One hundred and sixty-seven patients with newly diagnosed, previously untreated prostatic adenocarcinoma, during the period between July, 1971 and March, 1934, were studied. The histopathologic specimens of the prostate were classified by the Japanese General Rules of Prostatic Cancer (JGRPC) and by Gleason's classification. The number of patients with well, moderately and poorly differentiated prostatic carcinomas according to the JGRPC classification was 42, 74 and 51 respectively, and the number of the patients with Gleason's pattern 1, 2, 3, 4, and 5 was 24, 20, 32, 43 and 48, respectively. The number of patients with Gleason's score (primary plus secondary pattern) 2 to 4, 5 to 7 and 8 to 10 groups was 40, 76 and 51 respectively. There was no significant difference in survival rate between well and moderately differentiated groups, but the survival rate of the group with poorly differentiated tumor was significantly lower than that of the moderately differentiated (p less than 0.01) and well differentiated (p less than 0.001) groups. There was no significant difference in 10-year survival rate between the score 2-4 and score 5-8 groups, but the score 9-10 group had a significantly lower survival rate than the score 5-8 group (p less than 0.01) and score 2-4 (p less than 0.005) group. JGRPC and Gleason's classification are thought to be equally useful in estimating prognosis of prostatic cancer, but the former is more concise and easily applicable than the latter.
Cultured rat glomerular mesangial cells had relatively simple glycolipid composition which comprised mainly monoglycosylceramide, diglycosylceramide, isoglobotetraosylceramide and GM3. Beside these glycolipids, sulfatide, polar gangliosides and an unknown neutral glycolipid were detected in whole kidney and tubules. Polyclonal antibody against the cultured rat mesangial cells, which induced glomerular injury in vivo, bound specifically with isoglobotetraosylceramide.

The optimal frequency characteristic of Butterworth-Wiener filters (BWF) for improving the image quality of 201Tl myocardial SPECT was determined by a phantom experiment. Thirty two projection images of the phantom containing 11.1 MBq of 201Tl with 4 different cold lesions were collected during a 180 degree arc of a gamma camera. A set of the projection images were processed with each of 27 different BWFs, and SPECT images were reconstructed by Shepp-Logan filtered backprojection. The SPECT images were evaluated for their diagnostic ability to visibly detect the cold lesions by four nuclear medicine physicians. The lesion contrasts were used as an adjunctive tool to determine the optimum filter. The optimal combination of the parameters determining BWF characteristics (for the data of about 100 count/pixel at the myocardium) is: 1) cutoff of 0.25/pixel, 2) FWHM of 3 pixels, 3) noise/signal ratio of 0.02. FWHM and noise/signal ratio affected lesion contrast much less than cutoff frequency. Clinical myocardial SPECT images processed with the optimal BWF showed less noise and sharper delineation of the myocardium.
The neural inhibition of the gastrointestinal (GI) smooth muscle motility was studied by means of electrical transmural nerve stimulation (ETNS) in guinea-pigs. In untreated muscle strips, ETNS induced four types of response, consisting of three basic components, i.e., contraction and relaxation during ETNS, and after-contraction. Following atropinization, all the responses changed to only one type, i.e., relaxation during ETNS followed by after-contraction. The relaxation of the muscle strips induced by ETNS after atropinization was not significantly reduced under superimposing guanethidine treatment in any site of the GI tract. The maximal relaxation of the muscle strips induced by ETNS after atropine and guanethidine treatments was not uniform throughout the GI tract. The relaxation of the gastric body and colon was greater than that of the jejunum and ileum. The extent of the relaxation was significantly different even in the colon. Theophylline and phentolamine did not reduce the ETNS-induced relaxation following atropine and guanethidine treatments in the distal colon and taenia coli. The findings suggest the following: the non-adrenergic inhibitory (NAI) nerve is the main postganglionic nerve to inhibit the GI smooth muscle motility; and the role of adrenergic nerve is of little importance in respect to postganglionic inhibition. The innervation of the NAI nerve seems not to be uniform throughout the GI tract. Theophylline and phentolamine are not the specific antagonists of the NAI nerve.
A case of left anterior and middle cerebral arterial occlusion with angiographic features similar to Moyamoya disease was reported. IMP SPECT of the patient revealed the success of bypass surgery clearly. The patient complained of transient right hemiparesis with aphasia 4 times. The cerebral arteriography disclosed occlusions of left anterior and middle cerebral arteries at their proximal portions. Right internal carotid and its branches were normal. I-123 IMP SPECT study showed hypoperfusion in left temporal lobe, basal ganglia with incomplete reperfusion on the delayed (4 hours after injection) SPECT images. After the superficial temporal-middle cerebral artery anastomosis, I-123 IMP SPECT showed improvement of the brain blood flow. I-123 IMP SPECT was very useful in detecting the ischemic areas and evaluating the revascularizing surgery in this case.

