Classification of colorectal carcinomas with regional lymphatic metastases.

According to the UICC classification of colorectal patients with histologically confirmed regional lymph node metastases (pN1) are not further subdivided. In these patients, however, subgroups with widely differing prognosis can be established. A number of classifications proposed to cover these patients are discussed. The analysis of the Erlangen case material shows that it is decisive for the prognosis to know whether involved lymph nodes are located only at the branches of the supplying vessels ("peripheral"), or whether the lymph nodes along the trunks of these vessels ("proximal") are also involved. Among the patients with proximal lymph node metastases, the question as to whether the lymph node at the margin of the resected lymph drainage area ("marginal node") is involved or not, has no essential influence on the prognosis. The counting of metastatic lymph nodes does not represent an essential criterion for prognosis.

Frozen section diagnosis in tumors of the testis. Possibilities, limitations, indications.

When the clinical diagnosis is in doubt, biopsies obtained from areas suspicious for tumor of the testis can be examined using frozen sections after prior inguinal clamping. The correct tumor diagnosis was established in 81% of cases in the first biopsy, and in 11% in the repeat biopsy, making a total of 93% (n = 70). Frozen section examination is of considerably greater importance within the framework of lymphadenectomy in patients with non-seminomatous testicular tumours. In 1014 of such frozen section examinations, 98.4% of the frozen section findings were correct, 0.3% undecided and 1.3% false. The diagnostic sensitivity was 100%, the diagnostic specifity 98.6%, the technical sensitivity 84%, the technical specifity 99.8%. Lymph node metastases of up to 2 mm are only rarely detected by frozen section; for sizes between 2.1 and 10 mm, they are recognized in 85% of cases. In the setting of lymphadenectomy, the frozen section examination can be employed to monitor the extent of the dissection.

[Colorectal polyps - Diagnosis, classification, relation to carcinoma (author's transl)].

A growing incidence of colorectal polyps and carcinomas can be observed all over the world. Since 1 January 1978 the clinical-pathological appearance of colorectal polyps has been recorded at Erlangen. Endoscopy combined with histology is necessary for diagnosing them. Localization and histological classification of 2200 colorectal polyps are reported. The adenoma-carcinoma sequence is of paramount importance. Malignant areas can be demonstrated histologically in about 10% of all adenomas.

[Extended resection of pancreatic and periampullar carcinoma: regional, total and partial duodenopancreatectomy (author's transl)].

The prognosis of exocrine carcinoma of the pancreas is still quite bad; because of that, total duodenopancreatectomy following the procedure by Fortner has been our surgical approach since January 1978. Since that time surgery of periampullary carcinoma was extended as well in such a way, that first and second order lymph nodes were excised systematically. Following the introduction of these procedures the percentage of patients with exocrine pancreatic carcinoma operated upon rose from 12 to 35%, - the percentage of patients operated upon because of periampullary carcinoma correspondingly rose from 61 to 91% of all patients carrying these tumors. The incidence of lymphogenous metastases was 88% in 17 patients, which had surgery because of ductal pancreatic carcinoma, and 27% in 22 patients with periampullary carcinoma. 29% of patients with pT1-3 tumors and 71% of patients with pT4 tumors did have already lymphogenous metastases. In 22% of the cases, who would have been operated upon by conventional total duodenopancreatectomy metastases were found in the second order lymph nodes which were taken out according to the new more radical surgical approach; the corresponding figure for patients, who were operated upon by partial duodenopancreatectomy was 5%. Mortality of regional partial duodenopancreatectomy was 4% in our series, and mortality after regional total duodenopancreatectomy was similar to that of conventional pancreatectomy without dissection of lymph nodes.

[Safe aboral distance in the sphincter-preserving resection of the rectum].

The correlation between the aboral margin of clearance and the end results after anterior and deep anterior resection of the rectum were analyzed for 293 patients documentated in the Erlangen clinical-pathological registry of colorectal cancers. The distance between the aboral margin of the carcinoma and the resection line was measured under standardized conditions. The best prognosis (age-corrected 5-year survival rate and frequency of local recurrence) is given if the margin of clearance is more than 30 mm on resected fresh specimen without tension. This distance corresponds with 50 mm on the mobilized rectum in situ. This rule applied to carcinomas of Dukes' stage A and/or grade 1 as well as to more advanced tumors or tumors with a higher grade of malignancy.

[Burkitt lymphoma of African type in Europe (author's transl)].

A 34-year-old German woman who had never been to Africa developed a Burkitt lymphoma of African type during the fifth month of pregnancy. The diagnosis was confirmed during life by cytological and virological tests. There was a markedly increased antibody titre against the Epstein-Barr virus in serum as well as virus antigen in the tumour tissue. There was extensive involvement of almost all abdominal organs and of the breast. After spontaneous abortion of a female foetus in the sixth month of pregnancy cytostatic treatment was started, but without achieving remission. The patient died five weeks after admission to hospital; the autopsy confirmed the clinical findings.

