[Effect of various anti-cancer drugs on human renal cell carcinoma serially transplanted into nude mice].

Using two xenografts of human renal cell carcinomas serially transplanted in nude mice (AM-RC-1 and AM-RC-6), both of which maintained the basic histologic features of the original tumor and showed a constant growth rate, the effects of various anticancer agents against 2 target tumors were evaluated. MitomycinC (MMC), adriamycin (ADM), cisplatinum diaminodichloride (CDDP), 5-fluorouracil (5FU), 5-fluro-2'-deoxy-beta-uridine (FUDR) and human lymphoblastoid interferon (HLBI) against AM-RC-1, and MMC, ADM, CDDP, vinblastin (VBL) and etoposide (VP-16) against AM-RC-6. Drugs other than HLBI were administered 3 times in total every three to five days by intraperitoneal injection according to Battelle Columbus Laboratories Protocol and HLBI was injected daily for 10 days intraperitoneally. Anti-cancer effects were evaluated based on tumor growth curve and changes of histologic findings. In terms of tumor growth only MMC (in a dose of 3 mg/kg) revealed a statistically significant inhibitory effect against both AM-RC-1 and AM-RC-6 (respectively P less than 0.001 and P less than 0.05). Concerning AM-RC-1, a significant difference (P less than 0.01) was recognized in the ADM group (5 mg/kg) at the time of the second administration, but evaluation could not ultimately be done owing to appearance of acute toxity after the last dose. The most remarkable histologic changes by light microscopy were recognized in the MMC group (in a dose of 3 mg/kg) against AM-RC-1. They were degenerative findings such as intracellular and nuclear vacuolation, karyorrhexis, karyolysis, karyopyknosis and marginal hyperchromatosis, which corresponded to grade IIa of the classification of the National Cancer Center. The other drugs administered to AM-RC-1 exhibited only grade O to grade I changes. On the other hand, in AM-RC-6, histologic changes were mild (less than grade II) for all the drugs. Electron microscopic features were as follows. AM-RC-1: Marked increase of vacuole of organella was observed and lumens were filled with a large quantity of debris in MMC group (3 mg/kg). In the ADM group (5 mg/kg) there was debris in lumens, although almost no changes of organella were seen. CDDP groups (both 5.6 mg/kg and 2.8 mg/kg) showed autophagic vacuole in the cytoplasm and increased collagen fibers in the stroma but little changes of organella. AM-RC-6: Mild intracellular vacuolation was recognized in the MMC group (3 mg/kg). Watery degeneration and microfibrils were found in the cytoplasm in both ADM (5 mg/kg) and CDDP (5.6 mg/kg, 2.8 mg/kg) groups.

Evaluation of "M-point" in hepatic artery to identify left medial segment of liver. Angiographic study.

The hepatic arteries of 122 patients were analysed on angiography to identify the left medial segment of the liver. Left medial arterial branches were classified into three types: type I arising from the left hepatic artery on the umbilical portion of the portal vein; type II arising from proximal portion of left hepatic artery before reaching the umbilical portion of the portal vein; type III arising from right hepatic artery. Incidence of each type is 37.2%, 35.8% and 27.0%, respectively. The artery frequently kinks at the right side of the umbilical portion of the portal vein and a total of incidence is 68% and that of each type is 23.5%, 89.8% and 100%, respectively. We call this characteristic kinking point of the left medial arterial branches, the "M-point".

[Tumor markers in renal cell carcinoma].

Erythrocyte sedimentation rate (ESR), lactase dehydrogenase (LDH), carcinoembryonic antigen (CEA), haptoglobin, fibrinogen, CH50, Blc-globulin (C3), B1E-globulin (C4), T-cell, B-cell and phytohemagglutinin (PHA) were evaluated in 82 patients with renal cell carcinoma. Positive rates of ESR, LDH, CEA, haptoglobin, fibrinogen, CH50, C3, C4, T-cell, B-cell and PHA were 58.9, 27.1, 33.9, 21.9, 55.8, 34.2, 33.3, 34.8, 15.3, 8.0 and 39.1%, respectively. Positive rates of all serological tests in high stage (stage III and stage IV) renal cell carcinoma were higher than those of the low stage (stage I and stage II) group. Positive rates of ESR, fibrinogen, CEA, haptoglobin, C3 and C4 in the high stage group were 86.1, 81.8, 54.1, 57.1, 53.3 and 60.0% respectively. The level of ESR, LSH, CEA, fibrinogen, haptoglobin and CH50 were significantly elevated (p less than 0.001) above the normal value. Each serological test was not significantly elevated pre- or postoperatively, but in all serological tests, postoperative values were lower than the preoperative values. ESR (less than 100 mm/hour), and CEA in the positive group correlated significantly with poor prognosis (p less than 0.005). Haptoglobin and C3 in positive group also showed a significantly poor prognosis (p less than 0.05). In conclusion ESR and fibrinogen were thought to be useful for screening renal cell carcinoma patients ESR, CEA, haptoglobin, fibrinogen and C3 for estimation of staging, and ESR, CEA, haptoglobin and C3 for estimation of prognosis.

