The neurosurgical aspects of neurofibromatosis 2: diagnosis and management.

NF-2 is an extremely rare form of neurofibromatosis (NF) characterized by central system (CNS) neural crest-derived tumors and frequently cafe au lait spots (CLS). The purpose of this study was to report the clinical and imaging findings of seven patients with this disorder and to stress that value of surgical treatment in its management. Seven patients between 8 and 32 years of age who had NF-2 were included in the study. Clinical charts, surgical and pathological findings, and imaging studies were reviewed retrospectively. Patients were followed up for to 142 months. Clinical evaluation and neuroimaging studies detected the clinical criteria of NF-2 in all patients. Two deaths occurred after surgical intervention in our series. None of the patients suffered from recurrent tumor following surgery. Our results show that NF-2 is an uncommon entity which has a good prognosis after surgical intervention, in spite of the presence of multiple cranial and/or spinal lesions.

The treatment of cerebral oligodendrogliomas with particular reference to features indicating malignancy: report of seventy-seven cases.

Oligodendrogliomas are relatively rare tumors that arise from the oligodendrocyte or its precursors. The role of postoperative radiotherapy (RT) in these tumors still remains unclear. Data concerning a study on 77 histologically verified cases of oligodendrogliomas of the brain among a total number of 1884 cases of an intracranial glioma treated at the Hacettepe Medical Centre between 1964 and 1991 were reviewed and analyzed (6.5%). One patient died in the early postoperative period and 8 patients in pediatric age group with an aggressive from of the tumor died within 6 months of treatment. The results suggest that oligodendrogliomas which arise in childhood primarily in the intraventricular region should be considered potentially more malignant than other lesions of this type. Because of this, we believe that postoperative radiotherapy is necessary to prevent the recurrences.

Cerebrovascular occlusive disease: hydatidosis.

We reviewed six patients with cerebral hydatid embolism from the heart. Although hydatid disease is becoming less common in the world as a whole, it should be considered in the differential diagnosis of embolic stroke in children, especially in the infested areas where hydatidosis is endemic.

Asymmetry of sympathetic activity in a rat model of Parkinson's disease induced by 6-hydroxydopamine: haemodynamic, electrocardiographic and biochemical changes.

We studied the effects of experimental hemiparkinsonism upon sympathetic function in rat. The rats were divided into three groups: a group given intact control, one given lesioning with 6-hydroxydopamine (6-OHDA), and one given sham operation. One day after apomorphine testing following lesioning of the substantia nigra (SN) with 6-OHDA, heart rate (HR), mean arterial blood pressure (MAP), and electrocardiogram (ECG) were monitored. Plasma norepinephrine (NE), epinephrine (E), and dopamine (DA) levels were measured. Thereafter, immunohistochemical examination was performed to detect the extent of 6-OHDA lesions, using the avidinbiotinylated peroxidase complex (ABC) method. There was no difference in the total number of tyrosine hydroxylase (TH)-positive cells and rotation responses between the right- and left-sided 6-OHDA-treated groups. On the other hand, injury of rats with unilateral 6-OHDA resulted in haemodynamic, electrocardiographic, and biochemical changes. A significant difference was found between the right-sided 6-OHDA-treated rats and the left-sided treated ones. The MAP increased in the group given left 6-OHDA treatment and to lesser extent in the sham-operated group. In contrast, MAP did not increase in the group given right 6-OHDA treatment and was significantly lower than values in both the intact control rats and the sham-treated rats. Also, only the group given right 6-OHDA injury showed a fall in the value of HR. The plasma NE level was significantly decreased in the group given right 6-OHDA treatment compared with all other groups (P < 0.005). Our results indicate that right-sided lesioning of the nigrostriatal DA pathway in the central nervous system (CNS) has greater sympathetic consequences than left-sided ones. These results also suggest that there is a differential effect of right-sided SN lesions on sympathetic cardiac innervation. The mechanism behind the confronting impairment of autonomic nervous system (ANS) could in this experiment be attributable to an asymmetric representation of sympathetic function in the brain. However, further studies will be needed before final conclusions can be made.

Vertical displacement of stereotaxic target in the cat brain after burr hole production of the skull.

In order to observe the postoperative vertical displacement caused by various open functional stereotaxic procedures in the coordinate of brain target, we produced a similar surgical procedure in 14 cats. On preoperative computed tomography (CT) scans, certain well defined brain structures (the cortical surface of the brain and the floor of the third ventricle) were chosen as anatomical landmarks and their relationship to the skull were determined with the cat in a prone position. Thereafter, a burr hole was opened on the right side of the skull of each animal and a small dural incision was made. All animals were re-examined with CT scanning using the same technique and the distances to the skull were determined. There was a cortical descent in postoperative coronal CT images (range 1.2-3.6, mean 2.1 mm). On the other hand, the vertical coordinate of the floor of the third ventricle was found to be lower than it had been preoperatively, with an average of 0.6 mm lower (range -1.2(-)+0.3 mm). These results indicate that there was a good correlation between the degree of cortical descent and displacement of the vertical coordinate of the floor of the third ventricle (r = 0.70 and r = 0.69, respectively; P < 0.01). Our results also suggest that thalamic targets can be calculated by compensatory adjustment of the vertical coordinate by an additional distance of about half of the vertical descent. However, further experimental and clinical studies will be needed to determine the validity of this result in human beings.