Human prostatic adenocarcinoma: some characteristics of a serially transplantable line in nude mice (PC 82).

Transplantation of tissue from a cribriform human prostatic carcinoma resulted in a serially transplantable tumor line in nude mice, PC 82. During 2 1/2 years the tumor has not changed its histological appearance and remained a moderately differentiated adenocarcinoma. The cells contain large amounts of prostatic acid phosphatase. It has a slow rate of growth and regresses after castration and estrogen treatment. This preliminary information suggests that the PC 82 tumor line may be suitable model for hormone-dependent human prostatic cancer.

Prognostic groups in colorectal carcinoma.

A system of classifying colorectal carcinomas into five prognostic groups is described. This classification includes not only tumor spread, regional lymph node involvement, and distant metastases, but also resectability and R-classification (residual tumor or curability). All patients with colorectal tumors can be classified by means of this system. The five prognostic groups differ statistically significantly (p less than 0.01) from one another with respect to age-corrected 5-year survival rates.

Epstein-Barr virus-positive Burkitt's lymphoma in a German woman during pregnancy.

A fatal case of a Burkitt's lymphoma which occurred in a 34-year-old German woman during pregnancy is described. Nearly all organs showed either diffuse or nodular infiltration by tumor cells. Placenta and fetus were free of detectable tumor tissue. The patient had extremely high antibody titers (1 : 2056), both against Epstein-Barr virus capsid antigen (VCA) and the early antigen complex (EA). Within the tumor cells the Epstein-Barr virus-specific nuclear antigen EBNA and viral DNA was detected. A cell line established from a tumor biopsy displayed a translocation involving chromosomes 2 and 8. The role of Epstein-Barr virus in the development of Burkitt's lymphoma is discussed.

[Typical and non-typical lymphogenic spread of gastric cancer (author's transl)].

Incidence and distribution of lymph node metastases were examined in 100 gastrectomy specimens with 104 primary gastric carcinomas. Special emphasis was placed on the correlation with the location of the tumor and its size as well as the depth of infiltration and the tumor histology. Sixty-one percent of carcinomas with a diameter of up to 6 cm had lymph node metastases while this was the case in 83% of larger tumors. Infiltration of the muscularis propria only was associated with a 23% incidence of lymphogeneous metastases. This figure rose to 75% if the tumor reached the subserosa. Carcinomas of the diffuse (Lauren) or infiltrating (Ming) type show metastases more frequently (83% and 81% respectively) than those of the intestinal or expansive type (58%). In addition, several regions of lymphatic drainage are more frequently involved with the diffuse type. In only 2 patients (2% of all patients or 3% of the patients with lymphogeneous metastases) skipping of lymph nodes could be observed. In all other patients distant lymph nodes showed metastases only if the perigastric nodes next to the tumor were involved also. These findings support a differentiated, stage- and histology-dependent surgery.

[Developments in the diagnosis of colorectal carcinoma. Comparison of various diagnostic groups].

Epidemiological observations on colorectal cancer and the consequences of modern diagnostic methods are reported on patients treated in the Medical and Surgical Hospital of the University of Erlangen during the past 9 years. In this period the distribution of the rectal carcinomas according to the Dukes-classification was constant. Using coloscopy consequently as routine method and by proving occult intestinal bleeding as screening method, we succeeded in diagnosing colorectal carcinomas in an earlier stage of Dukes' classification.

[Has the calculation of the rate of tumor growth clinical significance?].

In 142 circular lung foci of 78 patients we determined the tumor doubling times (13 bronchial carcinomas, 7 benign pulmonary tumors, and 122 pulmonary metastases). They fluctuated between + 12 and + 8,863 days of tumor growth and a diminuation of -1,810 days of half-life. 44% of all pulmonary metastases doubled their volumes within 60 to 250 days. During the observation period the times of tumor growth varied not only in different pulmonary metastases of the same patient, but also in solitary circular lung foci of the same person considerably. There is no connexion between tumor doubling time and histological tumor type as well as the interval between surgery of the primary tumor and occurrence of pulmonary metastases. Though we found in patients of 10 to 30 years of age only quickly growing pulmonary metastases with a tumor doubling time of less than 60 days a corrlation between time of tumor growth and age could not be proved. By the determination of the tumor doubling time neither binding statements on the nature of a circular lung focus can be made nor clinical consequences for diagnostics or therapy be drawn.

[Do biopsies and operations lead to formation of distant metastases? (author's transl)].

In medical and public literature the radiologist Krokowski states that diagnostic and operative procedures of cancer disease often stimulates the development of metastases. He believes that 30--90% of metastatic disease is the result of prior operations. Consequently he requires a prophylactic radiation therapy before any operative therapy is done. The authors question the theses of Krokowski since there is no detail presentation of this methods, which therefore cannot be repeated.