[A family of hereditary communicating hydrocephalus with trigonocephaly].

We studied a family of hereditary hydrocephalus with trigonocephaly, affecting eight members (four males and four females) across two generations. A proband in this family was 51 year old man, who complained of nonprogressive gait disturbance after the age of 20 years. He was admitted to our hospital because of non-neurological disease. Accidentally urinary incontinence appeared and then CT scan was performed, which revealed marked dilatation of the lateral ventricles. He was short stature and showed trigonocephalic forehead, but his head circumference was 56 cm. Mild dementia, spastic gait and hyperreflexia were observed on neurological examinations. A lumbar puncture disclosed that cerebrospinal fluid pressure was 115 mm H2O and sugar, protein and cells were normal. His skull X-ray film demonstrated temporal buldging of the calvarium, but its breadth length and height were within normal limits. Sagittal, coronal and lomboid sutures were not fused. However, skull bones were thinned and the top of dorsum sellae was erosive. A marked symmetrical dilatation of lateral, third and fourth ventricles were found in CT scan, to the contrary, bilateral Sylvian fissures and cortical sulci were slightly widened. On R.I. cisternography, ventricular reflux was noted during 3 to 72 hours after the injection. No accumulation of R. I. was observed in the parasagital region. In the arterial phase of the right carotid angiography, slight displacement was noted in Sylvian branches of bilateral middle cerebral arteries.(ABSTRACT TRUNCATED AT 250 WORDS)

[A clinical study on primary renal pelvic tumors].

Sixteen cases of primary renal pelvic tumor treated at our Department between July, 1971 and June, 1984, were reviewed. The sixth decade predominated over other age groups and occupied 47.3% of all cases (average: 63.9 years). The sex ratio was 4.3:1 with male patients predominating over female patients. The incidence of the affected side was equal; 8 cases in the right and 8 cases in the left renal pelvis. The most common initial symptom was macroscopic hematuria in 13 cases (81.2%), followed by flank pain in 2 cases (12.5%). The major findings in IVP were filling defect in 11 cases (68.7%) and non-visualizing kidney in 4 cases (25.0%). Positive urinary cytology was obtained in 6 cases (49.1%) by voided urine specimen and 4 cases (50.0%) by catheterized urine specimen. Histologically, all cases were transitional cell carcinoma; 11 of them were low stage and 5 were high stage at the initial diagnosis. Nine patients(56.1%) were treated by total nephroureterectomy associated with partial cystectomy. The over all survival rate at 1,2,3,4 and 5 years was 86%, 78%, 78%, 68% and 68%, respectively, by the Kaplan-Meier method. The five year survival rate was 80% for the low stage group and 0% for the high stage group. (p less than 0.05, generalized Wilcoxon test). The five year survival rate was 83% for the low grade group and 40% for the high grade group. (p less than 0.05, Generalized Wilcoxon test) Among several factors, stage and grade of the tumor were the most influencing factors for prognosis.

[A clinical study on primary ureteral tumors].

Twenty-two cases of primary ureteral tumors treated at our Department between July 1971 and December 1984, were reviewed. The patients ranged from 28 to 83 years old (average 63.6 years). There were 17 males and 5 females (3.4:1). The affected side was predominantly the left (15) than the right (7). The most common site of the tumor growth was at the lower third of the ureter (14 cases, 63.6%). The most common initial symptom was macroscopic hematuria, which was seen in 17 cases (77.3%). The major finding of IVP was non-visualizing kidney in 11 cases (50.0%) and filling defect in 7 cases (31.8%). Positive urinary cytology was obtained in 9 cases (52.9%) by voided urine and 6 cases (66.6%) by catheterized urine. Total nephroureterectomy associated with partial cystectomy was performed in 14 cases (63.6%). Histologically, 19 cases were transitional cell carcinoma (86.8%). Associated growth of urothelial tumor in the bladder was found in 4 cases preoperatively and in 3 cases during the course of postoperative follow up. All of them were treated by transurethral resection. Overall survival rate at 1,2,3,4 and 5 years was 84, 71, 55, 46 and 35%, respectively, by the Kaplan-Meier method. The five year survival rate was 54% for the low stage group and 0% for the high stage group; 54% for the low grade group and 26% for the high grade group. (p value = not significant, generalized Wilcoxon test).

