[Echographic investigation of the calyceal system in cases of pyelitis gravidarum].

Dilatation of the upper urinary tract can be found even in pregnancies of healthy women. The frequency of dilatations in patients with pyelitis gravidarum is higher. Comparing the symptoms -- costovertebral tenderness and significant bacteriuria -- with the nephrosonograms it can be demonstrated that urinary tract infections incline to reoccur in patients with dilated upper urinary tract despite adequate antibiotic therapy. Echographic examination of the calyceal system during pregnancy might therefore be useful in detecting risk patients who have to undergo intense urologic surveillance.

[Intravesical prostaglandin E2 and placebo in urinary retention after gynaecological surgery (author's transl)].

In a randomized single blind trial we studied the effectiveness of intravesical prostaglandin E2 in the treatment of postoperative urinary retention after vaginal hysterectomy with colposuspension. One group of patients (n = 8) received 1.5 mg prostaglandin E2 in 50 ml 0.9% NaCl on the 9th postoperative day (72 hours after removal of the Foley catheter) and 50 ml NaCl without prostaglandin E2 on the following day. In the second group (n = 12) NaCl was instilled on the first day of treatment and prostaglandin E2 on the following day. Four hours after the instillation of prostaglandin E2 5 out of 8 women were able to void spontaneously, while after the instillation of NaCl spontaneous micturition occurred in only 2 out of 12 women (p less than 0.05). In both groups the average time period until patients were free of residual urine was 4 days after the initiation of therapy. Compared to placebo intravesical prostaglandin E2 is significantly more effective in achieving spontaneous voiding after gynaecological surgery.

Corneal sensitivity during the menstrual cycle.

Corneal sensitivity was determined in 5 women with regular menstrual cycles by means of a new aesthesiometer developed by Dräger. Throughout one menstrual cycle we measured corneal touch thresholds (CTT) daily and did serial determinations of urinary luteinising hormone and pregnanediol levels. CTT rose in 4 women with proved ovulation in the days before or on the day of ovulation, while corneal sensitivity remained unchanged in one anovulatory subject. We observed no changes in CTT prior to menstruation. The decrease in corneal sensitivity might be related to the preovulatory oestrogen peak.

[Incidence and importance of gestosis symptoms (author's transl)].

The assumption is made that prophylactic prepartum attention has helped to reduce the incidence of gestosis symptoms in recent years. However, random evaluation of findings obtained from 835 pregnant women in the First Gynaecological Hospital of the University of Vienna over a period of six months has shown that careful recording of all gestosis symptoms has remained to be imperative during pregnancy and on hospitalisation for delivery. Meconium staining, frequency of surgery, newborn weight, and Apgar score after one of five minutes were taken into consideration for evaluation of results. Gestosis symptoms were recorded from 186 pregnant women (22.3 per cent). Index 1 was recordable from over 50 per cent of these, Indices 1 to 3 from 170, and Indices of 4 and more from 16. A comparison with a control group showed for the above evaluated parameters that, for all practical purposes, there was no difference between pregnant women without gestosis symptoms, on the one hand, and those with Indices up to 3, on the other. Differences were clearly established, when it came to Indices of 4 and above. Therefore, the question is raised, if the term of "gestosis" should not be applied only to pregnant women with Indices of 4 and more.

Placental function, fetal distress, and the fetal/placental weight ratio in normal and gestotic pregnancies.

The ratio between fetal and placental weight is often thought to be a measure of the reserve capacity of the placenta. The aim of our study was to investigate the relationship between 1) endocrinologic parameters during pregnancy (serum placental lactogen - HPL, urinary estriol - E3), 2) the occurrence of fetal distress during labor, and 3) the severity of EPH gestosis, and the fetal/placental weight ratio at various gestational ages. The data from a total of 4911 consecutive pregnancies and deliveries were evaluated. Up to 37 weeks the mean fetal/placental weight ratio was significantly lower in infants with fetal distress. Up to 32 weeks there was a positive correlation between the percentage of women with low HPL and E2 levels and the percentage of infants with fetal distress, and the gestosis index. In addition there was a significant increase in the mean fetal/placental weight ratio in the group with moderate and severe gestosis. With advancing gestational age fetal/placental weight ratios were independent of the severity of EPH gestosis. It is concluded that until 37 weeks fetal distress is associated with a significant lower fetal/placental weight ratio. The morphologic and functional changes in placentas of gestotic pregnancies do not manifest themselves in either an increase or a decrease of the mean fetal/placental weight ratio after 33 weeks.

[The prognostic significance of stromal reaction and other histopathological parameters after Wertheim radical hysterectomy (author's transl)].

From 1969 to 1975 138 women were treated by Wertheim radical hysterectomy for invasive cancer of the cervix (28% stage IA, 54% stage IB, 9% stage IIA, and 9% stage IIB). 121 women were followed up for 5 years. The 5-year survival rate was 100% in stage IA cases, 88% in stage 1B and 75% in stage II patients. The following histopathological criteria and their prognostic significance were reviewed: stromal reaction, histological type, pattern of invasive spread, relationship to blood vessels and lymphatics, and regional lymph node metastasis. All patients with a strong stromal reaction were alive 5 year after the operation Undifferentiated carcinomas were seen much more frequently in the group of patients who died within 5 years. The pattern of invasive spread has no prognostic significance. All patients in whom the tumour had not spread to blood vessels and lymphatics survived more than 5 years. A strong stromal reaction seems to indicate a good host response and permits a more favourable prognosis to be made.

[Optical density and surface tension in amniotic fluid in the late pregnancy (author's transl)].

