[Socioeconomic benefits of coronary dilatation].

The socioeconomic benefits of coronary artery dilatation to the individual and the society in general, were assessed in a review of our first 158 cases in which we studied the length of in-patient treatment, the period off work and the number of patients returning to work after six months and one year. The population included 114 men and 44 women with an average age of 50 +/- 10 years, a history of anginal pain of 18.10 +/- 26.13 months, and of resistant angina of 4.33 +/- 5.72 months. Only one artery was diseased in 130 patients (82%), the remaining patients having multivessel disease. Left ventricular function was normal in 76% of the patients under study. Fifteen patients had already finished working at the time of dilatation and were excluded from the study. One hundred and nineteen (84%) of the 142 patients followed-up were still working before dilatation and 112 (79%) returned to work after 6 months. Of the 94 patients with a good initial result of dilatation, 76 returned to work (81%); 12 initial failures were treated medically, and 6 (50%) returned to work (p less than 0.05); 36 initial failures underwent aorto-coronary bypass surgery and 30.83% are back at work. At one year, 89% of the 103 patients followed-up were working, whilst only 83% of this subgroup were working before dilatation. The 91% of the 69 initial successes have returned to work; 71% of the 7 failures treated medically and 95% of the 27 failures who then underwent bypass surgery, have also returned to work.(ABSTRACT TRUNCATED AT 250 WORDS)

[Demonstration of plasmacytes in gingival immune defense units in dermoid cysts of the ovary].

Gingival units of immunological defense have been observed surrounding erupted teeth in the connective tissue of the wall of ovarian dermoid cysts. Using a modified immunoperoxidase technique, the production of IgA, IgG and J chains has been clearly detected. This technique works perfectly on decalcified specimens. Since dermoid cysts are not contaminated, the origin of the plasma-cells in these units is presumed to be triggered by other antigenic factors. These are present in epithelium and provided a possible explanation for the "homing" of lymph-cells migrating from the immune system.

[Experimental study of a biological micro-arterial prosthesis].

An arterial prosthesis prepared from human placental arteries, diameter 1.0 mm, length 20 mm, was evaluated after implantation into the infrarenal region of the aorta of the rat. The permeability rate after one year (70%) compares favorably with results after fresh arterial autografts. Histology showed that the prosthesis acted as a matrix colonizable by the tissues of the recipient. The internal surface of the tube was covered by a tissular layer in which fibrillary elements suggestive of elastic fibers were developing.

[Epidemiologic survey of intestinal parasitoses in Peruvian Amazonia].

An epidemiological survey was carried out in 4 native villages from tribe Cashibo, in Amazonia (Peru). Among 165 stools examination, 92% were positives for helminthic (hookworms: 72%, roundworms and whipworms: 55% each, strongyloides: 5%) and protozoan infections (Entamoeba coli and Endolimax nana: 50% each, Entamoeba histolytica 24%, Giardia intestinalis 17%). Balantidium coli is not seldom (6%). Polyparasitism, very frequent, is reported with a maximum of 10 species. The most important villages are the most infested. Several factors can explain these results: lack of hygien and treatment, and no sanitary education. These rates are among the highest recorded for the South-American Indian people.

[Case of a rare maxillary tumor: osteocementofibroma].

A 17-year-old young girl presented with a rare maxillary tumor, an osteocementoblastoma, which from the ectopic type of position of the wisdom tooth had apparently been present for many years pathologic findings are reported. Post-operative course was normal but regular follow-up is necessary because of the tendency for recurrence of the few cases reported in the literature of this benign tumor.

Morphogenesis of nuclear polyhedrosis virus from Autographa californica in a cell line from Mamestra brassicae (cabbage moth). Further aspects on baculovirus assembly.

Cultures of cell line IZD-MB 0503 from Mamestra brassicae were inoculated with nonoccluded nuclear polyhedrosis virus of Autographa californica (AcNOV). At 1 h postinoculation (p.i.) nucleocapsids were found in the cytoplasm near nuclear pores and within the nucleoplasm. Formation of virogenic stroma was observed at 7 h p.i. The first short empty capsids were seen at 10 h postinoculation (p.i.), followed by partially and completely filled nucleocapsids (11-12 h p.i.) with most capsids filled at 12 h and later. This suggests that nucleocapsid components, such as capsid and DNA-core, assemble in successive stages rather than simultaneously. Viral progeny, being released from the cells by budding, were observed at 13 h p.i.

Coronary artery surgery as a measure of vocational rehabilitation: an analysis of the working status of patients with mild angina randomly assigned to medical or surgical treatment.

Work status was studied in 76 male patients under the age of 60, who had been randomly assigned to surgical or medical treatment for coronary artery disease with relatively stable angina (Class I or II NYHA) One year after catheterization or surgery, there were as many patients employed in the medical (84%) as in the surgical (81%) groups, despite a marked improvement of functional symptoms in the latter group; 8% in surgical patients and 11% in medical patients were on social welfare. Surgery did not improve work resumption in patients who had been unemployed for seven months or more before inclusion into the study (71%, versus 67% in medical patients). It is concluded that aorto-coronary bypass surgery cannot be considered as a measure of vocational rehabilitation sufficient to improve employment in patients with mild stable angina, when compared to medical treatment.

