Intramesangial passage of mononuclear phagocytes in murine lupus glomerulonephritis.

By repeated biopsies, time-sequential changes of the injected colloidal carbon distribution were investigated in glomeruli of NZB/W F1 mice. The intraglomerular shift of the carbon, evaluated by counting the particles at several time intervals after carbon injection, demonstrated the movement of the carbon from the glomerular capillaries to the extraglomerular areas. Electron-microscopic examination disclosed that most of the increased cells of the glomeruli were mononuclear phagocytes rich in ingested carbon particles and that extracellular carbon was scarcely present. This strongly suggested that the carbon particles observed at the light-microscopic level reflected those ingested by the phagocytes. The mesangial cells per se scarcely ingested carbon particles. Instead, mononuclear cells, extending their cytoplasmic protrusions toward the dense (immune complex) deposits, were frequently noticed. It is concluded that the mononuclear phagocyte is a principal component of the hypercellular glomeruli, presumably contributing to the scavenging of the mesangium, and also that there is a pathway in the mesangium for these cells to shift from the capillary to the extraglomerular area by way of the vascular pole and lacis area.

[Clinical studies on 179 cases of prostatic cancer].

Between 1971 and 1984, 179 patients with prostatic cancer were admitted to our Hospital. These cases were studied retrospectively. The greatest number of patients were in their seventies. The patients ranged from 55 to 90 years old with an average age of 72.4 years. The most common symptoms were dysuria, urinary retention, pollakisuria and macrohematuria. About 76% of the chief complaints were related to urinary tract obstruction. There were 64 (35.8%), 21 (11.7%), 38 (21.2%) and 56 (31.3%) cases of stage A, B, C and D, respectively. Prostatic cancer was confirmed histopathologically at transurethral resection in 146 patients, transrectal needle biopsy in 25 patients, open prostatectomy in 3 patients, autopsy in 4 patients, and cryosurgery in 1 patient. The 1-, 3-, and 5-year actual survival rates of 162 cases were 89, 70 and 55%, respectively. The 5-year actual survival rate of stage A, B, C, and D was 72, 67, 55 and 34%, respectively. The 5-year actual survival rate for the cases treated with and without anti-androgen therapy was 72% and 73% for stage A, 66% and 75% for stage B, 58% and 44% for stage C and 37% and 12% for stage D, respectively. The cases treated with anti-androgen therapy was divided into the low dose (diethylstilbestrol less than 300 mg/day or hexestrol less than 30 mg/day) and high dose (diethylstilbestrol greater than or equal to 300 mg/day or hexestrol greater than or equal to 30 mg/day) groups.(ABSTRACT TRUNCATED AT 250 WORDS)

The role of carbonic anhydrase in gastric mucosal protection with special reference to H+ back diffusion and concomitant metabolic acidosis induced by acetazolamide.

It is suggested that carbonic anhydrase is implicated not only in gastric acid secretion, but in mucosal protection. It was reported that acetazolamide induced gastric mucosal lesions. But acetazolamide also caused concomitant metabolic acidosis by inhibiting H+ secretion from renal tubules. We investigated whether concomitant metabolic acidosis is implicated in gastric mucosal lesions induced by acetazolamide in vivo. We also evaluated the effect of acetazolamide on gastric H+ back diffusion in vivo. Correction of metabolic acidosis with sodium bicarbonate had no effect on the degree of the gastric mucosal lesions. Acetazolamide caused no change in gastric H+ and Na+ flux. These results suggest that metabolic acidosis induced by acetazolamide is not implicated in gastric mucosal lesions. Carbonic anhydrase has no effect on H+ back diffusion but may be implicated in mucosal protection by the disposition of back diffused H+ into the gastric mucosa.

Cyclic AMP in gastric juice does not reflect histamine H2 receptor activity in Heidenhain pouch dog.

It is strongly believed that cAMP mediates histamine H2 receptor activity, but does not mediate gastrin and acetylcholine stimulation of gastric acid secretion. Therefore, cAMP production could be a marker of H2 receptor activity. Whether endogenous histamine mediates gastrin and/or acetylcholine stimulation, at least partially, remains to be elucidated. If cAMP in the gastric juice reflects H2 receptor activity, we can investigate whether endogenous histamine mediates gastrin and/or acetylcholine stimulation in vivo. In this study, we investigated whether cAMP in the gastric juice reflected histamine H2 receptor activity in the Heidenhain pouch dog in vivo using different kinds of inhibitors of gastric secretion. Our hypothesis was as follows: Upon betazole stimulation, cimetidine, an H2 receptor antagonist, should decrease cAMP output into the gastric juice, but omeprazole, an H+, K+-ATPase blocker, should not, because it blocks at a site more peripheral than the H2 receptor and the production of cAMP. Sixty minutes after betazole administration, 4.0 mumol/kg of cimetidine and 0.18 mumol/kg omeprazole were administered intravenously and they inhibited gastric juice volume to a similar degree, that is, 49.6% and 52.1%, respectively. However, omeprazole caused a greater decrease in cAMP output than cimetidine. Inhibition with 4 mumol/kg/h of cimetidine or 0.2 mumol/kg of omeprazole from the beginning of betazole stimulation also caused similar decreases in gastric juice volume, 66.6% and 60.6%, respectively. Both inhibitors decreased cAMP output into the gastric juice in a similar fashion in the first two 30 minute periods. These results do not agree with our hypothesis.(ABSTRACT TRUNCATED AT 250 WORDS)

Bile duct carcinoma associated with multiple von Meyenburg complexes in the liver.