Ulcerative colitis complicated by idiopathic central serous chorioretinopathy with bullous retinal detachment.

A 44-year-old woman, hospitalized with severe diarrhea due to ulcerative colitis, was found to have a rare ocular complication, idiopathic central serous chorioretinopathy with bullous retinal detachment. The ocular symptom started with the flare-up of the ulcerative colitis. Steroid therapy was not effective on the ocular disorder, while her ulcerative colitis became dormant. Relationship between the two disorders is discussed.

Renal involvement in mixed connective tissue disease. Report of 5 cases.

5 cases with the compatible serological criteria of mixed connective tissue disease described earlier are presented. In 1 of them with a moderate degree of proteinuria, the renal biopsy disclosed membranous nephritis. However, despite the absence of overt clinical renal disease in the other 4 cases, biopsies disclosed membranous nephritis in 1 and mild mesangial proliferative glomerulonephritis in the remaining 3 cases. In the follow-up of these 4 cases, 2 subsequently developed abnormal urinalysis. Electron microscopic examinations demonstrated electron-dense deposits in glomeruli, and 4 of these patients also had microtubular structures in the endothelial cytoplasm. Contrarily to the original concept, our findings suggest that mixed connective tissue disease also induces immune complex disease.

Paradoxical functions of alveolar macrophages from Calmette-Guérin bacillus-immunized rats.

The effect of autologous alveolar macrophages on the lymphoproliferative response to mitogens was investigated using Calmette-Guérin bacillus (BCG)-immunized rats. The proliferation of lymphocytes obtained from normal rats was suppressed when the lymphocytes were cultured with alveolar macrophages at any proportion, but there was an enhancement in the lymphoproliferative response when the lymphocytes from BCG-immunized rats were incubated with alveolar macrophages at the ratio of 100:1. The supernatant of alveolar macrophage cultures from both normal and BCG-immunized rats showed a suppressive effect on lymphocytes while the supernatant of lymphocyte-alveolar macrophage cultures from BCG-immunized rats enhanced the lymphoproliferation. The results indicate that there are soluble factors elaborated by alveolar macrophages which have opposite effects on lymphoproliferation, and that these factors may be involved in the development of the hyperimmune state of the lung in tuberculosis.

Characterization of insulin-like growth factor I receptor on human erythrocytes.

[125I]Insulin-like growth factor I (IGF-I) specifically bound to erythrocytes; the binding was saturable, and time and temperature dependent. Steady state binding was reached at 16 h at 4 C, and specific binding averaged 14.3 +/- 0.7% (+/- SEM) at a concentration of 3.6 X 10(9) cells/ml in seven normal subjects. [125I]IGF-I binding to the cells was displaced by unlabeled IGF-I in a dose-dependent manner. Scatchard analysis indicated a linear plot, and Ka and number of binding sites/cell were 1.43 +/- 0.07 X 10(9) M-1 and 20.7 +/- 2.2, respectively. Compared to IGF-I, the relative potencies of multiplication-stimulating activity and insulin for displacing [125I]IGF-I binding were 20% and 1%, respectively. [125I]IGF-I binding to erythrocytes from patients with acromegaly was lower than binding to cells from pituitary dwarfs. An inverse correlation between plasma IGF-I level and the number of IGF-I-binding sites per cell was found (r = -0.75; P less than 0.005). These results demonstrate that [125I]IGF-I binding to erythrocytes can be used for clinical measurement of the IGF-I receptor.

Plasma growth hormone and somatomedin-C responses to continuous growth hormone-releasing factor infusion in normal adult men.

The pituitary growth hormone (GH) responses during a 20-hour iv infusion of saline or human GH-releasing factor (hGRF-44) at 40 micrograms/h, followed by an iv bolus injection of hGRF at 2 micrograms/kg body weight, were studied in four normal adult men. During saline infusion only one or two pulses of plasma GH were observed. However, during hGRF infusion up to eight or ten pulses of GH were measured with an amplitude not different from that obtained during saline infusion. The mean +/- SEM integrated amount of GH secreted was 107 +/- 38.2 ng/ml.h in response to hGRF infusion, which was greater than the value of 25.4 +/- 3.5 ng/ml.h obtained during saline infusion. Plasma somatomedin-C also increased after hGRF infusion, but not after saline. After saline or hGRF infusion most of the subjects still responded to an iv bolus injection of the peptide (2 micrograms/kg). These results indicate that hGRF infusion augments GH secretion by increasing the number, but not the amplitude of GH pulses and that the infusion does not cause the pituitary somatotrophs to lose their capacity and ability to respond to hGRF subsequently.