Changes in end-plate vascularity after Nd: YAG laser application to the guinea pig intervertebral disc.

The authors previously have reported the effects of neodymiumyttrium-aluminum-garnet (Nd: YAG) laser on the cartilage end-plates (CEPs) in an animal model. However, there is no experimental study in the literature which specifically addresses the effects of the laser on CEP vascularity. The aim of our study is to investigate the vascular response of the CEP to Nd: YAG laser. In this study, sixteen guinea pigs were used. Experimental intervertebral disc (IVD) degeneration was induced in 12 animals by a ventral disc incision to study the effects of Nd: YAG laser on the CEP of the degenerated IVD from the aspects of pathology and radiology. At 2 months after the experimental annular lesion, the CEP vascular channel counts had increased significantly (p = 0.00) in the cranial CEP to 104%, and to 99% in the caudal CEP. In the early period after laser irradiation the vessel counts from both CEPs decreased due to the extensive damage of Nd: YAG laser on disc tissue (p = 0.00). Accordingly, the value of CEP index and MR signal intensity of the CEPs (cranial and caudal) had decreased one month after laser application. Thereafter, both the CEP vascular channel counts and the MR signal intensity remained relatively constant throughout the course of the experiment, but the value of CEP index had increased to 68% of the normal IVD. From the results of present study, it is evident that laser application had a significant negative influence on the CEP vascularization of the degenerated IVD.

A prospective study of persistent back pain after pregnancy.

OBJECTIVES: To determine the prevalence of back pain after delivery and its relationship to individual factors. STUDY DESIGN: A cohort of 88 pregnant women, aged 14-46 years, who had suffered from back pain during pregnancy, and delivered at Aydin Maternity Hospital was selected. They had been followed up through pregnancy, and 6 months post partum filled out a questionnaire. The data were analyzed statistically. RESULTS: Follow-up showed that back pain at the time of delivery and 6 months post partum was reported by 59.1% and 43.2% of the women, respectively. The difference in prevalence of back pain between young women and older ones was statistically significant (P=0.000). The number of previous pregnancies increased the risk of back pain (P=0.000), but there was no difference in prevalence of back pain between women with heavy work and without heavy work before pregnancy (P=0.310). Furthermore, women with a history of back pain before pregnancy were found to experience more intense pain at 6 months post partum compared to those without a history of back pain before pregnancy (2.1+/-1.0 and 0.4+/-0.4. respectively. P=0.000). CONCLUSION: These results indicate that pregnant women with a previous history of back pain had a higher prevalence of back pain, especially in young multigravid patients.

A fatal acute subdural hematoma occurring after evacuation of "contralateral" chronic subdural hematoma.

Although chronic subdural hematomas (SDHs) are bilateral in about 20% of the cases, an acute SDH in one side associated with chronic SDH in "contralateral" side is an extremely rare and devastating condition. In this report, a case of "contralateral" acute SDH occurring after evacuation of chronic SDH in one side is presented. The clinical history, complaints, neurological and neuroradiological findings, and pathophysiological mechanisms of this uncommon entity are discussed and related literature is reviewed.

Role of surgical treatment in 70 patients with vertebral metastasis causing cord or root compression.

Spinal metastasis plays an important role in the survival rate and general condition of cancer patients. In this paper, we present 70 patients with metastatic spinal tumors, diagnosed and surgically treated in the Departments of Neurosurgery and Orthopedics of Ankara Numune State Hospital between 1984 and 1993. Our clinical observations suggest that the survival rate is influenced by the type of the primary lesion, pathology, and the patient's preoperative physical and neurological status.

Multiple meningiomas of the central nervous system without the stigmata of neurofibromatosis. Clinical and therapeutic study.

Multiple meningiomas are relatively rare tumors without known neurofibromatosis. In this paper, such eight cases of multiple meningiomas as described by CUSHING and EISENHARDT are presented. Certain aspects of diagnosis and surgical management of this rare condition are discussed with particular emphasis on the importance of the distinction among multiple meningioma, meningiomatosis, or recurrences of these tumors. Fortunately, many of these patients tolerate multiple surgical interventions well, although the removal of these tumors in critical areas is a difficult problem. Thus, we think that it is important to examine and supervise all patients who have had a meningioma for a possible occurrence of a second meningioma.

Extensive damage to the end-plates as a complication of laser discectomy. An experimental study using an animal model.

To date, there have been no reports of experiments demonstrating the effects of neodymium:YAG laser (Nd:YAG laser) on the vertebral end-plates. In this study the effect of Nd:YAG laser on end-plates was examined in 32 guinea pigs which were randomly divided into two groups. The first group was the control group, the second one the Nd:YAG laser group. All animals had experimental disc degeneration at three levels. Re-exploration was performed two months after the surgical ventral disc herniation. In the second group the same procedure was performed but at the end of the re-exploration, Nd:YAG laser irradiation of the degenerated discs was done. The wounds in both groups were closed again. Two months later all animals were sacrificed for histological and biochemical analysis. The cervical spine was excised en bloc and the overlying muscles were removed. Determination of hydroxyproline was done colorimetrically in the specimens harvested from each of these groups. Light microscopy was undertaken to evaluate the extent of morphological changes. The differences observed between the two groups were statistically significant (p < 0.05). From the results of this study, there is the question whether the Nd:YAG laser is a useful instrument in neurosurgery. Therefore, it remains to be proven whether or not this is of real benefit in the treatment of patients with degenerated disc disease.
