[Courses of pregnancy and deliveries of 107 women with longstanding desire for children. Comparison with normal group (author's transl)].

The records of a selected group of 107 women who became pregnant and delivered, after having been attended to for sterility at the First Obstetric and Gynaecological Department of the Viennese University Hospital, were reviewed and compared to data of a control group which had been selected at random. Mild gestosis was more often recorded from patients who had been treated for sterility (47 per cent), whereas moderate or severe gestosis was more often recorded from the controls. Tocolytic agents were orally administered to 30 per cent of those women who had been treated for sterility. Duration of labour was longer and dystocia more frequent in the sterility treatment group. The percentual rates of surgical delivery and caesarean section were about the same in both groups. SGA was recorded from 30 children of the first but only eleven children in the control group. The average birth weight of babies born to mothers, following treatment for sterility, was 120 g lower than that in the control groups. Particular attention should be given to placental insufficiency in any pregnancy, following treatment for sterility. There was obviously no increased frequency of severe obstetric complications.

[Cervical incompetence after previous cervical dilatation and curettage (author's transl)].

Out of a total of 3502 deliveries over a period of two years 254 patients (7.25%) had a surgical closure of the cervix according to Shirodkar because of cervical incompetence. All 3502 records were reviewed with regard to previous obstetrical and gynecological history. In the group with Shirodkar operation the percentage of women with a history of spontaneous or induced abortions was almost twice as high as in the comparison group. The reason for the development of cervical incompetence could be a trauma to the endocervix due to mechanical dilatation since diagnostic or therapeutic dilatation and curettage was found five times more often in women with Shirodkar operation than in patients without cervical incompetence. The results show that dilatation and curettage for any indication should be performed as carefully as possible, especially in younger women of childbearing age. Chemical dilatation of the endocervix--for example with prostaglandins--seems to be preferable to mechanical dilatation.

[The effect of antibiotic prophylaxis in vaginal obstetric procedures (author's transl)].

202 cases of vaginal obstetric procedures (forceps and vacuum extraction, breech delivery and manual removal) were reviewed with regard to assessing the value of prophylactic administration of antibiotics. Criteria for the evaluation were fever post partum, duration of stay in hospital and involution of the uterus. The results showed that the prophylactic administration of antibiotics had no significant effect on these parameters, with or without the simultaneous administration of oxytocics. Hence, in view of the risk of allergic reactions, toxicity and the selection of resistant strains the prophylactic administration of antibiotics does not seem to be justified in vaginal obstetric procedures. Scrupulous aseptic precautions during all vaginal or intrauterine procedures are considered essential for the prevention of infection.

[The effect of transcutaneous nerve stimulation on labor pain (author's transl)].

The analgesic effect of transcutaneous electric nerve stimulation (TNS) during delivery has been studied. 57 (55.9%) of 102 women treated considered TNS had given them good or very good relief from pain; 24 (23.5%) reported that they had received some relief, while 21 patients (20.6%) were of the opinion that they were not helped by the technique. CTG monitoring ocurred in all cases. No side effects on the fetus or the mother have been noted. A positive aspect of TNS was the quick course of delivery and the absence of obstetrical complications in the cases treated with this method. Therefore the designation of TNS as a new, riskless, but effective technique for pain relief during childbirth seems justified.

The role of intraoperative quick PTH measurements in primary hyperparathyroidism.

BACKGROUND: In patients with primary hyperparathyroidism (PHPT), circulating concentrations of intact parathyroid hormone (PTH), decline dramatically within minutes following surgical excision of hyperfunctioning parathyroid tissue. The magnitude of this decay correlates with the completeness of resection of hyperfunctioning parathyroid tissue and can be monitored during the operation. METHOD: Intraoperative Quick PTH (QPTH) monitoring and pitfalls of more than 350 patients, who were operated because of primary hyperparathyroidism are analyzed. Special attention is given to correct baseline values and interpretation of QPTH values. RESULTS: QPTH monitoring is able to distinguish reliably between single and multiple gland disease and is an indispensable prerequisite for any form of limited parathyroid exploration. Experience with QPTH monitoring is necessary to achieve the excellent results known from bilateral neck exploration. CONCLUSION: Applying correct baseline values and cautious interpretation of QPTH values results in excellent results. Nevertheless more data must be collected to allow reliable interpretation of QPTH monitoring in all patients with PHPT.

