Plasma growth hormone (GH) responses to single and repetitive subcutaneous administration of GH releasing factor (hpGRF-44) in normal and GH deficient children.

Synthetic human pancreatic GRF (hpGRF-44) was administered sc to 8 normal children with short stature and 11 patients with GH deficiency. After a dose of 100-200 micrograms hpGRF-44, mean plasma GH levels reached a peak at 15 min of 27.2 +/- 6.4 (+/- SE) ng/ml in normal children. However the responses were variable and peak plasma GH varied from 10.1 to 56.5 ng/ml. Eight of 11 patients with idiopathic GH deficiency did not respond to sc administration of 100-300 micrograms hpGRF-44. However, a plasma GH increase of more than 5 ng/ml occurred in 2 patients and to twice the basal level in 1 patient. Their GH values were 14.3, 5.2 and 3.4 ng/ml, respectively. After repetitive administration of hpGRF-44 (200 micrograms, twice a day) sc for 5 consecutive days, 2 of 8 patients restored their responsiveness, but the remaining patients did not show GH rise in response to both sc and iv bolus administration of hpGRF-44. Repetitive hpGRF-44 administrations for 5 days had no effect on somatomedin-C production.

Plasma growth hormone (GH) and somatomedin C response to continuous growth hormone-releasing factor (GRF) infusion in patients with GH deficiency.

Pituitary growth hormone (GH) responses during a 10-h iv infusion of saline or human GH-releasing factor (hGRF-44) at 500 ng/kg/h, followed by an iv bolus injection of hGRF-44 at 2 micrograms/kg body weight, were studied in 10 patients with GH deficiency. During saline infusion in 4 patients, small plasma GH increase were observed in 2 patients. However, during hGRF infusion in 6 patients, up to 4 or 13 pulses of GH secretion were observed. The mean integrated GH pulse area during hGRF infusion was 22.5 +/- 5.2 (SE) ng/ml X h, which was greater than that obtained during saline infusion. Plasma somatomedin C levels did not increase after hGRF infusion. After saline or hGRF infusion all patients responded to an iv bolus injection of the peptide. These results indicate that hGRF infusion augments GH secretion by increasing the number and amplitude of GH pulses and that the infusion does not cause pituitary somatotrophs to lose their capacity to respond to hGRF subsequently.

[A clinical study on testicular tumor].

The 19 cases of testicular tumors treated at our Hospital between 1974 and 1983, were reviewed retrospectively. The incidence of testicular tumors among the male outpatients in our urologic clinic was 0.31%. The patients ranged in age from 1 to 76 years old (average: 27.8 years old). The affected side was the left side in 11 and the right side in 8 cases. The most frequent symptom was a painless mass of the testis. Histopathological diagnosis according to the classification by Dixon and Moore was type I in 9 cases, type II in 4 cases, type II in 1 case, type IV in 4 cases. Thus germinal tumors accounted for 18 (94.7%) of the cases. One case (5.3%) of malignant lymphoma was non-germinal. As for the age distribution, two peaks were noted in testicular tumors, especially in cases with potential embryonal carcinoma, between 1 and 2 years and between 19 and 32 years. On the other hand, the cases with potential seminoma showed one peak between 25 and 44 years. The treatment consisted of high orchiectomy alone, orchiectomy with radiation, orchiectomy with chemotherapy or orchiectomy with radiation and chemotherapy. Retroperitoneal lymphadenectomy was not done in any case. The over-all 3-year and 5-year actual survival rates were both 92%.

Reticuloendothelial system and glomerular deposition of heat-aggregated human IgG.

