[Right heart ventriculography using Gelifundol].

Forty patients (14 women, 26 men, mean age 58 years) with suspected anomaly of the right heart and suspected tricuspid insufficiency or atrial septal defect were given four injections of Gelifundol in five-minute intervals via the right cubital vein. Before, during, and after each injection an echocardiogram was obtained (four-chamber-view). One subcostal view was recorded. Contrast echocardiograms were analyzed according to a subjective score scheme by two independent observers and by video-intensitometry. After injection of contrast medium no patient showed ECG changes, nor did any patient report side effects after the injection of Gelifundol. Concerning the subjective parameters there were no significant differences between both observers. Score points varied between 5.2 to 5.5, intensitometric maxima lay between 54.6 and 58.0 relative intensity units, and the half-time of the intensity maximum (T1/2) varied between 7.2 s and 8.1 s. There were no statistical differences between the two observers. In all patients a diagnotic improvement could be achieved. A tricuspid insufficiency which was not diagnosed in the native echocardiogram could be diagnosed in 13 patients with Gelfundol. Thus, Gelifundol seems well suited for the assessment of right ventricular lesions in man.

Heterotopic ossification with total hip endoprostheses in various models of thrombosis prophylaxis.

The influence of thrombosis prophylaxis on the occurrence of heterotopic ossification after implantation of total hip endoprostheses was analyzed in a series of randomized studies. When low-dose heparin or dextran 40 was administered, the rate of ossification was comparatively high (30.1% and 65.1%). When a combination of acetylsalicylic acid and an antirheumatic (oxyphenbutazone) or low-dose heparin with an antirheumatic (indomethacin) was administered, the ossification rate became significantly lower (6.2% and 16.7%). No severe ossification (Arcq degree III) was observed in the groups given an antirheumatic in addition to thrombosis prophylaxis.

Demographic study on the family planning behaviour of the German population: the importance of natural methods.

A total of 1,267 women in the F.R.G., aged from 15 to 45 years, were interviewed about their family planning behavior and the importance of natural methods of family planning (NFP) in a representative demographic study. It was found that 19.6% of the respondents were not sexually active. The others used various methods, as follows: pill, 38.4%; IUD, 10.3%; condom, 5.9%; diaphragm, 2.1%; spermicides, 0.8%; withdrawal, 3.4%; NFP methods, 3.9%. The main reason for rejecting a method was a possible medical risk. Of the responding women, 73% had some previous knowledge about NFP; 47% of them found such methods to be interesting, and approximately 20% indicated a high probability of the use of NFP methods in the future.

A large reservoir of sulfate and sulfonate resides within plasma cells from Ascidia ceratodes, revealed by X-ray absorption near-edge structure spectroscopy.

The study of sulfur within the plasma cells of Ascidia ceratodes [Carlson, R. M. K. (1975) Proc. Natl. Acad. Sci. U.S.A. 72, 2217-2221; Frank, P., Carlson, R. M. K., & Hodgson, K. O. (1986) Inorg. Chem. 25, 470-478; Hedman, B., Frank, P., Penner-Hahn, J. E., Roe, A. L., Hodgson, K. O., Carlson, R. M. K., Brown, G., Cerino, J., Hettel, R., Troxel, T., Winick, H., & Yang, J. (1986) Nucl. Instrum. Methods Phys. Res., Sect. A 246, 797-800] has been extended with X-ray absorption near-edge structure (XANES) spectroscopy. An intense absorption feature at 2482.4 eV and a second feature at 2473.7 eV indicate a large endogenous sulfate concentration, as well as smaller though significant amounts of thiol or thioether sulfur, respectively. A strong shoulder was observed at 2481.7 eV on the low-energy side of the sulfate absorption edge, deriving from a novel type of sulfur having a slightly lower oxidation state than sulfate sulfur. The line width of the primary transition on the sulfur edge of a vanadium (III) sulfate solution was found to be broadened relative to that of sodium sulfate, possibly deriving from the formation of the VSO4+ complex ion [Britton, H. T. S., & Welford, G. (1940) J. Chem. Soc., 761-764; Duffy, J. A., & Macdonald, W. J. D. (1970) J. Chem. Soc., 977-980; Kimura, T., Morinaga, M., & Nakano, J. (1972) Nippon Kagaku Zaishi, 664-667]. Similar broadening appears to characterize the oxidized sulfur types in vanadocytes. A very good linear correlation between oxidation state and peak position (in electronvolts) was found for a series of related sulfur compounds. This correlation was used to determine a 5+ oxidation state for the additional sulfur type at 2481.7 eV. (ABSTRACT TRUNCATED AT 250 WORDS)

Survival analysis of an uncemented ceramic acetabular component in total hip replacement.