[Hormonal pregnancy parameters in the serum following surgery for extrauterine pregnancy].

We did serial determinations of beta-HCG, total-HCG, prolactin, SP-1, and progesterone in maternal serum after operation of an ectopic pregnancy in 38 patients. Prolactin, progesterone and SP-1 rapidly returned to normal values after removal of the ectopic pregnancy. However, the half life of HCG was related to the type of operation: after conservative operation of ectopic pregnancy the half life of HCG was greater than after tubectomy (41.4 +/- 1.9 SEM hours versus 33.4 +/- 2.0 hours), possibly due to lack of complete removal of trophoblast tissue. This fact should be kept in mind when evaluating postoperative HCG levels.

[Facilitation of the diagnosis of colpitis in pregnancy: the line test as a quantitative parameter of vaginal discharge].

To define the vaginal discharge in pregnancy with a numeric quantitative parameter and to determine out its pathognomonic importance, the vaginal discharge of 140 pregnant and 33 non pregnant women was examined and the women were asked to indicate the subjective degree of discharge in a visual linear analog scale (Line-test). Pregnant woman marked high Line test values more frequently than non pregnants irrespective of detection of a vaginitis. With increasing gestational age and parity higher Line test values were found irrespective of the frequency of vaginitis. With a cut off level of 80 mm, the specificity of the Line test concerning vaginitis is high below cut-off, the possibility to have vaginitis is increased. The Line test is simple to use and - because of its specificity and sensitivity - has been found useful in evaluating the degree of vaginal discharge.

[Prognostic value of changes in the fundus oculi in EPH gestosis].

We examined the ocular fundus in 100 unselected pregnant women with EPH-gestosis of different severity. In 52 women there were no retinal changes, in 32 women we saw I hypertensive changes according to the classification of Keith-Wagener, in 10 cases II, and in 6 cases III or IV retinal changes. There was no direct relationship between the severity of EPH-gestosis and I and II retinal changes; however, severe retinal changes (III IV) were associated with moderate or severe EPH-gestosis. We found a significant relationship between the degree of hypertensive retinal changes and fetal mortality. In the group of gravidae with severe retinal changes 4 out of 6 infants died. The examination of the ocular fundus permits the direct evaluation of fundal changes and the early recognition of fetal jeopardy.

[Changing risks in uterine rupture].

Between 1975 and 1981 7 cases of rupture of the uterus were observed at this department with an incidence of 1:1200. In all cases therapy consisted of hysterectomy: There was no maternal death, one child died within 24 hours after delivery. Rupture of the uterus remains a serious complication. Careful attention to risk factors, the avoidance of traumatizing obstetric operations and the judicious use of oxytocics should further reduce the incidence of rupture of the uterus.

[Pathophysiology and therapy of magnesium deficiency in pregnancy].

We determined serum magnesium(Mg)-levels in 67 pregnant women in late pregnancy. 42 gravidae complained of nightly muscle cramps; 21 of them received 1.8 g monomagnesiumaspartate twice daily per mouth for 4 weeks. 2 and 4 weeks after the initiation of therapy serum Mg-levels were again determined. Serum Mg-levels were lower in pregnant women as compared to a control group of non pregnant women. Gravidae complaining of muscle cramps had significantly lower serum Mg-levels than women without muscle cramps. The administration of Mg was associated with a significant rise in serum Mg-levels as early as 2 weeks after the initiation of therapy. Our study indicates that nightly muscle cramps during pregnancy might be a sign of a latent magnesium deficiency which can be influenced by oral magnesium.

[HELLP syndrome in routine obstetrical care. Three case reports].

In recent years the syndrome of hemolysis, elevated liver enzymes and low platelets (H-ELLP) has attracted increasing interest in obstetrics as a serious complication of pregnancy, either alone or in combination with the classical symptoms of EPH-gestosis or eclampsia. In 1993, we observed 3 cases of severe HELLP syndrome in a total of 1126 deliveries. We present the clinical characteristics and the laboratory findings in these cases. A common symptom was general malaise and upper abdominal discomfort or pain. All patients were delivered by cesarean section of healthy infants. We conclude that it is no longer sufficient to emphasize edema, proteinuria and hypertension, but that the signs and symptoms of the HELLP syndrome present a new and increasingly important challenge in obstetric practice.
