Contribution to the epidemiology of multiple sclerosis in Poland.

An epidemiological survey of multiple sclerosis was carried out in western Poland. Uneven local distribution of MS was found within 198 communes making up the territory. The disease was less frequent in 1981 than in 1965. A marked increase of population in age classes under 20 during the last decade may account for the observed differences in the prevalence rate. In the town of Gniezno, a new MS cluster was discovered.

Therapeutic trials of multiple sclerosis and intrathecal IgG production.

In our clinical trial we have compared the effect of high doses of prednisone, ACTH alone or ACTH and cyclophosphamide on plasma and CSF albumin and IgG levels. We have found that the treatment with high doses of prednisone is more effective in the suppression of CNS IgG synthesis than ACTH, or cyclophosphamide. However the significance of this immunological phenomenon in the pathogenesis of MS is a very complex problem, since we have not observed any correlation between the depression of the intrathecal IgG synthesis and clinical results of MS treatment.

The effect of fatty acid deficient diet on the developing optic nerve of the rat.

Axon-myelin interrelationship was analysed morphometrically in developing optic nerves of Wistar rats maintained during the maturation on normal, lipid deficient and lipid enriched diets. The results obtained lead to the following conclusions: Myelinogenesis in the optic nerve of Wistar rats is severely retarded in animals maintained on a fatty acid deficient diet during the last period of intrauterine life and during their whole postnatal life. The axon diameter of the optic nerves in animals fed on a fatty acid deficient diet is normal in the first weeks of postnatal development, but it does not reach the values of normal ones in the young adult animals. The dietary deficit of fatty acids caused qualitative changes in the myelin-axon interrelationship in the course of myelinogenesis as indicated by an altered distribution of the ratios in the number of myelin lamellae to the axon diameter (the regression equation has changed from a linear to a parabolic one). Comparison of the morphometric and chemical studies of the developing myelin in the fatty acid deficiency experiments testify that the numerically normal myelin in young adult animals is still deficient in basic lipid classes.

The clustering of multiple sclerosis in various administrative subunits of western Poland.

A survey of multiple sclerosis was made in the western part of Poland, with special attention being paid to the distribution of the disease in various administrative subunits. The prevalence of MS for the 35 counties (mean 45.08 per 100,000 for definite and possible cases taken together) revealed significant differences between particular regions, with clusters of high prevalence rates occurring in two towns (Leszno, 130,97; Gniezno, 122.83). Local uneven distribution of multiple sclerosis was also found within 198 communes covering the territory of the 35 counties. There were four clusters in eight neighbouring towns and communes with a high prevalence ranging from 74.37 to 141.54, and several with a very low prevalence.

Epidemiology of multiple sclerosis in western Poland--a comparison between prevalence rates in 1965 and 1981.

An epidemiological survey of multiple sclerosis was carried out in the western part of Poland. The estimated prevalence rate on December 31, 1981 was compared with that established on the same territory on January 1, 1965. The disease was less frequent in 1981. The prevalence rate for definite cases was only 42.87 versus 51.19. A marked increase of population in the younger age groups (those under 20) during the last decade may account for the observed differences in the prevalence rate. In the town of Gniezno, a new MS cluster with a prevalence rate of 118.10 per 100,000 population was discovered.

On the fine structure of the distribution of primary nervous system neoplasms in Western Poland.

The incidence rates of primary nervous system neoplasms in 5 districts (large administrative units) of Western Poland with a relatively homogeneous health care system but some contrasts in economic activity, as well as in 224 communes (smallest administrative units) located within these 5 districts, with a total population of 3,097,000 were estimated on the appointed day (31.12.79). Testing the hypothesis, assuming a homogeneous distribution of nervous system neoplasms over the region under investigation, that the incidence rates were distributed as a Poisson variable has revealed a focal distribution of these tumours; in 15 communes the frequency was higher than 9.0 and deviated from the remainder in the 5% right tail of the distribution. The main conclusion from the presented results was that tumours of the nervous system, of which 90% were intracranially located, may occur in clusters or foci, and this could prove to be of clinical as well as statistical import.

[Epileptic seizures in young patients with hypodense lesions in the CT image].

Progress in neurodiagnostic medical technology, and in computer tomography in particular, has raised fresh problems in connection with epilepsy. Among our patients we have observed three case of epilepsy, in whom the CT image unexpectedly revealed hypodense lesions of various sizes in the cerebrum. The three are young patients aged 20 to 25 years with negative neurological findings. The above changes in the CT image are extremely difficult to interpret. It is not even know whether the lesions must be considered the cause or a consequence of the epileptic seizures or whether the pathomorphological processes are basically identical or completely different in all three patients.
