A comparison of rapid-healing prosthetic arterial grafts and autogenous veins.
The patency rates and healing characteristics during a nine-month period of internal velour Dacron grafts, external velour Dacron grafts, and Sparks-Mandril grown grafts were compared with autogenous jugular veins when used to bypass short segments of the canine femoral artery. Graft thrombosis occurred in three of 11 external velour grafts, in six of 11 internal velour grafts, and in three of 11 Sparks-Mandril grafts, None of 33 autogenous veins occluded. Graft thrombosis was not correlated with the degree of graft healing but rather with a hyperplastic proliferative intimal lesion observed at the proximal anastomosis of the occluded grafts.
The use of the Hickman catheter in the management of patients with leukaemia and other malignancies.
The use of the Hickman right atrial catheter in 25 patients with leukaemia and other malignancies is reported, and it is concluded that with careful selection of patients, the benefits of this semi-permanent catheter outweigh the risks of infection. The catheter may be useful in the intensive care of children or adults with non-malignant conditions.

A man of 38 years of age was found to have a type I endocrine polyadenomatosis in 1969. He was operated upon for removal of tumor of the islets of Langerhans with lymph gland metastases, and the head of the pancreas was removed. This was followed at a later date by ablation of two parathyroid adenomas. A clinically silent adenoma of the left adrenal was not removed and a silent and enclosed pituitary tumore was discovered. There were no clinical or hormonal signs of progression of the pancreatic tumor ten years later, but hypertension and behavioural disorders had developed. Catecholamine levels were normal. Selective blood aldosterone levels were just within significant values. A massive increase in prolactin secretion (more than 100 times the normal) was noted. This could be reduced by bromocriptine, and the possible role of prolactin in the behavioural disorders present is discussed.

Confronted with the difficult problems presented in acute appendicular peritonitis, whether generalized or multiple focused, the authors used the "lavage irrigation" method in ten cases. They analysed the local and general conditions that led them to employ this method, in light of the evolution of this technic which they had observed in their experiences whit it. They have attempted to elucidate, using information previously available in the literature, the indications, the steps, and the eventual complications of the "lavage irrigation" of the peritonemum in the child.

A 25-year-old man with familial pyruvate kinase (PK) deficiency who have been given only 6 transfusions during his lifetime died of cardiac failure from hemochromatosis (HC). This previously unreported association of PK and HC deficiency does not appear to be fortuitous but to be related to possible early hemolysis and intramedullary destruction, features of ineffective erythropoiesis. The role of splenectomy in the progression of the HC could, as in some thalassemias, be an unfavourable factor. Serum iron levels should therefore be measured during the course of AHPK.

Hearing in military personnel.
Impaired hearing is an occupational hazard prevalent not only in industry but also in the armed forces. A survey to establish its prevalence in the SAF land forces was conducted. A sample of 2,564 subjects obtained through multistage proportional sampling was studied of whom 1,521 "normal" subjects were finally analysed. 403 subjects i.e. 26.5%, showed evidence of impaired hearing. The highest incidence was in the Artillery. The frequency most commonly affected was 6,000 Hz and the commonest presenting symptom was tinnitus. A Hearing Conservation Programme is described. Comparisons with the situations in Singapore's industries are made.
False positive results in V D R L slide test following smallpox immunization.
A study is conducted on the incidence of false positive results in the serological test for syphilis in 300 military recruits following smallpox immunization. It is found that 3.7% of individuals developed false reactivity in the V D R L Slide test. The false reactivity appeared two to four weeks following immunization, and disappeared from two to eight weeks later. All reactive sera were negative in the FTA-ABS test.

The clinical significance of gallstones and their radiological investigation.
The symptoms of 122 patients with gallstones were correlated with the radiological findings. No specific indigestion was present which could be termed 'flatulent dyspepsia'. Sensitivity to fatty foods occurred in 69 per cent, heartburn in 42 per cent, regurgitation of of acidtasting or bitter fluid to the mouth in 31 per cent and increased passage of flatus from the stomach upwards in 38 per cent. If the gallbladder concentrated contrast medium or an oral cholecystogram but did not contract after a fatty meal, the patients suffered less heartburn than if the gall bladder functioned normally. However, since surgeons rarely perform a cholecystectomy for flatulent dyspepsia alone, knowlege of gallbladder function may be unnecessary.
