Eighty-one patients with diffuse large cell lymphoma localized in the head and neck were treated with radiation between 1971 and 1981. Of these, 52 had Waldeyer's ring disease, 14 nodal, and 15 extranodal disease. Overall survival and relapse-free survival rates at five years after radiation therapy alone were 69% and 52%, respectively. Five-year disease-free rates of patients with Waldeyer's ring disease were 91% in stage I, and 71% in stage II, which are much better than those of other extranodal and nodal diseases. According to mass size, the five-year relapse-free rate for the small-mass less than 5 cm in diameter group was much better (72%) than that for the large-mass group (29%).

Histologic transformation in the course of non-Hodgkin's lymphoma (NHL) has been reported to occur in 18 to 30% of the cases. Less favorable prognosis in cases with initial low grade malignancy followed by emergence of high grade malignancy has been previously described. In the previous literature, the histologic transformation has been examined mostly in nodal NHL. In the present study, histologic transformation in the course was investigated on 20 cases with early extranodal NHL. All these 20 cases were diffuse lymphomas, and were composed of 7 cases with low grade malignancy and 13 cases with high grade malignancy. Histologic transformation was not observed in any of these cases. These findings indicate that the frequency of histologic transformation is much lower in extranodal NHL than in nodal NHL. The prognostic significance of these findings is also discussed.
Since the Rappaport classification provides little information for the prognosis of extranodal non-Hodgkin's lymphoma (NHL), the Kiel classification was used for 71 patients with head and neck extranodal NHL, stages IE and IIE. The ratios of low grade malignancy ( LGM ) and high grade malignancy ( HGM ) were about 60 and 40%, respectively. The lesions comprised 37 non-thyroid lymphomas and 34 thyroid lymphomas. 8 cases of thyroid NHL did not fit into a rigid classification scheme, and were separatively analyzed. In the whole group of patients and the subset of 37 patients with non-thyroid NHL, the 5-year survival rates for LGM were significantly better than for HGM . LGM showed a more favorable prognosis than HGM in 26 patients with thyroid NHL, but the difference was not statistically significant. 8 cases of thyroid NHL showed diffuse proliferation of centrocytes or centroblasts in which various numbers of nodules consisting of large centroblasts and centrocytes with high mitotic activity were scattered. The cases behaved as HGM . From these findings, we suggest that the Kiel classification in extranodal NHL is helpful.

A clinical study comparing the use of radiation to the combination of bleomycin and radiation was carried out in 61 patients with carcinoma of the lower gum. For the 28 patients treated by radiation, the dose was 30-74 Gy, and, for the 33 treated by combination therapy, the dose of bleomycin was 45-120 mg and that of radiation was 20-52 Gy. The 5-year survival rate of the radiation-alone group was 0/12 (0%), and that of patients undergoing surgery after radiation was 7/16 (44%), whereas that of the combined bleomycin and radiation alone was 7/15 (47%) and that of surgery after combined treatment was 14/18 (78%).
Using Iridium-192 wires through a rectal template after Syed, interstitial brachytherapy was conducted in a patient with inoperable adenocarcinoma of the rectum. 67-year-old man with constipation and change in the stool caliber underwent external radiotherapy (4,000 cGy/4 W) to the whole pelvis including the perineum, followed by interstitial implant using a template, at the Department of Radiology, Osaka University Hospital. Marked tumor regression, marked circumferential fibrosis and a remarkable decline of CEA titers (pre-RT: 35.8 ng/ml, post-RT: 6.2 ng/ml) were observed until 7 months post-RT. The domestic production of Iridium-192 wires has made possible the integrated use of brachytherapy in the perineal region in Japan.

From September, 1974 through December, 1979, a total of 249 patients with carcinoma of the cervix uteri Stage IIb and III were randomly allocated to either remotely controlled high-dose-rate intracavitary radiotherapy or manual afterloading low-dose-rate therapy, with radiotherapy of 20 Gy in 2 weeks to Point A to whole pelvis and 40 Gy in 4 weeks to the parametria. The dose to Point A by intracavitary radiotherapy was 40-60 Gy with one or two fractions in the low-dose-rate group and 30 Gy for the high-dose-rate group by 3 fractions with a once a week schedule. The purpose of this paper is to compare the results between the groups and to clarify the problems in the high-dose-rate group clinically. The local control rate was higher in the high-dose-rate group; however, the complication rate was also higher in this group than in the low-dose-rate group. The dose schedule and the place of rectal dose measurement is discussed. The overall cumulative survival rate was nearly the same in both groups (55% at 5 years), although some difference was noted in each stage. The most common cause of death was distant metastasis outside the pelvis and the second most common was intercurrent disease in Stage IIb and local failure in Stage III.

This paper reviewed the development of brachytherapy for the past and at present. In Japan, remotely controlled high-dose-rate afterloading intracavitary treatment for uterine cervix carcinoma has become quite common and interstitial radiotherapy with 192Ir hairpin technique for intraoral carsinomas has also been in routine practice at an increased number of institutes in Japan. These has played a great role in eliminating the personnel exposure. Our present efforts are now directed to extend indications to some other cancers, such as bile-duct, pancreas, rectum and prostate. Nearly 20 patients with intraluminal insertion of 192Ir-wire and template technique with 192Ir-wire has just started to apply for rectal had prostate cancer. Permanent implant of 125I has been also of much interest.
