signifies that the patient incorporates his partner or opponent's intentions into his appreciation of events, enabling him to act more in accordance with the reality around him.

[Cardiovascular reflex evaluation for detecting autonomic neuropathy in chronic dialysis patients].

3 cardiovascular reflexes were tested in 25 patients on regular haemodialysis: measurement of the beat-to-beat variation in heart rate in the supine position and during deep breathing, measurement of heart rate response to change in posture (30:15 ratio). 8 patients (32%) had abnormal results in at least two of the tests, 3 patients (12%) had abnormal results in all 3 tests. Beat-to-beat variation was found to be the most sensitive test, the heart rate difference during deep breathing showing a direct correlation to the nerve conduction velocity of the peroneal nerve. Hence, a common cause can be postulated in the pathogenesis of the autonomic lesions and the sensorimotor polyneuropathy in the patients under investigation. Furthermore, the group with the most severe radiological calcification due to Mönckeberg's medial sclerosis had a significantly lower mean heart rate difference during deep breathing than the group with no radiological calcification. A similar correlation between autonomic neuropathy and Mönckeberg's medial sclerosis has recently been described in diabetic patients.

Elimination of amino acids in chronic renal failure.

Elimination of parenterally administered amino acids was investigated in patients with chronic renal failure (CRF) (10 on conservative treatment = CT, 13 on regular hemodialysis therapy = HD). In a bolus injection protocol 0.1 g amino acids as a 10% solution containing essential and nonessential amino acids were infused and pharmacokinetic parameters of 17 amino acids were calculated. The mean elimination half life was increased by 40% in CT and 87% in HD (p less than 0.001 CT and HD versus controls). Total clearance was reduced in CT (p less than 0.001 versus HD and controls). In CT clearance of phenylalanine, proline, alanine, histidine and arginine was reduced, of none of the amino acids elevated. In HD clearance of methionine, lysine, aspartic acid and serine was increased, of proline decreased. The total transfer rate was reduced in CT (p less than 0.025 versus controls) and transfer of threonine, aspartic acid, glutamic acid, alanine, histidine and arginine was reduced in these patients. Mainly due to elevated basal plasma concentration transfer of methionine was elevated in CT and HD. In dialysis patients transfer of isoleucine, tryptophan, glycine and serine was increased. Despite variations of absolute values of clearance between the two groups investigated relative clearance rates of amino acids were similar because of a uniform increase of clearance of about 37% in HD compared to CT (p less than 0.001). Results indicate that the elimination of parenterally administered amino acids is grossly altered in uremia and that in patients on CT and HD a uniform elimination pattern can be observed.

AIDS in a child: the family physician's role.

Children or adults with AIDS may be seen first by family physicians, who must be aware of the epidemiology, signs and symptoms, and initial laboratory evaluation of these patients. Numerous complications are to be expected. The physician can provide reassurance that AIDS is not transmitted through casual contact, can offer valuable support to the AIDS patient and the family, and can serve as a source of accurate information.

[Results of a multicenter study of the early detection of glomerulonephritis. 1].

In the framework of a multicentric retrospective study between selected clinics of the Republic Austria and the GDR anamnestic, clinical and paraclinical data were investigated in their valency for the early recognition of glomerulonephritis. Data of 583 patients were evaluated. Hereby it was shown that independent of the size of excretion and the reproducibility the findings "proteinuria" are of particular significance for the early recognition. The serological investigations usually performed within the diagnostic of glomerulonephritis proved as insignificant for the early recognition. Since the establishing of an exact diagnosis is up to now possible only with the help of invasive methods, a call on research is made to develop reliable, non-invasive diagnostic methods.

[Comparative pharmacokinetics of the beta-1 receptor blookader celiprolol after a single oral administration in subjects with health kidneys and in patients with impaired renal function].

Pharmacokinetic parameters were determined after the oral administration of a single tablet containing 200 mg celiprolol HCl to 8 healthy volunteers with normal kidney function and 6 patients with stable impaired renal function of different degrees of severity. The plasma levels and the urinary excretion of celiprolol were followed up for 48 hours after administration with the help of specific chemo-analytical methods. The substance was equally well absorbed both by healthy volunteers and patients with renal disease. While the cumulative renal excretion (0 to 48 hours after administration) was reduced from 18.2% in healthy volunteers to 2.3% of the applied dose in kidney patients, the global elimination constant k fell from 0.104 h-1 in healthy volunteers to 0.072 h-1 in the patients with renal disease. It can be assumed that in patients with impaired renal function at least a part of the lacking renal elimination of celiprolol can be taken over by "extrarenal elimination". According to the method of Dettli a reduction in dosage of the beta 1-blocker, celiprolol is not necessary in patients with more or less impaired kidney function.

High levels of plasma protein C in nephrotic syndrome.

In patients with severe nephrotic syndrome determinations of plasma protein C: Ag levels (8 patients: 5 adults, 3 children) and protein C activity (3 out of 8 patients) revealed significantly elevated plasma protein C concentrations. Furthermore we observed a significant inverse correlation of protein C: Ag to AT III: Ag levels. No protein C: Ag could be detected in the urine of two patients studied. We conclude from our data, that changes of plasma protein C do not contribute to the high thrombotic tendency in nephrotic syndrome.