A 61-year-old white woman was found to have disseminated abdominal carcinoma at autopsy. The primary site proved to be a bile duct carcinoma arising from ductal epithelium of multiple von Meyenburg complexes of the liver. No other neoplastic lesion was found elsewhere after careful, gross, microscopic examination. This case is thought to represent malignant transformation of the hamartomatous growth, previously reported only exceptionally.

Loss of glomerular responses to vasoconstrictor agents in rabbits recovering from ARF.

Glomerular responses to angiotensin II (AII), arginine vasopressin (AVP), and norepinephrine (NE) were estimated in rabbits recovering from uranium-mediated nephropathy or ischemic acute renal failure (ARF) to examine roles of intraglomerular events in resistance to ARF. Uranyl acetate (UA, 0.8 mg/kg) produced ARF in some animals but did not in others. Rabbits recovering from UA-induced ARF were highly resistant to a rechallenge with a larger dose of the agent (2 mg/kg). Their glomeruli did not respond to AII, AVP and NE in vitro. In animals having not experienced ARF following the initial insult, however, resistance to the rechallenge was lower than in animals recovering from ARF, and the glomerular response to contractile stimuli was well sustained. A two hour clamping of the renal artery induced ARF in uninephrectomized rabbits. These animals were not resistant to an additional ischemia in the recovery phase, despite inhibited glomerular contractile responses to AII. These data indicate a nonspecific inhibition of glomerular responses to contractile stimuli in the recovery phase of ARF. It is unlikely, however, that resistance to ARF can be attributed to the loss of the glomerular contractile response.

Radioimmunoassay for insulin-like growth factor I (IGF-I) using biosynthetic IGF-I.

We produced antiserum to insulin-like growth factor I (IGF-I), and developed a specific and sensitive radioimmunoassay (RIA) for IGF-I using the biosynthetic IGF-I. This antiserum to IGF-I was specific for IGF-I; no cross-reactivities with multiplication stimulating activity, porcine insulin or human growth hormone (hGH) were detected. The sensitivity was 10-25 pg/tube with 50% displacement at 125 pg/tube. The intra- and inter-assay coefficients of variation for IGF-I were 5.4 and 9.7%, respectively. The plasma IGF-I levels as determined by RIA in normal adults (N = 46), patients with active acromegaly (N = 31), and pituitary dwarfs (N = 31) were 21.6 +/- 1.0, 157.3 +/- 17.0, and 2.5 +/- 0.3 ng/ml (Mean +/- SEM), respectively, indicating the levels were GH-dependent. The plasma IGF-I levels were significantly increased from 2.2 +/- 0.2 to 26.5 +/- 3.2 ng/ml after hGH administrations for three consecutive days in five pituitary dwarfs. The IGF-I levels were low in patients with hypothyroidism and liver cirrhosis, but were normal in patients with chronic renal failure. These data confirm previous reports and this radioimmunoassay proves useful in evaluating plasma IGF-I levels.

[The study of a two-dimensional polar representation of the cardiac SPECT image--a new method of displaying left ventricular wall motion].

A new method was developed to display left ventricular wall motion using two dimensional polar representation of cardiac SPECT image. After intravenous administration of 740 MBq (20 mCi) 99mTc (in vivo labeling of red blood cell), ECG-gated SPECT image of cardiac pool was recorded (32 directions, 180 degrees). The short axis images of left ventricle were constructed at end-diastole (ED) and end-systole (ES). Then the functional images of (ED-ES)/ED were calculated at each compatible slices and displayed in color according the degree of wall motion. In 8 cases with cardiac diseases this method was applied and clinically useful functional images could be obtained. The area of akinesia or hypokinesia are successfully demonstrated clearly separated from the area of normokinesia. We conclude that this method is useful to show left ventricular wall motion and to evaluate the segment and grade of abnormal wall motion of left ventricle.

[Study on heterotransplantation of human malignant urogenital tumors in nude mice: results of transplantation and the characteristics of the transplanted tumors].

Between July, 1981 and June, 1984, 40 malignant urogenital neoplasms were heterotransplanted into nude mice to clarify the biological characteristics of malignancies and screen the effective anticancer drugs against them. The primary tumor take was confirmed in 7 tumors (17.5%), including 4 of 14 renal cell carcinomas (28.6%) and 3 of 13 bladder cancers (23.1%). No primary take was recognized in renal pelvic, ureteral, prostatic and testicular cancers. Three of the 7 tumors (43%) with primary take (7.5% in total) have been serially transplanted for more than a year. These three established tumors were all renal cell carcinomas and were designated as AM-RC-1, AM-RC-6 or AM-RC-11. The take rate of tumor tissues obtained from primary and metastatic lesions was 18.2% (6/33) and 14.3% (1/7), respectively. The duration from tumor inoculation to take was examined in seven transplantable tumors. The shortest was 12 days in renal cell carcinoma, the longest was 36 days in bladder cancer and the average was 24.4 days. The correlation between the time from tumor extraction to inoculation and transplantability was investigated. The take rate was 29.4% within 3 hours, and 18.2% beyond 3 hours. The obvious effect of anti-asialo GM1 antibody on tumor take, which inhibits the natural killer activity of mice or rats selectively, was not demonstrated owing to the small number of cases. The histopathological features of seven original tumors with primary take were examined. All of the 4 renal cell carcinomas were shown to be histologically high grade and advanced. Two of the 3 bladder cancers were poorly differentiated. Therefore, the differentiation or growth rate of the tumor was suggested to be closely correlated with transplantability. The three strains of renal cell carcinomas transplanted serially for a long-term period, AM-RC-1, AM-RC-6 and AM-RC-11 disclosed a constant growth after the 7th, 9th and 10th inoculation respectively. The renal cell carcinomas serially transplantable to nude mice maintained the basic histologic findings of the original tumor even after long-term serial transplantation.
