[Hyperparathyroidism after radioactive iodine therapy for Graves' disease: a case report].

Herein we report a 36-year-old man with hyperparathyroidism and a past history of internal irradiation to the thyroid. Twelve years previously at age 24 years he had received 8 mCi of radioactive iodine for Graves' disease. An additional dose of 4 mCi was required 3 years later. A right lower parathyroid adenoma (28 X 23 X 20 mm, 5.7 g) was found at neck exploration. Although the association of external ionizing radiation to the head and neck and the subsequent development of hyperfunctioning parathyroid glands has been described in recent years, there are only 4 cases in the literature of parathyroid surgery for hyperparathyroidism secondary to earlier treatment with radioactive iodine for Graves' disease. In a long-term follow-up of 180 patients treated with radioactive iodine for Graves' disease, neither hypercalcemia nor hypophosphatemia was found. Whether internal radiation therapy can be a causative factor in the development of hyperparathyroidism should be elucidated in future. However, it seems reasonable to suggest that patients whose hyperthyroidism has been treated with radioactive iodine should have their serum calcium levels examined at 5-year intervals.

Treatment of infected urinary stones in rats by a new hydroxamic acid, "N-(pivaloyl)glycinohydroxamic acid.

The effectiveness of a new urease inhibitor, N-(pivaloyl)glycinohydroxamic acid, in the treatment of infected urinary stones was investigated. The hydroxamic acid markedly inhibited and alkalinisation of urine and stone formation when it was administered orally to rats with urinary tract infection caused by Proteus mirabilis; its inhibitory effect was potentiated by concomitant treatment with Cephalexin. This compound may become a useful medicine for the treatment of struvite stones.

Transient anuria following administration of angiotensin I-converting enzyme inhibitor (SQ 14225) in a patient with renal artery stenosis of the solitary kidney successfully treated with renal autotransplantation.

We report on a patient with renal artery stenosis who had only 1 kidney and in whom acute renal impairment developed with transient anuria after the administration of captopril. Although inhibition of the angiotensin-converting enzyme is considered to be efficient treatment for hypertension associated with chronic renal failure, care should be taken when captopril is administered initially to a patient with decreased renal function, under the conditions of sodium and water depletion. The renovascular hypertension in this patient was treated successfully by renal autotransplantation.

Therapeutic effect of an aromatic retinoic acid analog on rats with bladder carcinoma upon administration alone or in combination with mitomycin C.

An aromatic retinoic acid analog (Ro 10-9359) was given orally in combination with intraperitoneal administration of mitomycin C (MMC), in an attempt to reduce the toxicity and enhance the therapeutic effect. Male ACI/N rats were inoculated subcutaneously with BC50-TC cells, an established bladder carcinoma cell line of ACI/N rats. The chemotherapy was initiated at 16 days after the inoculation and continued for 4 weeks thereafter. Tumor growth was significantly inhibited in the rats given 100 mg/kg/week Ro 10-9359 or 0.3 mg/kg/twice a week MMC. Neither additive nor synergistic effect was apparent when these two drugs were given simultaneously. No effect was seen when 0.1 mg/kg/twice a week MMC was given alone or in combination with Ro 10-9359. MMC may suppress Ro 10-9359 activity directly or indirectly by affecting the responding cells. Therefore care should be taken when prescribing retinoids with MMC or possibly with other cytotoxic agents.

[Effect of cis-diamminedichloroplatinum (II) (CDDP) on bladder cancer on rats induced by N-butyl-N-(4-hydroxybutyl) nitrosamine (BBN)].

The effect of CDDP was studied on rat bladder tumors induced by N-butyl-N-(4-hydroxybutyl)-nitrosamine (BBN). A total of 140 rats was divided into 7 groups and 0.05% BBN in drinking water was given for 8 weeks. Half doses of LD50 of CDDP, mitomycin C, cyclophosphamide were injected to rats at the 16th or 20th experimental week. All rats were sacrificed at the end of 24 weeks and bladders were removed. Each of the groups receiving anticancer drugs had significant reduction in mean tumor number per rat compared to the control group (p less than 0.05). CDDP was the most effective in reducing tumor size when it was administered at the 16th week (p less than 0.01).

Evaluation of chemotherapy of prostatic cancer in nude mice.