Myasthenia gravis in the elderly: report of 37 cases.

The treatment of myasthenia gravis in the elderly is controversial. Thirty-seven myasthenic patients with onset of the disease after the age of 60 were followed for a period of 14 years. All of the 37 patients received anticholinesterase drugs during this period, ten underwent thymectomy, and 24 were treated with corticosteroids. At present, one patient is in remission, 28 are improved, one is unchanged, and seven have died. Only one death was directly related to myasthenia. In the authors' experience thymectomy can be an effective treatment of myasthenia gravis in elderly patients; corticosteroid therapy can also be useful in addition to or as an alternative to surgery. Using a "personalized" schedule the authors obtained good results in 78 per cent of their patients.

[2-dimensional contrast echocardiographic diagnosis of associated atrioventricular canal and cor triatriatum].

2D echocardiography provided the diagnosis of partial atrioventricular canal associated with cor triatriatum in a 4 year old child. The apical four chamber view showed an ostium primum and the presence of an abnormal transverse echo in the left atrium, dividing it into ventral and dorsal chambers. Contrast echocardiography in the four chamber apical view showed an atrial right to left shunt and only opacification of the ventral chamber of the left atrium at the mitral orifice. On the other hand, the dorsal chamber which received the pulmonary veins could not be opacified by contrast. The contrast injection allowed the diagnosis to be confirmed by demonstrating the division of the left atrium into two chambers.

[Pacemakers in infants and young children. Comparative study of pleural and lumbar modes of implantation].

The authors report their experience of 62 implantations of pacemakers in babies and small children by two different methods. The pleural cavity was chosen as the site of implantation in 22 patients aged 8 months to 10 years, weighing 4,2 to 26 kg, with a maximum follow-up period of 7 years. Reoperation was required in 11 cases: 6 replacements of mercury batteries, 5 electrode dysfunctions, 1 electrical leak syndrome and 1 septic complication. In the other 40 patients aged 24 hours to 17 years, weighing 2,2 to 37 kg, with a maximum follow-up period of 5 years, the pacemaker was implanted in the lumbar region. Reoperation was required in 4 cases; 1 pacemaker replacement for infection and 3 electrode faults with pacemaker replacement. It is 5 years since the authors abandoned the pleural cavity in favour of the lumbar area as the site of implantation in babies and small children. Programmable pacemakers with lithium power sources are preferred for their many well known advantages. The use of the pulse generators and the progressive improvement in the electromechanical properties of the pacing electrodes which have become more and more reliable, have made reoperation a rare occurrence. In addition, replacement of implanted material only requires a small lobotomy in the great majority of cases. Recurrent thoracotomies, difficult for the surgeon and dangerous to the patient, are thereby avoided.

[Rehabilitation and return to work of patients after aortocoronary bypass].

A pilot project with medical, physical and psychosocial evaluation and a physical and psychosocial rehabilitation program included 59 men, under age 60, inactive for less than one year and who underwent aortocoronary bypass surgery between november 1978 and march 1980. A control group of 60 comparable patients was studied by questionnaire one year after the operation. The percentage of return-to-work was not significatively different: respectively 92 p. 100 and 89 p. 100. A previous study on a similar population determined 9 predictive sociodemographic and medical factors: age, angina class, duration of symptoms, associated vascular disease, non cardiovascular illness, education, physical workload, length of preoperative unemployment, annual income. The evaluations of this study showed the importance of the psychosocial factors and alcoholism. In comparison with our previous studies, the increased percentage of return-to-work (from 69 p. 100 to 89 p. 100) is mostly due to a shorter preoperative period of inactivity; the percentage of patients operated on within three months of inactivity increased from 44 p. 100 to 74 p. 100 in the last ten years. In the group of patients with a good or excellent preoperative prognosis, 94 p. 100 were working after one year. We conclude that a strategy for improving return-to-work after surgery is to decrease the period of preoperative inactivity. By using nine objective predictive factors and a psychosocial evaluation, it is possible to screen patients with a poor return-to-work prognosis and to submit them to an individualized rehabilitation program.

[Pulmonary arteriovenous aneurysm].

A case of large arteriovenous aneurysm in the right upper lobe of a 27-year old man is reported. The pulmonary angioma was responsible for a 30% shunt with hypoxia and polycythaemia but was only discovered while looking for the origin of a cerebral abscess. A few weeks after surgical treatment of the cerebral abscess, the right upper lobe was excised with immediate correction of the hypoxia. This case shows that pulmonary angiomas can sometimes be revealed by neurological complications, and that visceral angiomas should not remain undiagnosed. They should be considered as part of a Rendu-Osler disease, the familial character of which may contribute to an early diagnosis.
