Records of prescriptions that originated from one group practice and were dispensed at one pharmacy were maintained for one year. The information recorded included the age and sex of the patients, the name of the prescribing doctor, and the drug(s) prescribed and their cost. Analysis of the records showed considerable differences in average prescription costs among doctors. For all the major therapeutic groups, repeat prescriptions were more expensive than new prescriptions, children had cheaper prescription costs than adults, and prescriptions for women were cheaper than those for men. Within an age-sex group or a therapeutic group, however, prescription costs were similar for each doctor. These results indicate that the differences in overall prescribing costs among doctors were not due to different management of the same disorders, but were due to different types of patients being seen.
Phenylalanine methyl ester (PM) is a decomposition product of the sweetening agent aspartame. The potential for absorption of PM was examined following intragastric and intraduodenal administration of 20-mg doses of [14C]PM to rhesus monkeys implanted with hepatic portal vein cannulae. Small amounts of unchanged PM (less than 0.1 micrograms/ml) were detected in portal and peripheral blood samples during the first 1-2 hours after administration, but none was detectable (less than 0.001 micrograms/ml) at later times. Comparison of the areas under the PM and total 14C blood concentration-time curves indicated that only 0.2% of the administered PM reached the portal blood unchanged, and 0.1% or less reached the peripheral blood unchanged. Blood and plasma from monkeys and humans hydrolyzed PM in vitro at very similar rates, but plasma PMase activity was much higher in the dog and rat than in the monkey or human. Hydrolysis of PM by intestinal mucosa homogenates was also faster for the rat and dog than for the monkey. The in vitro results suggest that absorption of intact PM in the human would be no greater than that found in the monkey.
The following plain film findings of retrocrural disease were demonstrated by two patients: a) silhouetting, by a soft-tissue density, of a previously seen inner margin of the crus, b) lateral displacement of the crus, c) lateral deviation of the inferior portion of the paraspinal line, and d) abrupt obliteration of the upper portion of a well defined crus.
The author describes an original surgical technique to cure inguinal hernias by means of a nylon mesh. Drawing his conclusions from 208 cases he has operated on, he explains the indications of his technique and indicates the results he has obtained. This material if perfectly well tolerated and the rate of recurrence is very low, lower than two percent.
We report results of experiments designed to characterize the Type 1 and Type 3 copper sites in Rhus laccase depleted of Type 2 copper (T2D). Use of the Lowry method for determining protein concentration yielded the value 5620 +/- 570 M-1 cm-1 for the extinction of the 615-nm absorption band of this protein. Anaerobic reductive titrations with Ru(NH)3)6(2)+ and Cr(II)aq ions established the presence of three electron-accepting centers, which are reduced in a complex manner. Treatment of T2D laccase with a 70-fold excess of H2O2 induced a new shoulder at 330 nm (delta epsilon = 660 M-1 cm-1), as well as intensity perturbations at 280 and 615 nm. Comparison of difference spectra show that this 330-nm band derives from a Type 3 copper-bound peroxide and not from a reoxidized Type 3 site. Dioxygen reoxidation of ascorbate-reduced T2D laccase produced new difference bands at 330 nm (delta epsilon = 770 M-1 cm-1) and 270 nm (delta epsilon = 13,000 M-1 cm-1), the former assigned to a bound peroxide which is a dioxygen reduction intermediate. In the corresponding epr spectrum of this material new Cu(II) g parallel features (A parallel approximately 130 G) indicative of an isolated copper ion and a triplet signal near 3,400 G were observed, originating from the Type 3 sites of separate T2D laccase molecules. Reoxidation by ferricyanide or by dioxygen as mediated by iron hexacyanide did not produce these changes. Thus the magnetism of the reoxidized Type 3 site in T2D laccase can be perturbed as a consequence of aerobic turnover. The suggestion is advanced that there are presently three forms of T2D laccase, possibly metastable conformational isotypes, accounting for the apparently contradictory reports on the properties of this protein.
New epr features consistent with a novel type of Cu(II) are observed in partially reduced Type 2 copper depleted laccase molecules. Cu(II) hyperfine lines appear near 2590 G and 2770 G, and a rhombic g1 feature is also observed. These reflect a Cu(II) emergent on reductive disruption of the binuclear Type 3 site in T2D laccase. Additionally, much of the new, magnetically isolated Cu(II) is retained on full reoxidation of partly reduced Type 2 copper depleted laccase. The proportion of disrupted Type 3 Cu(II) sites remaining after reoxidation appears to depend on the prior distribution of electrons within T2D laccase.

Normal coordinate analysis that utilizes a general valence force field and the Wilson FG matrix method has been applied to several structural models representing the active site of the blue copper protein, azurin. The models included tetrahedral and square planar CuN2SS', trigonal CuN2S, and trigonal bipyramidal CuN2SS'O structures in which the Ns are imidazole nitrogens of histidines, S is the thiolate sulfur of cysteine, S' is the thioether sulfur of methionine, and O is a peptide carbonyl oxygen. For constant Cu--ligand bond lengths and initial force constants, the force field was refined against the most intense of the observed frequencies (424, 404, 369, and 261 cm-1) in the resonance Raman spectrum of Pseudomonas aeruginosa azurin. The most satisfactory fit between observed and calculated frequencies occurs for tetrahedral and trigonal structures. The calculations provide detailed assignments for the resonance Raman spectrum of azurin and reveal considerable mixing of Cu--S(Cys) and Cu--N(His) vibrational modes. The trigonal model is favored because it is shown that the approximately equal to 260-cm-1 vibration is an invariant feature in the resonance Raman spectra of blue copper proteins, even those lacking a methionine in the vicinity of the copper atom. The present analysis ascribes the high frequencies of the Cu--ligand stretching modes and the resonance enhancement to the coupled nature of their vibrations and the Franck-Condon overlaps with predominant (Cys)S leads to Cu(II) charge transfer bands in the visible region.

Accurate differentiation of traumatic, inflammatory and congenital changes in the upper cervical vertebral column requires specific knowledge of the complex anatomic conditions in this segment of the spine, as well as of its embryological specialities and its regional pathology. This article deals with the analysis of the most important traumas of the upper cervical vertebral column and the relevant differential diagnosis.

Gastro-intestinal mural infiltration can be diagnosed by ultra sound from a typical pattern of echos. This consists of a strong central echo surrounded by a circular structureless margin which indicates thickening of the gut wall. In the present paper, the significance of this appearance is discussed with particular reference to the differential diagnosis. The phenomenon is an important diagnostic criterion where there is suspicion of the presence of malignancy as a result of radiological or endoscopic examination. This is particularly important in cases where the suspicion of malignancy cannot be confirmed by biopsy and histology. It is to be expected that the routine use of ultra sound, and paying attention to the above phenomenon, will lead to further diagnostic measures in difficult diagnostic cases, such as intensive radiological examination or endoscopy with biopsy. This will lead to more rapid and more economical diagnoses.

The primary coordination environment of the "blue" copper ion in oxidized azurin has been elucidated by x-ray absorption spectroscopy. The most striking feature is the unambiguous presence of a very short copper-sulfur distance at 2.10 +/- 0.02 A. Nitrogen ligands, presumed to be from imidazoles, are found at 1.97 A. There is some evidence that the copper coordination sphere may be completed by a second sulfur, the distance of which is determined with much less certainty.