Follow-up examinations of 67 implants of cement-free ceramic sockets show the need for an exact definition of failure, to warrant comparable evaluations of results. Statistical survival analysis offers the possibility of presenting both the incidence of failures and the dates of their occurrence. If only revision surgery with removal of the socket is considered to be a failure, our material shows the "survival quota" of the ceramic socket after 8 years to be 96.7% +/- 2.2%. If radiological signs of loosening are included in the evaluation of failures, the "survival quota" of the stable implants is reduced to 81.9% +/- 6.9%.

Effects of anthrapyrazole antineoplastic agents on lipid peroxidation.

The effects of three anthrapyrazoles and an aminoacridine derivative on doxorubicin- and iron-stimulated lipid peroxidation in rabbit hepatic microsomes have been characterized. Two anthrapyrazoles, CI-937 and CI-942, were potent inhibitors of lipid peroxidation with 15 microM drug inhibiting the rate of peroxidation 70 to 90%. In contrast CI-941 was relatively ineffective in inhibiting lipid peroxidation with only 35% inhibition occurring at 100 microM drug. CI-921, an aminoacridine derivative, diminished lipid peroxidation by 65% at 15 microM. All four drugs failed to decrease the rate of doxorubicin-stimulated NADPH oxidation at concentrations less than 50 microM, suggesting that inhibition of lipid peroxidation was not the result of diminished enzyme activity. CI-937 formed a 2:1 complex with ferric ion, KD = 47 microM, which was reversible with EDTA.

[Results of a representative survey of family planning behavior in West Germany 1985].

A report on the result of a representative survey of family planning behavior in the Federal Republic of Germany in 1985. Interviewers from the EMNID Institute questioned 1267 German women aged between 15 and 45. Of the women interviewed, 6.1% had been sterilized, while in 1.3% of cases, their partner had been sterilized. Regarding the use of reversible methods of contraception, the following figures were established: pill, 37.1%; IUD, 10.3%; condom, 5.9%; rhythm method, 3.9%; withdrawal 3.4%; vaginal diaphragm, 2.1%; mini-pill, 1.3%; locally effective chemical substances, 0.8%. A few women (1.9%) stated that they used a mixture of methods. No information about family planning methods was offered by 13.4% of the women interviewed. The figures obtained were further differentiated according to age, marital status, school education, religion, number of children, desire for more children, profession, and net income. The authors' own questions concerned the early use of contraceptives, the reasons for abandoning a particular method, the reasons for choosing a certain type of contraceptive, sources of information about family planning, the frequency of changes from one type of contraceptive to another, and opinions about the rhythm method. Finally, the result of the EMNID survey is compared with that of known studies both at home and abroad.

[Ophthalmologic implants--a risk at high altitudes?].

In the course of an experimental study typical ophthalmological implant materials were transported to altitudes of up to 48,000 feet. Spherical prostheses manifested practically no changes in volume, while scleral sponge material for detachment surgery released death bubbles during decompression. It is pointed out that less severe effects are likely to occur in this respect under clinical conditions, and likewise that the measured values are only of consequence for the patient in cases of rapid decompression.

A selenomethionine-containing azurin from an auxotroph of Pseudomonas aeruginosa.

The production and spectroscopic properties of an L-selenomethionine-containing homolog of Pseudomonas aeruginosa azurin are described. The amino acid substitution was carried out by developing an L-methionine-dependent bacterial strain from a fully functional ATCC culture. Uptake studies monitored using L-[75Se]methionine indicated that L-selenomethionine was incorporated into the protein synthetic pathway of Pseudomonas bacteria in a manner analogous to L-methionine. Several batches of bacteria were grown, and one sample of isolated and purified selenoazurin (azurin in which methionine was substituted by selenomethionine) was found (by neutron activation analysis) to contain 5.2 +/- 0.8 seleniums/copper. Correspondingly, a residual 0.35 methionines, relative to 6.0 in the native protein, were found by amino acid analysis in this azurin sample. The redox potential and extinction coefficient of this selenoazurin were found to be 333 +/- 1 mV (pH 7.0, I = 0.22) and 5855 +/- 160 M-1 cm-1 at 626 +/- 1 nm, respectively. Visible electronic, CD, and EPR spectra are reported and Gaussian curve fitting to the former spectrum allowed assignment of the selenomethionine Se----Cu(II) transition to a band found at 18034 cm-1, based upon an observed 450 cm-1 shift to the red from the analogous band position in the native protein. The data are consistent with a relatively more covalent copper site stabilizing the reduced, Cu(I), form in the selenoprotein. A role for the methionine as a modulator of the blue copper site redox potential by metal----ligand back bonding from Cu(I) is discussed in terms of a ligand sphere which limits the valence change at copper to much less than 1 during a redox cycle.