A 52-year-old woman had a newly recognized disorder of familial hypoceruloplasminemia, blepharospasm, retinal degeneration, and high-density areas in CT of the basal ganglia and liver scan. Immunofixation electrophoresis disclosed apoceruloplasmin deficiency. Kinetic, x-ray analysis, and histochemical study showed accumulation of iron in liver and brain, but not of copper. Intestinal copper absorption was reduced, but liver uptake was increased. Ceruloplasmin is involved in iron metabolism, and the findings suggest that hypoceruloplasminemia due to lack of apoceruloplasmin was causally linked to the iron deposition in basal ganglia and other organs, leading to blepharospasm and retinal degeneration.
We defined duodenal ulcers (DUs) not healed within 3 months with H2-antagonists as "intractable", because the recurrence rate of these DUs is higher than that of DUs healed within the same periods, and etiological differences are suggested to exist among these two groups. To verify the pathophysiological differences, gastric analysis including modified sham-feeding (MSF) were performed in 12 intractable, 10 tractable DUs and 5 healthy controls. By MSF stimulation, acid output increased in all subjects and the mean acid output of MSF amounted to about 60% of the tetragastrin maximum. Mean acid output of intractable DU cases was significantly higher than the controls in any stimulatory state and was different only in MSF with tractable DU. No significant changes of serum gastrin were recognized during MSF and both serum pepsinogen I and II are higher in DU cases than the control in basal state, but the two DU groups show no significant difference or increase by MSF or gastrin stimulation. These data suggest that vagal activity of intractable DUs is higher than not only healthy subjects, but tractable DUs and participation of gastrin in the increase of acid output by MSF might be denied.

Two cases of bilateral renal cell carcinoma are reported. The first case is of a 54-year-old male who visited our hospital on March 7, 1984 complaining of colicky pain in his left flank. Intravenous urography showed a large mass in the upper pole of the left kidney causing deformity and dislocation of the upper calyces. There were no remarkable findings in the right kidney. Abdominal CT-scan and arteriography revealed a round and hypervascular tumor with soft tissue mass density in the upper pole of both kidneys. Nephrectomy of the left and segmental resection of the upper pole of the right kidney were performed on April 11, 1984. The second case is a 47-year-old male who visited our hospital complaining of total gross hematuria. Intravenous urography showed a large soft tissue mass at the lower pole of the left kidney. Abdominal CT-scan revealed a large tumor mass associated with central necrosis in the left kidney and also a small tumor lesion at the center of the contralateral kidney. Bilateral nephrectomy was performed on December 19, 1984, and the patient was referred to hemodialysis treatment. The cut section of the nephrectomized right kidney specimen revealed multiple minor accessory tumors. Both patients have been doing well without any evidence of recurrence or metastasis.

A 44-year-old male patient visited our clinic complaining of back pain since January, 1984 and feeling of weakness at the lower extremities, sensory and vesicorectal disturbance since March of the same year. Hypesthesia and hypalgesia were observed at Th10 or below. The chest X-ray findings revealed an egg-sized tumor mass at the level of Th8. Metrizamide CT also demonstrated thoracic epidural mass. The characteristic findings of hemangioma showing delayed appearance of the tumor stain were also observed by angiography. The tumor was then confirmed as dumbbell-shaped hemangioma growing in the pleural cavity and the spinal canal. At the first surgical operation, total resection of the tumor in the spinal canal was performed by the postero-lateral approach. Although the second surgical intervention of total removal of the tumor in the pleural cavity had also been scheduled, the surgery was postponed because the patient had to return to work. The 2-year follow-up study revealed no recurrence of the tumor. And the tumor was found slightly diminished on chest X-ray. At present, the patient is still under observation. In this report, the authors discuss the therapeutic methods for thoracovertebral hemangioma including our case and reviewed in the literature related to this type of tumor.