The effect of chemotherapy for treatment of prostatic cancer was studied in two different types of human prostatic tumors grown in nude mice based on growth curves and histological examination. As single agents, cis-platinum and vincristine showed significant growth inhibition in both strains; cyclophosphamide, methotrexate, Adriamycin, and peplomycin were effective in only one strain, whereas 5-fluorouracil (5-FU), ACNU, and Estracyt were not effective in either strain, even at relatively high doses. A study was also carried out on multimodal combination chemotherapy with fewer side effects, and such therapy is in clinical use. The treatment is composed of VPM (vincristine, peplomycin, and methotrexate) followed by CCF (cis-platinum, cytosine arabinoside, and 5-FU) and ACF (Adriamycin, cytosine arabinoside, and 5-FU). Antitumor effects related to synchronization of tumor cells were investigated by sequential autoradiography and DNA histography.

Therapy for urolithiasis with hydroxamic acids. IV. Prevention of infected urinary stone formation with N-(pivaloyl)glycinohydroxamic acid.

With the aim of finding a prospective therapeutic compound with a promising potential for the treatment of urolithiasis, we evaluated the effectiveness of a new potent inhibitor of urease, N-(pivaloyl)glycinohydroxamic acid. The present study revealed that N-(pivaloyl)glycinohydroxamic acid effectively inhibited the alkalinization of urine and the stone formation in vitro and in vivo, due to its strong inhibitory potency against the ureolytic activity of intact Proteus mirabilis. The possibility of the clinical application of this compound in the prevention of struvite stone formation caused by infection of urea-splitting bacteria awaits evaluation of the safety of this compound.

Hypertension with elevated renal vein renin secondary to unilateral hydronephrosis.

A case of hypertension secondary to unilateral hydronephrosis is reported. Renin levels were elevated in peripheral veins and in the renal vein draining the affected kidney. An infusion of angiotensin II analogue resulted in a decrease in blood pressure. After nephrectomy, blood pressure and peripheral plasma renin activity returned to normal. Renal vein renin and angiotensin II analogue studies were useful in the diagnosis and evaluation of curability by surgery in this case.

A simple method of predicting progression of polycystic kidney disease by 99mTc-dimercaptosuccinic acid renal scintigraphy.

Renal scintigraphy with 99Tc-dimercaptosuccinic acid (DMSA) was used to determine the prognosis of patients with adult polycystic kidney disease. In 40 patients, DMSA renal uptake rate, serum creatinine and blood urea nitrogen (BUN) were examined in different periods, at least twice a year. These parameters were plotted on semilogarithmic paper against the course of observation period (years or months in duration). As the impairment of renal function progresses, these parameters gradually worsen, DMSA renal uptake decreases and serum creatinine and BUN elevate linearly. In the terminal stage when hemodialysis is required, total DMSA uptake decreased to less than 1%, and this paralleled the levels of 100 mg/dl of BUN and 10 mg/dl of serum creatinine. Accordingly, when linear lines of parameters are extended to those levels obtained in hemodialysis patients, the years and months which coincide on the abscissa can be accounted for when initiation of the hemodialysis is taken into consideration. Analysis of this linear relation in individual patients allows for an estimate of the progression of this disease and may assist in the determination of the effects of conservative treatment. This method also indicates that the level of DMSA uptake at the initial diagnosis is a critical factor in assessing the prognosis of the patient.

[Mortality rates by causes of deaths in the area aggregated by dyeing factories in Kyoto (author's transl)].

In 1971 and 1973, Yoshida, et al. reported a higher relative risk of urinary bladder cancer among the workers of dyeing factories in Kyoto city. In order to confirm this, death certificates in Kyoto city from 1969 to 1972 were retrospectively investigated. Kyoto city was devided into three areas, that is, areas with high, medium and low clusterings of dyeing factories, and the differences of the mortality rates of all causes of deaths among these three areas were examined. As a result of this study, a statistically significant difference of the mortality rate of bladder cancer could not be found for males. But, the relative risk of bladder cancer in the areas with high and medium clusterings of dyeing factories compared to the area with low was found to be 1.45. Therefore, the relationship between dyeing work and bladder cancer could not necessarily be denied. It is, thus, necessary to carry out a prospective study, by which a more precise result can be obtained. In addition, our study revealed a significantly high mortality rate of skin cancer among the areas with high and medium clusterings of dyeing factories for males, observing a relative risk of 3.88. The observed association between skin cancer and dyeing work should be further studied.