The influence of functional impairment of reticuloendothelial system on glomerular deposition of heat aggregated human IgG (HAIgG) was studied. I-125 HAIgG was injected to mice treated either with dextran-containing 0.9% saline or with the saline alone. The HAIgG clearance rate from the blood decreased in the dextran-treated mice. The chronologically measured HAIgG contents in the liver, spleen and glomeruli increased in the dextran-treated animals. Immunofluorescent technique also demonstrated an increase in HAIgG and host C3 in the glomeruli. The glomerular hypercellularity was not demonstrated morphologically. Assuming that the function of the hepatic Kupffer cells principally affect the clearance rate of the foreign macromolecules from the blood, it is likely that the dextran administration impaired the function of these cells to digest the phagocytosed HAIgG, and that consequently the injected HAIgG stayed longer in the blood. Thus, increased supply of the HAIgG from the blood and/or the dextran-induced reduction of digesting function of the mesangial cell per se might have led to the accumulation of glomerular HAIgG. We suggest, therefore, that even small amount of immune complex may induce immune complex-mediated glomerular lesion, when associated with long lasting functional impairment of the Kupffer cells.

Intraglomerular distribution of fibronectin in primary glomerular diseases.

The intraglomerular distribution of fibronectin was examined in 152 renal biopsy specimens with the direct immunofluorescence technique. Fibronectin was detected only in the mesangial area in normal glomeruli, while in proliferative glomerulonephritis, it extended from the mesangium to the pericapillary regions proportionately with the proliferative changes. In membranoproliferative glomerulonephritis, fibronectin globally extended in the glomeruli. Also in membranous nephropathy, fibronectin was observed in the capillary walls despite an absence of proliferation. Electron microscopic examinations revealed that the glomeruli with the pericapillary distribution of fibronectin were mostly found in stage II or III, as determined by WHO criteria. The double immunofluorescence technique demonstrated that in membranous nephropathy fibronectin was on the inner side of the glomerular basement membrane, and that in membranoproliferative glomerulonephritis fibronectin extended concomitantly with the expansion of mesangial matrix. These findings suggest that intraglomerular fibronectin is localized only in the mesangial area in normal glomeruli, and that the pericapillary distribution of fibronectin occurs not only by mesangial proliferation but also by additional unknown mechanisms.

Influence of alternate-day prednisolone administration on urinary electrolyte excretion.

The administration of 30 mg of prednisolone on an alternate-day schedule produced an increase in sodium and potassium excretion, followed by a compensatory decline on the succeeding nontreatment day. Water and electrolyte excretion in patients receiving a daily steroid treatment was intermediate between that on the treatment and nontreatment days in the patients on an alternate-day schedule. Fractional sodium and potassium excretion was significantly lower on the nontreatment days as compared to the treatment days. Body weight loss, elevated serum albumin, slightly reduced serum potassium and significantly decreased cortisol concentrations were found in the morning on the nontreatment days, while serum sodium and plasma aldosterone concentrations, and the creatinine clearance rate did not change significantly. Replacement of steroid-induced body fluid loss lessened a decline in water and electrolyte excretion on the nontreatment days. Reduced water and electrolyte excretion on the nontreatment days might have been associated with decreased extracellular fluid volume, suppressed cortisol secretion, or both.

[A study on the levels of cefoperazone in urological organs].

The concentration of Cefoperazone (CPZ) in urological organs and serum was measured after the intravenous administration of 2 g of CPZ. Serum levels on healthy adults attained a maximal value of 122.1 +/- 4.2 micrograms/ml at 30 minutes. Serum levels of patients operated under general anesthesia attained a maximal value of 159.0 +/- 13.9 micrograms/ml at 30 minutes. Serum concentration on operated cases continued to be 1.3-2.2 fold higher than that of the healthy adults. Prostate tissue levels attained a maximal value of 80.9 +/- 3.6 micrograms/g at 30 minutes, vesical tissue levels were 52.1 +/- 1.3 microgram/g at 60 minutes and renal tissue were 94.0 +/- 14.5 micrograms/g at 90 minutes. Judging from the minimal inhibitory concentration of CPZ, CPZ seemed to be clinically useful in the treatment of urological infections.