Sequelae of induced abortion.

In the long-term prospective controlled study reported here, 1509 general practitioners and 795 gynaecologists in England, Scotland and Wales are cooperating in providing information on the sequelae of abortion, especially on the problems of later pregnancies, subfertility and all reported morbidity, in particular psychiatric illness. Morbidity within 21 days after induced abortion, and considered to be related to induced abortion, was found in 10% of 6105 women who had an induced abortion in their index pregnancy, and there were major complications in 2.1%. The main factors affecting morbidity were the place of operation, gestation at termination, the method of termination, sterilization at the time of operation, and smoking habits. Several differences between National Health Service and private sector operations were found which could affect the morbidity rates. Possible means of reducing early morbidity are discussed. The outcome of the first post-index pregnancy in 745 women whose index pregnancy had ended in induced abortion and in 1339 controls was also compared. There was no statistically significant difference between cases and controls. Further analysis of a large number of pregnancies is required to permit confident interpretation of these observations.

Radiologic manifestations of the syndrome of neurocrest and colonic tumors.

This report describes the radiologic findings in the recently described syndrome of multiple colonic polyps and malignant tumors of neurocrest origin. One patient had multiple endocrine neoplasia syndrome type 11B with malignant medullary thyroid carcinoma, another had malignant pheochromocytoma, and the third had malignant carcinoid tumor. All patients had multiple colonic tubular or tubulovillous adenomas. In 2 patients, there were 8 sessile or pedunculated polyps distributed throughout the colon. In the patient with multiple endocrine neoplasia syndrome type 11B, the colon was carpeted with small polyps, 2-6 mm in size.

Alterations in left ventricular function, coronary hemodynamics and myocardial catecholamine balance with MDL 17043, a new inotropic vasodilator agent, in patients with severe heart failure.

To evaluate changes in myocardial energetics and systemic and cardiac sympathetic activity associated with improved left ventricular function after MDL 17043, a new inotropic vasodilator agent, systemic and coronary hemodynamics and myocardial catecholamine balance were determined in 17 patients with severe heart failure. After the administration of MDL 17043, cardiac index increased by 67% and pulmonary capillary wedge pressure decreased (25 +/- 5 to 14 +/- 7 mm Hg, p less than 0.01), indicating improved left ventricular function. Coronary sinus blood flow (75 +/- 29 to 111 +/- 51 ml/min, p less than 0.01) and myocardial oxygen consumption (9.9 +/- 3.3 to 11.8 +/- 5.4 ml/min, p less than 0.05) increased despite decreased myocardial oxygen extraction (11.7 +/- 2 to 10.1 +/- 3.3 vol%, p less than 0.05) and a higher coronary sinus oxygen content. Although transmyocardial lactate extraction remained unchanged, increased myocardial oxygen consumption has potential deleterious effects on myocardial metabolic function. Arterial norepinephrine concentrations and transmyocardial norepinephrine release also remained unchanged. These findings suggest that MDL 17043 improves left ventricular pump function, but produces no detectable change in systemic and cardiac sympathetic activity. Improved left ventricular function is associated with increased myocardial oxygen consumption despite primary coronary vasodilation.

Barrett oesophagus: analysis of 19 cases.

This article reviews our experience with 19 cases of Barrett oesophagus. Radiological findings included hiatus hernia, gastro-oesophageal reflux, strictures and oesophagitis. The strictures were most commonly distal. The recently described lace-like reticular pattern of the mucosa was noted in one case of the 15 who had air-contrast views. It was associated with an adjacent stricture. The low incidence of this radiological finding emphasises that Barrett oesophagus is a histological diagnosis. Six patients had only the non-specific and common radiological findings of reflux with or without hiatus hernia and one patient had a normal examination. These seven cases demonstrate the lack of suggestive radiological findings in some patients.
