Tolerance to corticoids? Guinea pig epithelium as an experimental system.

The antiacanthogenic effect of topical applied steroids is well known and maybe exactly assessed and quantified by means of the test procedure described. A 0.1% clobetasol-17-propionate solution induces an antiacanthogenic effect of 27.5% after a 28-day treatment period. After 84 days, the skin further treated with the corticosteroid shows an antiacanthogenic effect of 23.7% and after 118 days of 39%. A steroid tolerance of epithelial layer on guinea pig skin cannot be derived from our results.

[Pseudocondylomata of the vulva (author's transl)].

The present casuistry study deals with condylomata acuminata-like lesions of the vulva of two sexually mature 20 and 22 years old Turkish sisters, and a 34 year old Yugoslavian women. Based on clinical and histological particularities the verruciform structures are interpreted as - atavistic? - malformation without functional significance. Their relation to the papillas of corona glans are proved in detail.

[Cutaneous mold fungus granuloma from Ulocladium chartarum].

Cutaneous granulomas due to the mold fungus Ulocladium chartarum (Preuss) are described in a 58 year old woman. This fungus is usually harmless for mammalian. It is thought that a consisting immunosuppression (Brill-Symmer's disease, therapy with corticosteroids) was a priming condition for the infection. The route of infection in this patient described is unknown.

[Multiple systemic neuromas of the skin and mucous membranes].

A 58-year-old man is described with fibrillar neuroma of the lips, tongue, mucous membranes of the mouth, and larynx. Also tumors of the capillitium, the neck the trunk, and the arms were found. There were no clinical or blood chemical signs of pheochromocytoma, medullary carcinoma of the thyroid or medullated corneal nerve fibers. It is considered that there is no association with the multiple endocrine adenomatosis type IIb, or with the neurofibromatosis von Recklinghausen.

[Recurring eruptive penile angiomas].

A patient is described with atypical vascular lesions occurring successively on the glans penis which proved to be capillary angiomas histologically. Some of them could not be differentiated either clinically or microscopically from pyogenic granulomas. Although the lesions had many cytologic characteristics of malignant tissue, they were considered to be examples of pseudomalignancies because of their benign biologic behavior.

[Lymphogenic metastasing of malignant melanoma (author's transl)].

Data of 378 melanoma patients treated surgically between 1969 and 1979 were evaluated in a retrospective analytic study. Histological criteria such as level of invasion (according to Clark) and tumour thickness (according to Breslow) were related to tendency for lymphogenic metastases and to life expectancy of patients. The level of invasion proved a better prognostic criterium than tumour thickness. In 202 lymphadenectomised patients it could be shown that the probablity of lymphogenic metastases is directly proportional to the level of invasion of the primary tumour. Histologically demonstrable lymphnode metastases were found in 47% of males and 38% of females with melanomas of invasion level III. Figures for melanomas with invasion level IV were 64% of males and 41% of females, and 100% of males but only 16% of females in melanomas of invasion level V. With negative lymphnode involvement the 5 year survival rate is significantly higher in females with invasion level IV and V or for melanomas with a vertical diameter of more than 2.25 mm than in males. Histologically established lymphnode metastases lead to a definitely worse prognosis for both sexes. A higher survival rate for females can then no longer be demonstrated.

[Histology of atypical localized cutaneous erythematosus].

Clinical and histological dates are presented from a study of a group of 32 patients with chronic cutaneous lupus erythematosus. All patients had wide-spread DLE as defined by the presence of lesions above and below the neck, on the upper and lower extremities. Cutaneous punch biopsies showed a great variability of the histological picture. The resulting aspects of histological differential diagnosis are discussed.

[On the encapsulated intraneural granular cell tumor (author's transl)].

Two cutaneously and subcutaneously localized nodules of the left mamma from a 66-year-old patient were excised. The lesions were flesh-colored, firm and well circumscribed. Sections of the tumors show the typical aspects of granular cell tumor. The lesions were both surrounded by a condensation of fibrous tissue which formed a capsule. This condensaton of fibrous tissue resembled and on some sections was in continuity with the perineurium of a nerve. In addition to that is the finding of granular cells situated within a nerve fascicle and apparently replacing several, or all, of the nerve fibres.

[Histology of the pigmented granular rankenneuroma (author's transl)].

We report on a case of a 40 years old female patient with a lentil-sized tumor of the left pinna present since early childhood. Histologic examination revealed pigmented granular "rankenneuroma". The lesion is unencapsulated but fairly well marginated in the dermis. It is composed of varying numbers of spindle-shaped or polyhedral cells arranged in intertwining and parallel bands separated by strands of collagen. The fascicles follow the preexisting nerve truncs. All layers of the dermis may be involved, but only rarely is the superficial subcutaneous tissue invaded. The fascicles of the tumor are generally constructed in a similar fashion. In the center one see argyrophile granular alterated unpigmented cells- Those elements are surrounded by mostly bipolar cells containing varying amounts of melanin. Silverstaining shows transition from spindle-shaped cells to granular degenerated elements. It is thought that the tumor most likely originates from the proliferation of the granular degenerated Schwann cells. The characteristic feature of tumor parenchyme is the association of the granular Schwann cells with cutaneous melanoblasts.

[Malignant cutaneous lymphoma. Histology and clinical aspects with special reference to plastic-embedded tissue specimens].

The basis of this paper is the terminology of Non-Hodgkin's lymphomas based on Gerard-Marchant, Hamlin, Lennert and others, and known as the "Kiel Classification". We have come to the opinion through histological studies of 70 malignant skin lymphomas, that this concept can also be used for malignant lymphoreticular proliferations of the skin, especially since advances in cell differentiation have shown lymphatic origin of different skin tumors.

Animal experiments investigations on the action of topical corticoids on the normal guinea pig skin.

By means of the test procedure described, the action of topical corticosteroids on the untreated skin of guinea pigs may be exactly assessed and quantified. Of significance is the statistically verifiable observation that the steroid-induced epidermal hypoplasia is completely reversible after the withdrawal of the corticosteroid preparation. Moreover, the onset of a reactive epidermal spreading occurs, which exceeds the initial value to a small extent.

[Histology and clinical aspects of cutaneous neurinomas].

27 year old male patient suffered for four years from multiple cutaneous and subcutaneous neurilemmomas of the right lower leg. The patient showed no symptoms of neurofibromatosis von Recklinghausen. The small tumors attained a size not greater than 1.5--2 cm in diameter. On pressure and sometimes spontaneously they proved to be painful. Histologically, the tumors are composed of two parts, the more solid portion (Antoni A tissue) and the loose, cystic portion (Antoni B tissue).

[On neurogenic granular cells and their relationships to the so-called granular cell myoblastoma (author's transl)].

A report on granular cells found in biopsy material is presented. Morphologically identical patterns were seen in a pigmentated granular 'rankenneuroma' and in a patient with a morphea of the tongue. The independent development of granular cells in tumorous and nontumorous tissue supports the idea that the so-called granular cell myoblastoma originates from degenerative alterations of Schwann cells.

[Histology of a plexiform neuroma with Vater-Pacini-lamellar-corpuscle-like structures].

From the finger of a 11 year old boy a tumor that had been present since birth was removed. Histological examination showed a great number of immature nerve bundles, and structures resembling Pacinian corpuscles. It could be shown that the organoid structures are a result of a proliferation of the so-called perineurial cells. These formations do not correspond with real tactile corpuscles, rather they can be considered as neoplastic structures of the perineurium.