113 amniotic fluid samples from pregnant women between the 28th and the 41st week of gestation were examined for optical density and surface tension. Surface tension measurement was done by means of the Wilhelmy-balance which shows a very good correlation with the L/S-ratio. Optical density was measured at 405, 580, 650, 750 and 900 nm wave length. For the comparison with the surface tension optical 650 nm was used, since at this wave length only minimal absorbance of known biologic pigments occurs. Pulmonary maturity was diagnosed when the surface of the hysteresis area (dynamic surface tension measurement) was more than 20 cm2 or when optical density at 650 nm was false positive in 3,7% and false negative in 40,7%. The same percentage was found in the predication of fetal lung maturity by this method after fetal outcome. The measurement of the optical density is a rapid and simple method, but because of the high number of false negative results it is advised to use one of the more sophisticated methods for the prediction of fetal lung maturity in cases of a negative optical density reading at 650 nm.

Corneal sensitivity in pregnancy.

Only few studies are available on changes in corneal sensitivity during pregnancy, and most of the earlier work was done with instruments which gave only a rough approximation of the threshold of corneal sensitivity to touch. The new esthesiometer developed by Draeger permits, for the first time, an exact and reproducible determination of the threshold of corneal sensitivity to touch. We studied corneal sensitivity in 86 pregnant women between the 13th and 40th week of gestation. The thresholds were significantly higher in the study as compared to a control group of non-pregnant women. The decrease in corneal sensitivity was not related to duration of gestation, weight gain during pregnancy or mean arterial pressure at the time of examination. Possible mechanisms responsible for changes in corneal sensitivity in pregnancy are discussed.

[Clinical pattern of malignant lymphoma of the female genital tract (author's transl)].

Reported in this paper are three patients with malignant lymphoma. The first case is a low-grade lymphoma of the ovary, the second a low-grade lymphoma in the retroperitoneal region, and the third metastasis in both adnexae, resulting from high-grade lymphoma with primary location in the stomach. The disease was detected only in advanced stage in all three cases. Only in the third patient could the primary tumour be removed, owing to its more readily accessible location, with the adnexal metastases being removed one year later. The issues relating to malignant lymphoma in the region of the female genital tract are discussed, with reference being made to relevant literature.

[Experiences with a new questionnaire for preoperative evaluation of female urinary incontinence (author's transl)].

A new questionnaire with an urge-score and stress-score described by Gaudenz was put before 90 incontinent women scheduled to undergo vaginal repair. In addition to the history cystometry and simultaneous urethral pressure profilometry was done. According to the questionnaire 70 patients had evidence of stress-incontinence and 20 patients of urge-incontinence. A good correlation was found with the results of the urodynamic evaluation. The score does not reflect the severity of urinary incontinence. In patients with symptoms of both stress- and urge-incontinence the score emphasizes urge-incontinence. When a urodynamic work-up is not possible the questionnaire with the urge-score and stress-score is a very helpful tool for differentiating between stress- and urge-incontinence.

[Irradiation therapy of recurrent cervical carcinoma (author's transl)].

A report is presented of 141 irradiated cases of recurrent cervical carcinoma, 48 (34%) of the patients have survived for more than 2 years and 21 (15%) have survived yet more than 5 years. It is noticed, that the survival-rate is neither dependent on the localisation of recurrence, nor the beginning date nor the method of radiation therapy. It is pointed out, that the re-irradiation of primarily irradiated patients give also good results.

First-trimester serum prolactin levels in normal and complicated pregnancies.

Subfractional activities and some properties of beta-glucuronidase were studied at different ontogenetic stages of the human placenta. Maximum activity was localized in normal pregnancies rose from 13.58 ng/ml in the 6th week to 52.46 ng/ml in the 15th week of gestation with marked individual variability. In tubal pregnancies, serum hPRL was low and did not increase over a period of 3--6 weeks. In women with spontaneous abortion, hPRL levels often were in the normal range with a drop only immediately prior to expulsion of the fetus. In patients with missed abortion the drop in serum prolactin levels seemed to coincide with the death of the conceptus.

[Biorhythm theory and primary irradiation of inoperable cancer of the cervix (author's transl)].

Hundred patients with inoperable cancer of the cervix, who had been treated by primary irradiation, were reviewed with regard to biorhythm on the days of radium application. 5-year survival was lower in patients with unfavorable biorhythm; the differences, however, were not significant. It is concluded that the calculation of biorhythm for the day of radiotherapy has little or no influence on the results of therapy.

[Influence of body weight and weight increase on the development and severity of an EPH gestosis (author's transl)].

3385 consecutive records of deliveries at the Ist Department of Obstetrics and Gynecology of the University Hospital, Vienna, were reviewed. Particular attention was paid to increased weight gain during pregnancy--defined as a total weight gain of more than 13,4 kg--and EPH-gestosis. Patients were put into one of two groups: (1) no increased weight gain during pregnancy, and (2) increased weight gain of over 13,4 kg. Each group was further divided into women with no or only slight symptoms of gestosis (gestosisindex 0 to 3) and gravidae with moderate or severe EPH-gestosis (gestosis index 4 or greater than 4). The four groups of gravidae were compared with regard to perinatal complications and fetal outcome. While the average weight gain for all gravidae was 13,2 kg, patients with EPH-gestosis did not gain significantly more than non-gestotic women. However, body-weight before pregnancy was positively correlated with the symptoms and the severity of EPH-gestosis. It was concluded, that weight gain per se has very little, if any influence on the appearance of toxemia. Patients, who are overweight at the onset of pregnancy must be considered at risk for the development of moderate or severe EPH-gestosis.
